Central Venous Catheter Insertion by Experienced Hands Using Anatomic Signs Does Not Eliminate Complications

Introduction: Central venous catheterization (CVC) is a commonly used procedure in child surgery and intensive care units. Due to small vein diameters, the fact that veins have not been fully developed and anatomical differences, difficulties in obtaining catheters in the appropriate diameter and size, catheter procedures in children are technically more difficult in comparison to adult patients. The purpose of this study is to evaluate the CVC procedures with anatomic signs by experienced (35, 14, 4 years) anesthesiologists and the associated complications.

Method: 499 cases under the age of 18 who were inserted CVC (temporary, port or Hickman) between the years 2014-2017 were included in this retrospective study. All catheters were inserted using anatomic signs.

Findings: The median age of the patients was 24 months (1 day-216 months) and 50.1% were male. 53.7% were given intratracheal general anesthesia and 46.3% were given mask anesthesia. 74.3% of the catheters were inserted from the right subclavian vein and 51.7% were inserted through single punction. 48.3% of all catheters were inserted by a faculty member while attending and assistant anesthesiologists initiated 65.3% of catheter procedures and were able to insert 51.7% of them. 1.4% of patients (7 patients) could not be inserted an SVC despite all efforts. Temporary arrhythmia developed in 6% (n=30) of the cases, while pneumothorax developed in 1.4% (n=7) and artery punction in 8.4% (n=40). A meaningful correlation was detected between left vena jugularis interna (p<0.001), left vena subclavian (p=0.002), right vena subclavian (p=0.003) insertion, more than two punctions (p=0.007) and permanent catheter (p=0.010) procedures and complications. No correlation was detected between the experience of the physicians undertaking the procedure and the complications (p>0.05).

Conclusion: In CVC insertions using anatomic signs, complications are seen more frequently in children. Subclavian insertions, repetitive punctions, and permanent catheter procedures constitute risk factors for CVC complications. Even though a decrease in general complication rates was detected from implementation by experienced hands, it is seen that experience does not eliminate complications in CVC insertions using anatomic signs.

Deneyimli Ellerde Anatomik İşaretler Kullanılarak Santral Venöz Kateter Girişimi Komplikasyonları Yok Etmez

Giriş: Çocuk cerrahisi ve yoğun bakım kliniklerinde santral venöz kateterizasyon (SVK) sık uygulanan bir işlemdir. Çocuklarda damar çaplarının küçüklüğü, damar yapıların tam gelişmemiş olması ve anatomik değişiklikler, uygun çapta ve boyutta kateter temininde güçlükler nedeniyle kateter uygulamaları erişkin hastalara göre teknik olarak daha güçtür. Bu çalışmada deneyimli (35, 15, 4 yıllık) ellerde anatomik işaretlerle SVK uygulamaları ve karşılaşılan komplikasyonların değerlendirilmesi amaçlanmıştır.

Metod: Bu retrospektif çalışmaya 2014-2017 yılları arasında SVK (geçici, port veya Hickman) uygulanan 18 yaş altındaki 499 olgu dahil edildi. Kateterlerin tümü anatomik işaretler kullanılarak yerleştirildi.

Bulgular: Hastaların median yaşları 24 ay (1 gün-216 ay) idi ve %50.1’i erkekdi. %53.7’sine intratrakeal genel anestezi, %46.3’üne ise maske anestezisi uygulandı. Kateterlerin %74.3’ü sağ subklavian venden ve %51.7’si tek ponksiyonla takıldı. Tüm kateterlerin %48.3’ü öğretim üyesi tarafından uygulanırken, anestezi uzmanı ve asistanı kateter işlemlerinin %65.3’ünü başlattı ve %51.7’sini takabildi. Hastaların %1.4’ne (7 hasta) tüm girişimlere rağmen SVK takılamadı. Olguların %6’sında (n=30) geçici aritmi, %1.4’ünde (n=7) pnömotoraks ve %8.4’ünde (n=40) arter ponksiyonu gelişti. Sol vena jugularis interna (p<0.001), sol vena subklavian (p=0.002), sağ vena subklavian (p=0.003) yerleşim, ikiden fazla ponksiyon (p=0.007) ve kalıcı (p=0.010) kateter uygulamaları ile komplikasyonlar arasında anlamlı ilişki saptandı. İşlemi uygulayan hekimlerin deneyimi ile komplikasyonlar arasında ise ilişki saptanmadı (p˃0.05).

Sonuç: Anatomik işaretlerle SVK yerleştirmede komplikasyonlar çocuklarda daha sık olarak görülmektedir. Subklavian girişimler, tekrarlayan ponksiyonlar ve kalıcı kateter uygulamaları SVK komplikasyonları için risk faktörlerini oluştururlar. Deneyimli ellerde uygulanması ile genel komplikasyon oranlarında azalma saptansa da anatomik işaretler kullanılarak SVK yerleştirmede deneyimin komplikasyonları yok etmediği görülmektedir.
