Single incision laparoscopic gastric transposition

Introduction: Single incision laparoscopic surgery (SILS) is widely performed in adults but its use in pediatric population is generally limited to appendectomy, cholecystectomy and intervention to ovary. We present the first case of gastric transposition that was performed with SILS technique.

Case: A five year old boy suffering from severe dysphagia due to ingestion of an industrial type of cleaner was under esophageal dilatation program for more than 12 months. Long-segment stricture did not respond to dilatations with 3 weeks interval, repeated sessions of mitomycin application and strict anti-reflux treatment. Esophageal replacement was indicated upon radiologically approved severe esophageal stricture necessitating dilatations with mild to moderate bleeding. Gastric transposition was planned. Stomach was mobilized, gastroesophageal junction was divided and retrosternal dissection up to cervical region was performed. Via cervical incision, proximal esophagus was prepared, stomach was pulled up and esophagogastric anastomosis was performed. Operation was terminated following laparoscopic bilateral gastric fixation. Operative time was 4 hours. There was no complication except right pneumothorax that necessitated intraoperative tube thoracostomy. Patient was fed orally on postoperative 5th day and was discharged without problem.

Conclusion: Gastrik transposition with SILS technique may be performed successfully in children.

Tek insizyonlu laparoskopik gastrik transpozisyon

Giriş: Erişkinlerde yaygın olarak uygulanan tek insizyonlu laparoskopik cerrahinin (TİLC) kullanım alanı çocuklarda genellikle apendektomi, kolesistektomi ve over girişimleri ile sınırlı kalmıştır. Bu çalışmada TİLC tekniği ile uygulanan ilk gastrik transpozisyon olgusunun sunulması amaçlanmıştır.

Olgu: Endüstriyel tipte temizlik maddesi içimine bağlı uzun segment özofagus darlığı gelişen 5 yaşındaki erkek olguya 13 ay süreyle 3 haftalık aralarla özofagus dilatasyonu uygulandı. Sıkı anti-reflü tedavi ve tekrarlanan mitomisin uygulamalarına rağmen dilatasyon araları uzatılamayan olguya gastrik transpozisyon planlandı. TİLC tekniği ile mide mobilize edilip gastroözofageal bileşke kesildi ve retrosternal tünel hazırlandı. Servikal insizyon ile proksimal özofagus hazırlanarak mide boyuna çekildi ve özofagogastrik anastomoz uygulandı. Laparoskopik bilateral gastrik fiksasyon eklenerek operasyon tamamlandı. Ameliyat süresi 4 saatti. İntraoperatif tüp torakostomi gerektiren sağ pnömotoraks dışında ameliyat sırasında ve sonrasında komplikasyon olmadı. Beşinci gün ağızdan beslenen olgu sorunsuz taburcu edildi.

Sonuç: Çocuklarda gastrik transpozisyon TİLC tekniği ile başarıyla uygulanabilir.
