Comparison of the accounts of invoice of ultrasound guided hydrostatic reduction and open surgery in intussusception

Comparison of the accounts of invoice of ultrasound guided hydrostatic reduction  and open surgery in intussusception

AIM: In the treatment of intussusception in childhood, rather than open surgery, different treatment approaches are used in selected cases. Ultrasound guided hydrostatic reduction with saline is one of them. In this study, the accounts of invoice of ultrasound guided hydrostatic reduction with saline and open surgery are compared.

MATERIAL AND METHOD: We retrospectively reviewed the data of 48 patients who were treated with the diagnosis of intussusception  in pediatric surgery department between January 2011 and June 2017. Gender, age, lenght of hospitalization, treatment approach and account of invoice of the patients were reviewed. Non-operative approach was not performed in cases with overall poor condition or with peritonitis symptoms. The data was statistically analyzed by Mann Whitney U test.

RESULTS: 31 of the cases were male, 17 were female and the mean age was 48 months.  30 patients underwent open surgery and 18 patients had ultrasound guided hydrostatic reduction with saline. In cases who underwent open surgery, the mean lenght of hospitalization was 5.13 days and their account of invoice was 1094.60 TL, whereas patients who had ultrasound guided hydrostatic reduction with saline, the mean duration of hospitalization was 2.6 days and account of invoice was 841.30 TL.  When two treatment methods were compared, statistically significant difference was found in terms of invoice amount (p <0.05) and duration of hospitalization (p <0.01).

RESULT: In our study, in intussusception treatment, we found that ultrasound guided hydrostatic reduction with saline is more economically advantageous than the open surgery due to its lower account of invoice. Ultrasound guided hydrostatic reduction with saline is known as a simple, efficient, reliable treatment approach with low complication rate.  In addition, since it is an affordable treatment approach, it should be considered as the first treatment option in all cases unless there are any contraindications

İnvajinasyonda  açık cerrahi ile USG eşliğinde  hidrostatik  redüksiyonun  fatura tutarlarının  karşılaştırılması

AMAÇ:  Çocukluk çağında  invajinasyon tedavisinde  açık cerrahi dışında seçilmiş olgularda  farklı tedavi yöntemleri kullanılmaktadır. USG eşliğinde serum fizyolojik ile  hidrostatik redüksiyon bunlardan biridir. Bu çalışmada açık cerrahi ile USG eşliğinde serum fizyolojik  ile  hidrostatik redüksiyon fatura turarlarının karşılaştırılması yapılmıştır.

GEREÇ VE YÖNTEM:  Ocak 2011 – Haziran 2017 tarihleri arasında kliniğimizde invajinasyon tanısı ile tedavi  gören  48 olgunun kayıtları geriye dönük olarak incelenmiştir. Olguların  yaşları, cinsiyetleri , yatış süreleri, tedavi biçimi ve fatura tutarları değerlendirilmiştir. Genel durumu bozuk yada peritonit bulguları olan olgularda nonoperatif yöntem uygulanmamıştır. Mann Whitney U testiyle elde edilen verilerin istatiksel analizi yapılmıştır.

BULGULAR: Olguların 31’i erkek, 17’si kız olup, yaş ortalaması 48 ay olarak bulunmuştur.  30 olguya açık cerrahi, 18 olguya USG eşliğinde serum fizyolojik ile  hidrostatik redüksiyon uygulanmıştır. Açık cerrahi uygulanan olguların ortalama yatış süresi  5.13 gün olup, fatura tutarı 1094.60 TL  bulunurken USG eşliğinde serum fizyolojik ile hidrostatik redüksiyon  tedavisi  uygulanan olguların yatış süresi  2.6 gün , fatura tutarı 841. 30 olarak bulunmuştur.  İki tedavi yöntemi karşılaştırıldığında fatura tutarı (p<0.05)  ve yatış süresi  (p<0.01) açısından istatistiksel olarak anlamlı fark saptanmıştır.

SONUÇ:  Çalışmamızda  invajinasyon tedavisinde  USG eşliğinde serum fizyolojik  ile  hidrostatik redüksiyonun açık cerrahiye göre fatura tutarının düşük olmasından dolayı ekonomik açıdan belirgin avantajlı olduğu saptanmıştır. USG eşliğinde serum fizyolojik  ile hidrostatik redüksiyon basit, etkili, güvenilir ve komplikasyon oranı düşük  bir tedavi yöntemi olarak bilinmektedir.  Bunlara ek olarak ekonomik bir tedavi yöntemi olması nedeniyle de, herhangi bir kontrendike  durum yoksa  tüm olgularda  ilk tedavi seçeneği olarak değerlendirilmelidir.
