Diafragmatic hernia in a newborn who underwent thoracentesis due to antenatally chilotorax: Is it iatrojenic?

Introduction/Aim: In congenital chylothorax management, antenatal thoracentesis, pleuroperitoneal shunt and pleuordesis are applicable procedures however all these techniques have variable risks and selection of indications must be carefully. In this study, a newborn with fetal chylothorax treated with antenatal thoracentesis and postnatally identified diaphragmatic hernia.

Case report: At another center a 3150 gr newborn boy with 39th gestational week and spontaneous vaginal delivery, he consulted because of need nasal oxygen requirement for mild respiratory distress and plain chest radiograph shows loops of intestine in the left hemithorax. He hospitalized in newborn surgery intensive care unit and contrast enema study shows colonic loops in left hemithorax. In case history 25th gestational week fetal echocardiography have normal findings however in antenatal ultrasonography, collapsed left part of lungs and pushed heart to the right because of pleural fluid in left hemithorax; therefore with antenatal thoracenthesis 90ml pleural fluid drained. Control ultrasonography after procedure shows that left part of lungs and heart located where should be, and drained fluid analysis exposed %93 lymphocyte ratio in favor of chylothorax; repeated fetal ultasonographic findings are normal.

During surgery in 3 days old, in the middle of left part of diaphragm 2cm diameter aperture with scarring margins, small bowel and colonic loops pass through left hemithorax. When intestinal loops take back to the abdomen, a 2mmX2cm length fibrotic fascia reach out of posterolateral corner of diaphragmatic defect to transvers colon and 1cm distal 2 dot like transvers colon serosa defect found with prolapsing bowel mucosa, as well as bowel loops has no infarction.  Consequently diaphragmatic herni considered iatrogenic due to antenatal thoracentesis. Left part of lungs well developed and there is no hernia sac. The fibrotic fascia excised, colonic laceration and diaphragmatic defect was repaired, the patient observed intubated in postoperative early hours and after extubation there was no respiratory distress. In postoperative seventh day he discharged with healing.

Results: Antenatal thoracenthesis because of fetal chylothorax defends fetal pulmonary delivery and cardiac functions, after that antenatal any pathology undetected but diaphragmatic hernia symptoms arised postnatal early period. As a result after procedures and postnatal early period, who had fetal invasive prosedures should be examined carefully therefore complications releated invasive procedures.

Antenatal şilotoraks nedeniyle torasentez yapılan bir yenidoğanda diyafragma hernisi: Yatrojenik mi?

Giriş/Amaç: Konjenital şilotoraks tedavisi için antenatal dönemde toransentez, plevroperitoneal şant veya plörodezi uygulanabilen yöntemlerdir; ancak tüm bu yöntemlerin mevcut riskleri nedeniyle girişim endikasyonu dikkatle seçilmelidir. Bu çalışmada, fetal şilotoraksı antenatal torasentez ile tedavi edilen bir diyafragma hernili yenidoğan sunuldu.

Olgu Sunumu: Dış merkezde 39 haftalık spontan vaginal doğumu takiben hafif solunum sıkıntısı sebebiyle nazal oksijen verilen bebeğin akciğer grafisinde sol hemitoraksta barsak ansları ile uyumlu görünüm nedeniyle konsülte edildi. Cerrahi yenidoğan yoğun bakım ünitemizde interne edilen 3150 gram erkek yenidoğanın lavman opak grafisinde kolonun sol hemitoraks içinde olduğunu görüldü. Gebeliğin 25.haftasında fetal ekokardiyografinin normal olduğu, fetal US’da sol akciğeri kollabe eden ve kalbi sağa iten plevral mayi nedeniyle fetüsün sol hemitoraksından 90ml plevral mayi boşaltıldığı; işlem sonrası sol akciğer ve kalbin sol hemitoraksa yerleştiği; aspirat sıvısında %93’lük lenfosit oranının şilotoraks lehine değerlendirildiği; tekrarlanan fetal US’lerinde akciğerlerin normal görüntülendiği öğrenildi.

Üç günlük iken yapılan operasyonda, sol diyafragmanın ortasında kenarları skatris dokusu şeklinde kalınlaşmış yaklaşık 2 cm çaplı defektten ince barsakların ve kolonun toraksa çıktığı izlendi. Barsaklar batına alındığında defektin posterolateral köşesinden transvers kolona uzanan 2mmx2cm’lik fibrotik bir bant saptandı, intestinal anslarda dolaşım bozukluğu yoktu ancak fibrotik bantın 1 cm ilerisinde transvers kolon üzerinde 2 adet noktasal tarzda seroza defektinden mukozal prolapsus mevcuttu. Bu bulgular, herninin torasenteze bağlı yatrojenik geliştiğini düşündürdü. Sol akciğerin gelişimi iyi olup herni kesesi yoktu. Bant eksizyonu, kolon laserasyonu ve diyafragma hernisi onarımı yapılan hasta postoperatif ilk saatler entübe takip edildi, ekstübasyon sonrası solunum sıkıntısı görülmedi. Postoperatif yedinci gün sorunsuz taburcu edildi.

Sonuç: Fetal şilotoraks nedeniyle torasentez yapılması, fetusun akciğer gelişimi ve kardiyak fonksiyonlarını korunmuştur, girişim sonrası antenatal takiplerinde herhangi bir patoloji saptanmamış olsa da postnatal erken dönemde diyafragma hernisi bulguları ortaya çıkmıştır. Fetal girişim uygulanan olgular işlem sonrası dönemde ve postnatal erken dönemde fetal girişime bağlı oluşabilecek komplikasyonlar açısından dikkatlice değerlendirilmelidir.
