EVALUATION OF PRIMARY SPONTANEOUS PNEUMOTHORAX CASES IN ADOLESCENTS

Aim: A retrospective study was performed to evaluate the clinical features and treatment alternatives of primary spontaneous pneumothorax (PSP) in adolescents.

Methods: The cases with PSP from January 2004 to June 2017 were evaluated in according to their age, sex, admission complaint, family history, using cigarette, clinical and radiological findings of PSP and treatment results.

Results: 9 cases with an average age of 14,7 years (range:10-17) were included in the study. Male-to-female ratio was 8:1. There was a history of PSP in family members in two of these cases (22%) and also in three cases (33%) had smoking history. The initial complaints were chest pain (n=7; 77%), acute onset of cough (n=1; 11%) and breathing difficulty (n=1; 11%). The oxygen saturations of all cases were within normal limits when they first admitted to the hospital. In six cases (66%) right sided, in two cases (22%) left sided and in one case (11%) bilateral pneumothorax were detected. Tube thoracostomy was performed in seven cases. Two cases were observed with supplemental oxygen. Tube thoracostomy was performed to both sides in one case. Preoperative chest computed tomographic (CT) scans were obtained in seven cases. Bullae in six cases (n=6) and subpleural blebs in one case (n=1) were demonstrated on CT scans. Diagnostic bronchoscopy was performed in two cases. Four cases were observed without any intervention. Pleurodesis with talc was performed in four cases with pneumothorax for longer than a week in follow-up. Four cases had recurrent PSP within one year. Pleurodesis was performed with talc in three of them and with autologous blood in one case. Blep excision was performed in a case, who had a pneumothorax despite pleurodesis.

Conclusion: All cases should be examined detaily with CT, because of the presence of bullae in the majority of adolescent with PSP. Tube thoracostomy and oxygen supplementation, are considred as initial and adequate treatment of PS in most cases. Prolonged air lekas can require pleurosdesis as the first line treatment.

ERGENLERDE PRİMER SPONTAN PNÖMOTORAKS OLGULARININ DEĞERLENDİRİLMESİ

Amaç: Ergenlerde primer spontan pnömotoraks (PSP) olgularının klinik özellikleri ve tedavi alternatiflerini değerlendirmek üzere geriye dönük bir çalışma yapılmıştır.

Gereç ve Yöntem: Ocak 2004 - Haziran 2017 yılları arasında PSP nedeniyle takip edilen olgular yaş, cinsiyet, başvuru şikayeti, aile öyküsü, sigara kullanımı, PSP’nin klinik ve radyolojik bulguları ile tedavi sonuçları bakımından incelenmiştir.

Bulgular: Çalışmaya yaş ortalaması 14.7 yıl (min:10 - maks:17) olan 9 olgu dahil edilmiştir. Erkek kız oranı 8:1’dir. İki olguda (%22) ailede PSP, üç olguda (%33) ise sigara kullanma öyküsü bulunmaktadır. Başvuru şikayetleri göğüs ağrısı (n=7; %77), ani başlayan öksürük (n=1, %11) ve solunum sıkıntısıdır (n=1, %11). Başvuru anında tüm olguların oksijen satürasyonları normal sınırlardadır. Altı olguda (%66) sağ, 2 olguda (%22) sol, bir olguda ise bilateral pnömotoraksa rastlanmıştır. Tüp torakostomi 7 olguda uygulanmış, 2 olgu oksijen desteği ile takip edilmiştir. Tüp torakostomi olgulardan birinde her iki tarafa da uygulanmıştır. Olguların 7’sinde bilgisayarlı tomografi ile yapılan değerlendirmede bül (n=6) ve subplevral bleblere (n=1) rastlanmıştır. İki olguda tanısal amaçlı bronkoskopi yapılmış, 4 olgu ise herhangi bir girişim yapılmaksızın takip edilmiştir. İzlemleri sırasında 1 haftadan uzun süren pnömotoraksı olan 4 olguda talk ile plöredezis yapılmıştır. 4 olgu tekrarlayan pnömotoraks nedeniyle 1 yıl içinde yeniden başvurmuştur. Bu olgulardan 3'üne talk ile plöredezis yapılırken birinde otolog kan kullanılmıştır. Plöredeze rağmen pnömotoraksı olan bir olguya torakatomi ile bül eksizyonu yapılmıştır.

Sonuç: Ergenlerde PSP olgularının önemli bir kısmında büllöz akciğer lezyonları olması nedeniyle tüm olgular BT ile detaylı incelenmelidir. Tüp torakostomi ve oksijen tedavisi çoğu olgunun sağaltımı için yeterli iken, hava kaçağı devam eden olgularda plöredezis ilk seçenek olarak kullanılabilir
