Pneumoperitoneum in the Newborn: Is Surgical Intervention Always Indicated?

The detection of the pneumoperitoneum of free air under the diaphragm on abdominal x-ray almost always indicates a perforation in the gastrointestinal system and requires urgent surgery. Intraabdominal free air due to intrathoracic free air in infants who do not require surgery and especially connected to the mechanical ventilator creates a difficult dilemma for operation decision and unfortunately it is not always possible to avoid laparotomy. In our study, two cases with intraabdominal free air were presented, in which the abdominal graphs were almost the same, while the abdomen was swollen and the ileal perforation was detected in the operation. On the other hand, In another case, the surgical problem was not encountered.

The Case 1:     

A 37-week female infant with 2600 gr born from 32-year old mother by normal vaginal delivery was admitted to the neonatal intensive care unit due to respiratory distress and abdominal distention. Free air was detected under diaphragm on the standing direct abdominal graph (SDAG) in the patient with moderate-bad general condition. The patient underwent the operation while she was one-day old. Perforation in the 10 cm segment was detected in the terminal ileum. This section was removed and ileostomy was opened. On the fourth postoperative day, the patient’s feeding was started and a 20-day follow-up was planned.Then, she was discharged.

The Case 2:

A 23-week female infant with a birth weight of 570 gr born from 31-year old mother by normal vaginal delivery, was connected to the mechanical ventilator in the neonatal intensive care unit due to developing respiratory distress. Two-sided chest tube was inserted to the four-day old newborn with bad general condition due to bilateral intrathoracic free air and intraabdominal free air. Despite the disappearance of air in the chest during the follow-up, abdominal distention and free air under diaphragm on SDAG continued in the same manner. Therefore the patient was undergone the operation, but any intraabdominal surgical pathology was not detected. The patient died at seventh hour after the operation.

In our first case, the classical approach was sufficient as expected, whereas in the second case, the baby became increasingly worsening in general condition and very low birth weight. In addition, in the second case, it was thought that if there was a problem within the abdomen, the baby would be lost. Therefore, this led to the decision for undergoing operation. Although medical treatment and close follow-up were recommended in these cases in the literature, half of the cases were diagnosed by unnecessary laparotomy. Similar aspects of both cases aim to draw attention to the difficulty of avoiding surgery despite all the experience and opportunities of Pediatric Surgery and Neonatology.

Yenidoğanlarda karın içi serbest hava: Her zaman ameliyat gerekir mi?

Karın grafisinde diyafragma altında serbest hava görülmesi hemen daima mide-barsak sisteminde bir delinmeyi gösterir ve acil ameliyat gerektirir. Ameliyat gerektirmeyen ve özellikle mekanik ventilatöre bağlı bebeklerde göğüs içi serbest havadan kaynaklanan karın içi serbest hava varlığı cerrahi karar yönünden çözümü güç bir ikilem yaratır ve ne yazık ki laparatomiden kaçınabilmek her zaman mümkün olmamaktadır. Çalışmamızda karın grafileri hemen hemen aynı olan, sert karın şişliği bulunan ve ameliyatta birinde ileum delinmesi saptanırken diğerinde herhangi bir cerrahi sorunla karşılaşılmayan iki karın içi serbest hava olgusu sunulmuştur.

Olgu 1:     

32 yaşındaki annenin 37 haftalık, 2600 gr, normal vajinal yolla doğan kız bebeği; solunum sıkıntısı ve karın şişliği nedeniyle yenidoğan yoğun bakım ünitesine yatırıldı. Genel durumu orta-kötü hastanın çekilen ADKG’de diyafragma altında serbest hava saptandı ve 1 günlükken ameliyata alındı. Terminal ileumda 10 cm’lik segmentte delinme belirlendi. Bu bölüm çıkarılıp ileostomi açıldı. Posoperatif 4. günde beslenmeye başlandı, 20 günlükken izlemi planlanarak taburcu edildi.

Olgu 2:                                                                          

31 yaşındaki annenin 23 haftalık, 570 gr, normal vajinal yolla doğan kız bebeği, gelişen solunum sıkıntısı nedeniyle yenidoğan yoğun bakım ünitesinde mekanik ventilatöre bağlandı. Genel durumu kötü bebeğe 4 günlükken gelişen iki taraflı göğüs içi serbest hava ve karın içi serbest hava nedeniyle iki taraflı göğüs tüpü takıldı. İzlemde göğüs içi havanın kaybolmasına karşın; karın şişliği ve ADKG’de diyafragma altı serbest hava aynı şekilde devam ettiğinden ameliyata alındı ancak karın içi cerrahi bir patoloji saptanmadı. Hasta ameliyat sonrası 7. saatte kaybedildi.

İlk olgumuzda klasik yaklaşım beklendiği gibi yeterli olmuş ama ikinci olgumuzda bebeğin giderek kötüleşen genel durumu ve çok düşük doğum ağırlığı; karın içi bir sorun varsa bebeğin kaybedileceği düşüncesiyle ameliyat kararı verilmesine yol açmıştır. Literatürde de bu olgularda medikal tedavi ve yakın izlem önerilmesine karşın olguların yarısına yine gereksiz laparatomi ile tanı konulmuştur. Her iki olgumuzun benzer yönleri, Çocuk Cerrahisi ve Neonatoloji’nin tüm deneyim ve olanaklarına rağmen ameliyattan kaçınabilmenin zorluğuna dikkat çekmek amacındadır.
