PERINATAL MANAGEMENT OF THE GIANT SACROCOCCYGEAL TERATOMA

Background/Aim: Sacrococcygeal teratomas, which account for 70% of childhood teratomas, are the most common extragonadal tumors in neonates. If the fetal mass reachs giant dimensions, patient may die due to non-immun hydrops with high output cardiac insufficiency and vascular shunts. In this study,  evaluations of the perinatal approach and results in a case with a giant sacrococcygeal teratoma were aimed. 

Case Report: A female neonate with a giant sacrococcygeal mass was born as 41-week  and 6200 gr via C/S from a 23-year-old mother who had gestational hypertension and polyhydramnios; and baby was transfered to the Surgical Neonatal Intensive Care Unit. Prenatal echocardiography was normal, emergency surgical excision was planned to prevent  developing cardiac insufficiency due to cystic and lobulated giant mass with 64x46 cm extending from cocyx to perineum at saccrococcygeal region. Eight hours later post conception, umblical vein and radial artery catheterization were performed before the operation, and surgical excision were completed in 4-hour. 3270grams’ mass which adherent to the rectum was totally excised with presacral processes and coccyx without rupture. Intravenous fluid at 20ml/kg/hr and erythrocyte suspension at 15ml/kg/hr were administered during the operation. Patient was taken to surcical neonatal intensive care unit as entubated and mechanical ventilation was done. One unit of human albumin and erythrocyte suspension, fresh frozen plasma were given until stabilisation of the hemodynamy at first day.  stable in the peroperative  and early postoperative period. Oral feeding was started after the extubation on the 12th hours and discharged on the 8th days. Histopathological evaluation was reported as mature teratoma. The high preoperative serum levels of AFP (37786.8ng/ml),  B-HCG (30.8 mIU) ve CEA (3.25ng/ml) declined normal levels after total resection of giant saccrococcygeal teratoma

Conclusion: Giant sacrococcygeal teratomas usually associated with high mortality rate can be successfully treated with a good team work and perinatal management. Giant sacrococcygeal teratomas should be urgently operated in the first hours of the postnatal period without cardiac insufficiency and hemodynamic instability; and prognosis and survival affect in the positive direction by remowing of the mass totally with coccyx without rupture.

DEV SAKROKOKSİGEAL TERATOMDA PERİNATAL YÖNETİM

Giriş/Amaç: Çocukluk dönemi teratomlarının %70’ini oluşturan sakrokoksigeal teratomlar, yenidoğanlarda en sık görülen ekstragonadal tümörlerdir. İntrauterin dönemde teratomun dev boyutlara erişmesi, olgularda yüksek outputlu kardiak yetmezlik ve vasküler şantlara bağlı non-immun hidropsa yol açarak ölümle sonuçlanabilir. Bu çalışmada, dev sakrokoksigeal teratomlu olguda perinatal yaklaşım sürecinin ve sonuçların değerlendirilmesi amaçlandı.

Olgu Sunumu:  Prenatal Ultrasonda sakrokoksigeal kitle saptanan, gestasyonel hipertansiyon ve polihidramniyoz nedeniyle takipli, 23 yaş anneden 41 haftalık C/S ile 6200 gr doğan kız bebekte dev sakrokoksigeal kitle mevcuttu. Hasta doğumhaneden yenidoğan cerrahisi yoğun bakım ünitesine alındı. Prenatal ekokardiografisi normal bulunan hastanın sakrokoksigeal lokalizasyonda koksiksten perineye uzanan 64x46cm boyutlarında lobüle, kistik kıvamda dev kitlesi nedeniyle kardiyak yetersizlik gelişmeden acil olarak ameliyat edilmesine karar verildi. Postnatal sekizinci saatte ameliyata alınan hastada umbilikal ven ve radyal arter kateterizasyonu yapıldı. 3270gram ağırlığındaki kitle, rektuma çok yapışık olup presakral uzanımı koksiks ile birlikte total olarak rüptüre edilmeden çıkarıldı. Dört saat süren ameliyat boyunca 20ml/kg/saatte intravenöz sıvı, 15ml/kg/saatte eritrosit süspansiyonu verildi. Entübe halde yenidoğan cerrahisi yoğun bakım ünitesine alındı, mekanik ventilasyon yapıldı. Birer ünite Human Albümin, Eritrosit Süspansiyonu, Taze Donmuş Plazma verilen hastanın peropetif ve erken postoperatif hemodinamisi stabil seyretti. Postoperatif 12.saatte ekstübe edildi ve oral beslenmeye başlanıldı, 8.gün taburcu edildi. Histopatolojik değerlendirme, matür teratom olarak raporlandı. Preoperatif yüksek değerlerde saptanan AFP(37786.8ng/ml), B-HCG(30.8 mIU) ve CEA(3.25ng/ml) tümör belirteçlerinin üçüncü ayda normal değerlere gerilediği görüldü. Hastanın takiplerine devam edilmektedir.

Sonuç: Genellikle mortal seyreden dev sakrokoksigeal teratomlu olgular iyi bir ekip çalışması ve perinatal yönetim ile başarılı bir şekilde tedavi edilebilir. Dev sakrokoksigeal teratomlu olguların postnatal ilk saatler içerisinde kardiyak yetersizlik ve hemodinamik instabiliteye girmeden acil olarak opere edilmesi ve kitlenin rüptüre edilmeden koksiks ile birlikte çıkarılması, prognoz ve sağkalımı olumlu yönde etkilemektedir.
