Congenital short esophagus with intrathoracic stomach

Introduction and Aim: Presentation of a quite rare foregut malformation with documentation of radiologic and endoscopic features is aimed. The described anomaly has also been called as “Type 4 Hiatal Hernia” or “Upside-Down Stomach”.

Case Presentation: A-16-month old girl presented because of vomiting. She was 6.3 kg and 73 cm (both <-3SD) with a syndromic face and webbed neck. She was previously operated on for cervical spinal myelomeningocele as a newborn during which time radiological studies including an MRI scan were consistent with intrathoracic stomach and short esophagus. A barium swallow showed esophagogastric junction was located at the mid-thoracic level. The stomach and pylorus were in the thorax. The gastroduodenal junction was extending towards left side. An upper GI endoscopy revealed severe esophagitis with an esophageal length of 12 cm.  At surgery, whole stomach and part of mid-transverse colon were found within the thorax. There was no hernial sac. She underwent Toupet fundoplication after an extensive esophageal mediastinal mobilization till carina. The diaphragmatic defect was repaired primarily. A Stamm gastrostomy was added. A barium swallow on postoperative second week and third month showed intact fundoplication lying below diaphragm. The postoperative course is uneventful at the tenth postoperative month and she is primarily fed orally.

Conclusion: The medical nomenclature for the described rare anomaly is not uniform. A congenital short esophagus is thought to be caused by an antenatal hiatal herniation with resultant delay in the elongation of esophagus. Apart from clinical signs like vomiting and failure to thrive, the condition has the potential to cause gastric volvulus with resultant necrosis. An abdominal approach is preferable for surgery and it should be kept in mind that an esophageal lengthening procedure might be necessary.

 

 

İntratorasik mideli doğumsal kısa özofagus

Giriş ve Amaç: Son derece nadir bir öbarsak anomalisinin radyolojik ve endoskopik bulgularıyla birlikte sunumu amaçlandı. Tanımlanan anomali, “Tip 4 hiatal herni” veya “Başaşağı dönmüş mide” olarak da adlandırılmaktadır.

Olgu Sunumu: 16 aylık kız hasta, kusma yakınması ile getirildi. Ağırlığı 6,3 kg ve boyu 73 cm’di (ikisi de <-3SD). Sendromik yüz görünümü ve yele boynu vardı. Servikal miyelomeningosel nedeniyle, yenidoğan döneminde, dış merkezde ameliyat edilmişti. O tarihte yapılan MRG ve diğer radyolojik tetkiklerde kısa özofagus ve intratorasik mide görüntülenmişti. Üst GİS kontrastlı çalışmada özofagosgastrik bileşkenin mid-torasik seviyede olduğu görüldü. Mide ve pilor torakstaydı. Gastrodudodenal bileşke sola uzanıyordu. Üst GİS endoskopisinde ağır özofajit bulguları görüldü ve özofagus 12 cm olarak ölçüldü. Ameliyatta midenin tamamının ve mid-transvers kolonun toraksta olduğu görüldü. Herni kesesi yoktu. Karina seviyesine kadar özofagus diseksiyonu yapılarak Toupet fudoplikasyonu yapıldı. Diyafragmatik açıklık primer onarıldı. Stamm gastrostomi uygulandı. Ameliyat sonrası ikinci hafta ve üçüncü ayda yapılan kontrastlı çalışmada mide ve fundoplikasyonun diyafragmanın altında yer altığı ve intakt olduğu görüldü. Ameliyat sonrası dönem sorunsuz seyretti. Primer olarak ağızdan beslenmektedir.

Sonuç: Tıbbi literatürde tanımlanan anomaliye farklı isimler verilmiştir. Doğumsal kısa özofagusun, doğum öncesi gerçekleşen hiatal fıtıklaşma nedeniyle, özofagusun uzayamamasından kaynaklandığı düşünülmektedir. Kusma ve büyüme gelişme geriliği gibi klinik bulgular dışında, bu anomalinin, mide volvulusu ve nekrozuna yol açma potansiyeli vardır. Cerrahide abdominal yaklaşım tercih edilmelidir. Ameliyatta özofagus uzatıcı tekniklerin gerekebileceği akılda tutulmalıdır.
