First case of the utilization of an implanted port catheter in the portal vein for long-term parenteral nutrition

Aim: To present the first case of long-term utilization of portal vein for total parenteral nutrition in English literature.

Case: A four-month-old girl was referred to our interstinal failure and rehabilitation center with intractable diarrhea and intestinal failure.Extensive evalutation document IPEX-like syndrome (auto-immune hemolytic anemia, auto-immune thrombocytopenia, hypothyroidism and persisted diarrhae due to villous atrophy).She had a concomitant thrombotic disease (homozygous MTFr mutation); two out of four vascular accesses were obliterated at admission and remaining and to accesses were lost due to thrombosis under prevention with low molecular weight heparin at ten months of follow-up.Hence, a CT-angiography was necessary for further evaluation of the major vasculature, which revealed thrombosed inferior vena cava(IVC) as well as obliterated upper and lower extremity veins.Urgent laparotomy and assessment of iliac and mesenteric veins were necessary.Iliac veins and IVC was thrombosed up to hepatic level.The only patent vasculator was the portal system.Therefore, a 4.5-French port catheter was introduced into the inferior mesenteric vein(IMV) and advanced to the portal vein.The catheter was flushed continuously with saline solution containing low molecular weight heparin.Cyclic TPN was given toportal system under weekly controls of blood liver function tests and monthly ultrasonographic evaluations of the portal system.Deterioration in liver functions emerged after eight-weeks with the onset of candidial septicemia(Candida parapsilosis).Linear echogenic bands in the liver, peri-portal thickening, and ascites were evident on 8thweek ultrasonographic control.Portal venous thrombosis developed in 10thweek. The patient died due to sepsis within 11thweek of the catheterization.

Conclusion: In the light of the experience from this unique case, we may conclued that portal vein may safely be used for parenteral nutrition under close follow-up when necessary.

 

Portal vene port kateter: Uzun süreli parenteral beslenme amaçlı ilk deneme

Amaç: Bu çalışmada uzun süreli total parenteral nütisyon (TPN) amaçlı portal vene port kateter yerleştirmek zorunda kaldığımız ilk olgudaki deneyimimizin sunulması amaçlandı.

Olgu: Düzelmeyen diare ve barsak yetmezliği bulgularıyla barsak yetmezliği ve rehabilitasyonu merkezimize yönlendirilen 4 aylık kız bebekte yapılan tetkikler sonunda IPEX-benzeri sendrom (otoimmun hemolitik anemi, otoimmun trombositopeni, hipotiroidi, villöz atrofiye bağlı persistan diare) tanısı kondu. Homozigot MTFr mutasyonu pozitif olan olgunun başvuruda femoral venleri oblitere idi. Tromboz gelişmesi nedeniyle boyundaki santral venöz yolların da kaybedilmesi üzerine görüntüleme tetkikleriyle doğrulandıktan sonra acil laparotomy uygulandı. İliak damarlar ve inferior vena kavanın da oblitere olduğunun görülmesi üzerine açık tek sistem olan portal sisteme kateter yerleştirilmesi kararlaştırıldı. İnferior mezenterik venden girilerek 4,5 Fr port kateter portal vene ilerletildi. Kateter mezokolonda tespitlendikten  sonra arka ucu retroperiton alandan sol üst kadran cilt altına çekilerek port haznesine bağlandı. Katetere sürekli heparinli salin yıkaması uygulandı. Portal sisteme TPN siklik şekilde verilirken rutin haftalık karaciğer fonksiyon testleri (Tablo 1) ve aylık radyolojik kontroller uygulandı. Olguda 8. haftada Kandida sepsisi (Kandida parapsilosis) geliştiğinde karaciğer enzimlerinde bozulma, Doppler ultraon kontrolünde parankimde ekojenik bantlar, periportal kalınlaşma ve assit saptandı. Onuncu haftada portal ven de tromboze oldu. Hasta sepsis nedeniyle katterizasyonun 11. haftasında kaybedildi.

Sonuç: Mecbur kalındığında portal ven yakın izlem altında olmak şartıyla uzun süreli TPN amaçlı katterize edilebilir.
