WHAT HAVE CHANGED IN THE ANTERIOR ABDOMINAL WALL DEFECTS OF NEWBORNS? 20 YEARS EXPERIENCE OF A REFERENCE CENTER

Aim: The aim of this study was to investigate changes of the incidence, complications and mortality rates of patients with anterior abdominal wall defects over the years.

Materials and Methods: Patients who were operated due to anterior abdominal wall defect between 1998 and 2017 were evaluated retrospectively. The cases were firstly divided into two groups: Omphalocele (OM) and gastroschisis (GS). Then the patients were separated into two subgroups: those who were operated between January 1998 and December 2007 (Group A), and those who were operated between December 2017 and January 2008 (Group B). The diagnosis, incidence, prenatal diagnosis, presence of sepsis, surgical complications and mortality rates of the patients were evaluated.

Results:

Findings were parallel in both case groups. As in the OM cases, incidence of GS cases was increased in the second period (Group B) (Group A, Group B, GS, OM: 0.20%, 0.27%, 0.089%, and 0.18%, respectively). Prenatal diagnosis rates were also higher in Group B than in Group A. However, surgical complications decreased in Group B for both OM and GS groups; and mortality decreased in the second period (Group A, Group B, GS, OM: 45.5%, 29.2%, 60%, 50%, respectively) despite increased sepsis rates.

Conclusion: Although OM and GS were two different defect groups, their periodic findings were parallel. The reduction of complications and mortality rates can be achieved with the increase of prenatal diagnosis and with the birth of the cases in the centers where they can be treated.

YENİDOĞANDA KARIN ÖN DUVARI DEFEKTLERİNDE NE DEĞİŞTİ? REFERANS BİR MERKEZİN 20 YILLIK DENEYİMİ

Amaç: Bu çalışmada karın ön duvarı defekti olan hastaların insidansı, komplikasyonları ve mortalite oranlarının yıllar içerisindeki değişiminin araştırılması amaçlandı.

Materyal ve Metotlar: 1998-2017 tarihleri arasında karın ön duvarı defekti nedeniyle ameliyat edilen olgular geriye dönük olarak değerlendirildi. Olgular omfalosel (OM) veya gastroşizis (GS) olmak üzere iki gruba ayrıldı. Ocak 1998-Aralık 2007 tarihleri arasında ameliyat edilenler (A grubu) ve Ocak 2008-Aralık 2017 tarihleri arasında ameliyat edilenler (B grubu) iki alt gruba ayrıldı. Hastaların tanısı, insidansı, prenatal tanının olup olmadığı, sepsis gelişmesi durumu, cerrahi komplikasyonlar ve mortalite oranları değerlendirildi.

Bulgular:

Her iki olgu grubunda bulgular paralellik göstermekteydi. Omfalosel olgularında olduğu gibi GS olgularında da insidans ikinci dönemlerde (Grup B) artmıştı (Sırasıyla GrupA, Grup B, OM: %0.20, %0.27, GS %0.089, %0.18). Prenatal tanı konulma oranları da Grup B’ de Grup A’ dan yüksektir. Yine hem OM hem GS gruplarında Grup B’ de cerrahi komplikasyonlar azalmış, sepsis oranlarında artış olmasına rağmen mortaliteler ikinci dönemde azalmıştır (Sırasıyla GrupA, Grup B, OM: %45.5, %29.2, GS: %60, %50).

Sonuç: OM ve GS farklı iki defekt grubu olmasına karşın dönemsel bulguları paralellik gösterdi. Prenatal tanımlamanın artışı ve olguların tedavi edilebileceği merkezlerde doğurtulmaları ile birlikte komplikasyon ve mortalite oranlarının azaltılması sağlanabilir.
