Causes of Cholelithiasis in Children and Cholecystectomy Results

Introduction: Gallstones are increasingly common in children in recent years, though not as often as adults. In addition to hematological diseases, long-term use of TPN and sepsis-associated gallstones are seen more. In our study, we aimed to evaluate the operated cases with gallstones within literature


Material And Methods: Between 2012 and 2018, 97 patients who underwent cholecystectomy in our clinic were reviewed for gender, age, , application complaint, etiology, surgery, duration of stay and pathological data retrospectively.

Findings: The mean age of the 97 cases undergoing cholecystectomy (53 K, 44 E) was 13.4 years (11 months- 18 years). The most common complaints of application were abdominal pain and vomiting, but gallstone was found in 17 patients incidentally. A multiseptate gall bladder revealed in one case on the ultrasound. In all other cases, multiple stones were seen in the gall bladder. The mean size of the stones seen on ultrasonography was 12 mm (2-20 mm). When the etiology was investigated, hereditary spherocytosis in16 cases and thalassemia major in 5 cases were detected. Haemotologic etiology was detected in 21.6% of the cases. There was
cholelithiasis with choledocholitiazis in six cases. Sphincterotomy with ERCP followed by cholecystectomy was performed in these cases.
All cases underwent laparoscopic cholecystectomy and three cases underwent open surgery for technical reasons. In five cases, splenectomy was performed. The average length of hospital stay was 2,7 days. In the pathologic examination, multiseptate gallbladder in one case, gallbladder diverticulum in one case and chronic cholecystitis in other cases have been reported.

Conclusion: Gallstones are an increasing pathology in childhood. In recent years,
haematological causes as well as non haematological and idiopathic causes are more common. In the treatment, laparoscopic cholecystectomy should be performed safely when choledocholithiasis is ruled out.

Giriş: Safra taşları, çocuklarda erişkinlerdeki kadar sık olmasa da son yıllarda görülme oranları giderek artmaktadır. Hematolojik hastalıkların yanı sıra uzun süre TPN kullanımı ve sepsise bağlı safra taşları daha çok görülmektedir. Çalışmamızda, safra taşı nedeni ile opere edilen olguları literatür eşliğinde değerlendirmeyi amaçladık.

Materyal ve Method: 2012 – 2018 tarihleri arasında kliniğimizde kolesistektomi yapılan 97 olgu cinsiyet, yaş, başvuru şikâyeti, etyoloji, yapılan cerrahiler, yatış süresi ve patolojik verileri geriye yönelik incelendi.

Bulgular: Kolesistektomi yapılan 97 olgunun (45 K, 29 E) yaş ortalaması 13,4 yıl (11 ay- 18 yıl) idi. Olguların hastaneye başvuru şikayeti en sık karın ağrısı ve kusma idi, ancak17 olguda insidental olarak safra taşına rastlandı. Ultrasonografide bir olguda multiseptalı safra kesesi görüldü. Diğer tüm olgularda safra kesesinde multiple taş görüldü. Ultrasonografide görülen taşların ortamla boyutu 12 mm (2-20 mm) idi. Etyoloji araştırıldığında 16 olguda herediter sferositoz beş olguda ise talasemi majör tespit edildi. Olguların %21.6’ sında etyolojide hematolojik neden tespit edildi. Altı olguda safra kesesi taşı ile birlikte koledok taşı vardı. Bu hastalara ERCP ile sfinkterotomi yapılıp ardından kolesistektomi yapıldı. Tüm olgulara laparoskopik kolesistektomi yapıldı, üç olguda teknik nedenlerle açık cerrahiye geçildi. Beş olguda beraberinde splenektomi yapıldı. Olguların hastanede yatış süresi ortalama 2,7gün idi. Patolojik incelemede bir olguda multiseptalı safra kesesi, bir olguda ise safra kesesi divertikülü, diğer olgularda ise kronik kolesistit rapor edildi.

Sonuç: Safra taşları, çocukluk çağında sıklığı artan bir patolojidir. Son yıllarda etyolojide hematolojik nedenlerin yanı sıra non hematolojik ve idiopatik nedenler daha sık görülmektedir. Tedavide koledok taşı ekarte edildiğinde laparoskopik kolesistektomi güvenle uygulanabilir.
