Toraco-abdominopagus: multidisciplinary approach

Introduction

Conjugated twins appear only in monocaryonic / monoamniotic twin pregnancies and are very rare.Fusions are seen at different levels and sizes in the body,most common is thorax and abdomen.

Case

The girls those diagnosed with thoraco-abdominopagus by ultrasonography at 12th week of pregnancy were referred to our hospital after deliveryHeart, liver and intestines were related to each other in computed tomography.The baby with scoliosis of the conjoined twins and to the right of the operation table was called baby1 and other 2.Midline incision was performed and a single pericardium and two hearts were seen.Baby 1 had a rotatory axis of heart and both had an aberrant vein connecting the vena cava which was cut off and separated.The stomach in baby 1 was in the lower right position. When gastric outlet was followed, after the duodenum C, the baby 2’s duodenum was associated with baby 1’s as T-shaped.Jejunum,ileum and colon of Baby 1 were native. Baby2 had type 3b jejunal atresia and unused colon.Duodenal fusion was removed and primer anastomose was performed.In baby2,atresic segment was resected and side-side anastomosis was performed.There were two porta hepatis.It has been observed that bile duct was observed normal in baby 1 and rudimenter in baby 2, both of them were related to the duodenum.Liver was seperated by harmonic cautery, radiofrequency cautery, cusa and clips from ligamentum teres hepatis.The larger part of the liver remained in baby 1.The intestinal segment cauliflower in the 2nd baby was in a single vein and was in the form of a helix. In baby 2, the intestinal meso contained a vessel and was in the form of a helix.The malrotation was corrected and the small intestines were placed on the right and the colon on the left.The abdomen wall was freed from the posterior wall and the babies separated from each other. Heart was covered with bovine pericardium, and goreteks mesh was used in both abdomen and thorax.Both were exitus in postoperative 3rd and postoperative 73th day respectively due to respiratory and circulatory failure.

Conclusion

Thoraco-abdominopagus is a very rare condition with high mortality and requires multidisciplinary approach

Giriş

Yapışık ikizler sadece monokaryonik/monoamniyotik ikiz gebeliklerde görülür ve çok nadirdir. Gövdede değişik seviye ve boyutlarda füzyon görülür. En sık olarak toraks ve abdomende görülür. Torako-abdominopagus nedeniyle opere edilen olguların sunulması amaçlandı.

Olgu

Gebeliğin 12.haftasında yapılan ultrasonografi ile torako-abdominopagus tanısı konan kız hastalar doğum sonrası hastanemize sevk edildi. Hastalara çekilen bilgisayarlı tomografide kalp, karaciğer ve barsakların birbiriyle ilişkili olduğu görüldü. Yapışık ikizlerden skolyozu olan ve ameliyat masasının sağında olan bebek 1, solunda olan ise bebek 2 olarak adlandırıldı. Orta hattan insizyonla toraksa girildi, tek perikard ve iki kalp mevcuttu. Bebek 1’in kalbi apikale rotasyonda olup her iki bebekte vena kavaları bağlayan aberran bir damar mevcuttu, kesilip ayrıldı. Bebek 1’de midenin karın sağ alt kadranda olduğu gözlendi. Mide distali takip edildiğinde duodenum C’sinden sonra bebek 2’nin duodenumu ile birleştiği ve T şeklinde bebek 1’in jejunumu ile devam ettiği görüldü. Bebek 1’in ileum ve kolonu doğaldı. Bebek 2‘de tip 3b jejunal atrezi ve kullanılmamış kolon mevcuttu. Duodenal füzyon ayrılıp bebek 1 primer anastamoz edildi. Bebek 2’de atrezik segment rezeke edilip yan-yan anastomoz yapıldı. Kordonun karaciğere tek olarak girdiği devamında iki porta hepatis olduğu görüldü. Bebek 1’de normal, bebek 2’de ise rudimenter safra kesesi olduğu her ikisinin de duodenumlarla bağlantılı olduğu görüldü. Ligamentum teres hepatis hizasından karaciğer kuza, harmonik koter radyofrekans koter ve klipslerle sepere edildi. Karaciğerin daha büyük kısmı bebek 1’de kaldı. Bebek 2’deki intestinal segment mezosunu tek damar oluşturmaktaydı ve helezon şeklindeydi. Malrotasyon düzeltilip ince barsaklar sağ, kolon ise sola yerleştirildi. Karın duvarı posteriordan serbestleştirilip bebekler birbirinden ayrıldı. Kalp bovin perikard, karın ve toraks ise goreteks mesh yardımıyla kapatıldı. Bebek 1 postoperatif 3.gün bebek2 ise postoperatif 73. gün solunum ve dolaşım yetmezliği nedeniyle hasta kaybedildi.

Sonuç

Torako-abdominopagus mortalitesi yüksek, multidisipliner yaklaşım gerektiren çok nadir görülen bir durumdur.
