A rare cause of abdominal distension in the newborn: Congenital imperforate hymen

Introduction: Congenital imperforate hymen is the most common obstructive anomaly of the female reproductive system. The incidence is reported to be 0.0014-0.01%. While the cases with mild hydrocolpos are not diagnosed until the adolescent age, severe cases can be detected as large cystic masses in the abdomen in the prenatal period. Up to date, 22 prenatal cases have been reported.

Case: Our patient was born from a 30-year-old mother at 40th weeks of gestation as 3600 g. A mass was detected after birth. Abdominal ultrasound revealed a cystic mass of 69x42 mm in size at the bladder localization on postnatal day 3. Two weeks later, USG revealed a 70x50x100 mm cystic mass with a preliminary diagnosis of ovarian cyst, mesenteric cyst, and duplication cyst. Meanwhile, it has been reported that the volume of the bladder is increased and bilateral grade II hydroureteronephrosis exists. On the same day, the pelvic MRI result was reported as "Hypointense in T1 and a hyperintense cystic lesion in T2" on the right of the bladder (Figure 1). At the same day, the patient applied to the emergency service with the complaint of anuria all day. After urine culture sampling, she was referred to our department without noticing the status of the hymen. An uretral catheter was introduced, the passage of urine was seen. The patient was restless, the abdomen was distended and tender. The mass was palpable up to the umbilicus. At the urogenital system examination, hymen imperforatus is noted. After culture sampling through the hymen, hymenectomy was performed (Fig. 2). Sudden decrease in the distention of the abdomen and regression in clinical complaints of the patient were observed. Antibiotherapy was continued for 5 days and the patient was discharged without any problems.

Discussion: Although hymen imperforatus is typically diagnosed during the adolescence period, it should be considered in the differential diagnosis of cystic masses in the abdomen of the newborn. With the detailed physical examination, the diagnosis of this disease can be made easily and unnecessary investigations and operations can be prevented.

Yenidoğanda nadir bir batında distansiyon nedeni: Konjenital imperfore himen

Giriş: Konjenital imperfore himen kadın üreme sisteminin en sık görülen obstrüktif anomalisidir. İnsidansı %0.0014-0.01 arasında bildirilmektedir. Bu nedenle oluşan hidrokolposun hafif olduğu olgular adölesan yaşa kadar tanı almazlar iken ağır olduğu vakalar prenatal dönemde abdomende geniş kistik kitle olarak saptanabilirler. Şu ana kadar prenatal saptanan 22 vaka rapor edilmiştir.

Olgu: 30 yaşındaki anneden takipsiz gebelik sonucu 40 haftalık ve 3600 gr doğan kız hastada doğumdan sonra batında kitle saptanmış. Postnatal 3. günde yapılan batın USG’de mesane lokalizasyonunda 69x42 mm boyutlarında kistik kitle saptanmış. İki hafta sonra çekilen kontrol USG’de 70x50x100 mm olan kisti için over kisti, mezenterik kist, duplikasyon kisti ön tanısı ile pelvik MR istenmiş. Bu arada mesanenin hacminin artmış olduğu ve bilateral grade II hidroüreteronefroz olduğu da rapor edilmiş. Aynı gün çekilen MR sonucu ”Mesane sağ kesimi üzerinde T1’de hipointens, T2’de hiperintens kistik lezyon” şeklinde raporlanmış (Resim 1). Hasta aynı günün akşamı tüm gündür idrar yapmama şikayeti ile acil servise başvurmuş. Burada sonra ile idrar kültürü alınıp himendeki patoloji farkedilmeden tarafımıza konsülte edilmiş. Yapılan muayenede hasta huzursuz, batın distandü ve hassas saptandı. Umblikusa kadar varan kitle palpe edildi. İdrar sondası takıldı, idrar gelişi görüldü. Yapılan ürogenital sistem muayenesinde hymen imperforatus saptanması üzerine önce kültür için örnek alındı, sonrasında himende bistüri yardımı ile bir kesi yapılarak drenaj sağlandı (Resim 2). Hastanın batın şişkinliğinde ani azalma ve klinik şikayetlerinde gerileme saptandı. Antibiyoterapisi 5 gün devam edilen hasta sorunsuz taburcu edildi.

Tartışma: Hymen imperforatus tipik olarak adölesan dönemde tanı almasına rağmen yenidoğanlarda da batındaki kistik kitlelerin ayırıcı tanısında düşünülmesi gerekmektedir. Yapılacak detaylı fizik muayene ile hastalığın tanısı kolayca konulabilir ve gereksiz tetkik ve ameliyatların önüne geçilebilir.
