Mermaid baby: A historical case

Introduction: Sirenomelia is a rare anomaly seen in 60,000-100,000 births. The male/female ratio is 2.7/1 and is more frequent in monozygotic twins. In this syndrome, there are adherent legs giving the appearance of a mermaid in babies, as well as severe gastrointestinal tract, genitourinary, cardiovascular and central nervous system disorders. We report a mermaid syndrome was presented because of its rare occurrence.

Case: A baby was born 2500gr at the 36th week from the forty-five-year-old mother as a twin pregnancy. The patient was transferred to our clinic because of esophageal atresia and distal tracheoesophageal fistula, duodenal atresia, urethral agenesis, anorectal malformation, radius agenesis and conjoint lower extremities (simpus dipus) (Picture 1-2). The proximal esophageal pouch was aspirated intermittently. A suprapubic catheter was inserted into the bladder for drainage of the urine. Abdominal ultrasound revealed a malrotated solitary kidney. Hypoplasia of transverse aorta, atrial septal defect, and ventricular septal defect were detected on echocardiography. He died due to cardiac pathologies on the third day of the follow-up.

Discussion: Sirenomelia is a rare congenital anomaly known for thousand years (Picture 3). In the pathogenesis, the cause is the inappropriate mesodermal cell migration. The risk factors in etiology are the mother's age <20 or >40, identical twin, exposure to harmful factors in pregnancy, diabetic mother and male sex. The mermaid syndrome usually results in early death due to bilateral renal agenesis, oligohydramnios, and severe pulmonary hypoplasia. Although our patient had a solitary kidney and urethra agenesis, he didn't have pulmonary hypoplasia due to the fact that the twin was healthy. This played an important role in the patient's postnatal life. Very few surviving cases have been reported in the literature where the kidneys of these cases are functional. Our patient died due to cardiac pathologies on the third day of follow-up. It is important to guide perinatal consultation for possible pregnancy termination in the presence of oligohydramnios, bilateral renal agenesis and parallel conjoined lower extremities detected by ultrasonography performed in early pregnancy.

Denizkızı bebek: Geçmişten bir olgu

Giriş: Sirenomelia 60.000-100.000 doğumda bir görülen nadir bir anomalidir. Erkek/kız oranı 2.7/1'dir ve monozigotik ikizlerde daha sıktır. Bu sendromda bebeğe denizkızı görünümünü veren yapışık bacakların yanı sıra, ağır genitoüriner, gastrointestinal, kardiyovasküler ve merkezi sinir sistemi bozuklukları bulunur ve literatürde çok az sayıda yaşayan olgu bildirilmiştir. Burada denizkızı sendromu tanısı alan bir olgu nadir görülmesi nedeniyle sunuldu.

Olgu: 45 yaşındaki annenin 2. gebeliğinden ikiz eşi olarak 36. gestasyonel haftada C/S ile 2500 gr. olarak doğan olguda ÖA+TÖF, duodenal atrezi, üretra agenezisi, ARM, radius agenezisi ve alt ekstremitelerinin birleşik görünümde olması (simpus dipus) üzerine hasta kliniğimize nakil alındı (resim 1-2). Proksimal özofagus aralıklı aspirasyona alındı. Mesaneye suprapubik kateter takılarak üriner drenaj sağlandı. Yapılan batın USG’de soliter böbrek ve böbrek malrotasyonu saptandı. Ekokardiyografide transvers aorta hipoplazisi, atriyal septal defekt ve ventriküler septal defekt saptandı. Hasta izleminin 3. gününde kardiyak patolojileri nedeniyle ex oldu.

Tartışma: Sirenomelia yaklaşık olarak 2000 yıl öncesinden beri bilinen doğumsal nadir bir anomalidir (resim3). Patogenezinde mezodermal hücre göçünün uygun şekilde olmaması sorumlu tutulmuştur. Etiyolojisinde anne yaşının 20’nin altında veya 40’ın üzerinde olması, tek yumurta ikizi olma, gebelikte çeşitli zararlı etkenlere maruz kalma, diyabetik anne bebeği olma ve erkek cinsiyet risk etmenleri olarak bildirilmiştir. Denizkızı sendromu; genellikle bilateral renal agenezi, oligohidramniyoz ve ciddi pulmoner hipoplazi nedeniyle erken dönemde ölümle sonuçlanır. Olgunun ikiz eşinin sağlıklı olması nedeniyle olguda üretra agenezisi ve soliter böbrek olmasına rağmen pulmoner hipoplazi yoktu. Bu durum hastanın doğum sonrasında canlı olarak izlenmesinde önemli bir rol oynadı. Literatürde çok az sayıda yaşayan olgu bildirilmiştir ve bunlar genellikle işlev gösterebilen böbrekleri bulunan olgulardır. Olgu kardiyak patolojisi nedeniyle izleminin 3. gününde ex oldu. Gebeliğin erken döneminde yapılan ultrasonografi ile oligohidramniyos, bilateral renal agenezi ve paralel seyreden alt ekstremitelerin varlığında olası gebelik sonlandırması için perinatolojik danışmanlığa yönlendirmek önemlidir.
