“Skeptical approach” to sharp foreign body ingestion cases may prevent probable morbidity

AIM:

Foreign body ingestion is a frequently encountered problem among children although very few cases require surgical interventions. Especially in adolescents and in individuals experiencing psychological problems, patient management requires special attention. In this study, we aimed to present the challenges that came up in the management of a case of sharp foreign body ingestion with suicidal intent.

CASE:

A sixteen-year-old girl admitted with chest pain, 24 hours after breaking and swallowing blades of 4 fruit knives. On physical examination, vital findings were stable and there was no pathological finding. Chest X-ray showed one piece of a knife-shaped opacity in the proximal esophagus. Pelvis was not included in the graphy. Repeated chest and abdomen X-ray’s revealed two pieces of sharp radiopaque objects in the esophagus and two in the left lower abdominal quadrant. Immediate endoscopy was performed by rigid esophagoscope under general anesthesia. Two foreign bodies were removed from esophagus. There were several lacerated areas on the posterior wall in the middle portion of esophagus. No foreign bodies was detected on rectosigmoidoscopy. Intraoperative X-ray control showed that the foreign bodies relocated to the right lower abdominal quadrant. Three foreign bodies were removed from the caecum with enterotomy. The patient started oral intake following esophagography on POD 5. She was discharged without complication.

CONCLUSION:

In sharp foreign body ingestion cases, history taking and radiological evaluation may be challenging due to misleading statements of the patients and super-positions on X-ray’s. Two views X-ray control and meticulous study of these images displaying all thorax and the abdomen before any intervention, and utilization of intraoperative imaging are indispensable in order to prevent possible morbidities in these cases.

Kesici-delici cisim yutma olgularında “şüpheci yaklaşım” olası morbiditeyi önleyebilir

AMAÇ:

Yabancı cisim yutma, çocuklarda sık rastlanabilen bir problem olsa da olguların pek azında cerrahi girişim gerekir. Özellikle ergenlik çağında ve psikolojik sorunlar yaşayan bireylerde hasta yönetimi özel dikkat gerektirir. Bu çalışmada intihar amaçlı kesici-delici cisim yutma sonrası merkezimize başvuran olgunun yönetiminde karşılaşılan güçlüklerin sunulması amaçlanmıştır.

OLGU:

On altı yaşında kız olgu, kendi ifadesine göre 4 adet meyve bıçağı ucunu kırarak yuttuktan 24 saat sonra göğüs ağrısı şikayetiyle başvurdu. Yapılan fizik muayenede genel durumu iyi, vital bulguları stabil ve sistem bakıları olağandı. Akciğer grafisinde özofagus proksimalinde bir adet bıçak ile uyumlu opasite görüldü. Acil serviste çekilen grafiye pelvis dahil edilmediği için tekrarlanan grafilerinde, özofagusta iki adet, batın sol alt kadranda iki adet ucu keskin radyoopak cisimler saptandı. Genel anestezi altında yapılan acil rijid özofagoskopi ile özofagustan iki adet yabancı cisim çıkarıldı. Özofagus orta kesim posterior duvarda birkaç adet lasere alan mevcuttu. Karın sol altta görülen yabancı cisimlere ulaşılabilmesi amacıyla rektoskopiye geçildi. Bu bölgede herhangi bir doku hasarı ve yabancı cisim saptanmadı. Peroperatif görüntüleme ile kontrolde yabancı cisimlerin sağ alt kadrana yer değiştirmiş olduğu görüldü. Laparotomide çekumda saptanan üç adet yabancı cisim çıkarıldı. Postoperatif beşinci gün çekilen özofagografinin normal saptanması üzerine oral beslenen olgu sorunsuz taburcu edildi.

SONUÇ:

Yabancı cisim yutma şikayeti ile başvuran olgularda anamnez ve radyolojik görüntüler yanıltıcı olabilmektedir. Bu olgularda girişim öncesi tüm toraks ve batını kapsayan görüntülerin iki yönlü kontrolü ve ayrıntılı incelenmesi, girişim sırasında da radyolojik görüntüleme yardımının alınması olası morbiditeleri önlemek açısından vazgeçilmez öneme sahiptir.
