Colorectal carcinoma in childhood: which one is more difficult; diagnosis or treatment?

OBJECTIVE: Colorectal carcinoma is very rare in childhood and patients with advanced stage disease have a poor prognosis. Three cases diagnosed with colorectal cancer are presented.

Case 1: A 12-year-old boy was admitted to the outpatient clinic with abdominal pain, tenesmus and anemia. The patient was followed-up with a pre-diagnosis of IBD and was referred to our clinic upon the continuation of anemia and the addition of a bloody defecation complaint. Digital rectal examination revealed a mass. Radiological evaluations revealed rectosigmoid mass and diffuse metastasis in lung. Biopsy was performed. Pathological diagnosis was adenocarcinoma. Low anterior resection was performed. After a 3-year remission period, mass was detected in the anterior mediastinum. Biopsy revealed Stage 3 T-cell lymphoblastic lymphoma. The patient was in complete remission for 20months.

Case 2: A 16-year-old girl was followed for hyperintense lesion in left thalamus, combined immunodeficiency and bronchiectasis. The patient was consulted to the pediatric surgery clinic for rectal prolapse. The patient also was referred to pediatric gastroenterology because of anemia, diarrhea and intermittent abdominal pain. The biopsy of the mass filling the rectum and sigmoid colon revealed mucinous adenocarcinoma. Subtotal colectomy was performed after chemotherapy. Total colectomy was performed as the result of the polyps being dysplastic (adenomatous). The patient was in complete remission for 7 months.

Case 3: A 15-year-old boy was admitted to the emergency department with anemia, weight loss and diarrhea. Colonoscopy revealed a rectosigmoid mass and biopsy showed adenocarcinoma. Chemotherapy was started because of the inoperable stage. The mass was unresectable and end-colostomy was opened. Despite the treatment of CT and RT, he died after 13months due to her very aggressive clinical condition.

Conclusion: Raising awareness for early diagnosis of colorectal cancer, which is very rare in childhood is important in the course of the disease.

Çocukluk çağında kolorektal karsinom: tanısı mı zor, tedavisi mi?

Amaç: Çocukluk çağında kolorektal karsinom çok nadir izlenmekte ve genellikle ileri evre tanısı konan hastalar kötü prognoz ile seyretmektedir. Bu çalışmada kolorektal kanser tanısı alan üç olgu klinik seyir ve tedavi sonuçlarıyla sunulmuştur.

Olgu 1: On iki yaşında erkek çocuk karın ağrısı, tenesmus ve demir eksikliği anemisi nedeni ile çocuk polikliniğine başvurmuştur. İBH ön tanısıyla takip edilen hastanın anemisinin devam etmesi ve kanlı dışkılama şikayetinin eklenmesi üzerine hasta kliniğimize yönlendirilmiştir. Fizik muayenede rektal tuşede kitle palpe edilmesi üzerine yapılan radyolojik değerlendirmelerinde rektosigmoid kitle ve karaciğerde yaygın metastazları izlenmiştir. Yapılan kolonoskopide rektosigmoid düzeyde lümenin tamamını dolduran kitle saptanmış ve biyopsi yapılmıştır. Biyopsi sonucunda kolorektal adenokarsinom saptanan hastaya yedi kür kemoterapi, aşağı anterior rezeksiyon uygulanmıştır. Üç yıllık remisyon sürecinin ardından anterior mediastende 3,5x5 cm boyutlarında kitle saptanmıştır. Tru-cut biopsi sonucunda Evre 3 T hücreli lenfoblastik lenfoma saptanmıştır. Tedavisi tamamlanan hasta 20 aydır komplet remisyondadır.

Olgu 2: On altı yaşında kız hasta sol talamusta hiperintens lezyon, kombine immun yetmezlik, bronşektazi nedeniyle takip edilmektedir. Rektal prolapsus nedeniyle tarafımıza danışılan hastada anemi, ishal ve aralıklı karın ağrısı bulguları olması nedeniyle çocuk gastroenterolojiye yönlendirilmiştir. Kolonoskopide rektum ve sigmoid kolonu dolduran kitle biyopsisi musinoz adenokarsinom olarak sonuçlanmıştır. Kemoterapi sonrası subtotal kolektomi yapılmıştır. İzlenen yaygın poliplerin sonucu displastik (adenomatöz) olması sonucu ortak konsey kararı ile total kolektomi yapılmıştır. Tedavisi tamamlanan hasta yedi aydır komplet remisyondadır.

Olgu 3: On beş yaşında erkek çocuk anemi, kilo kaybı, ishal şikayeti ile acile başvurmuştur. Çocuk gastroenteroloji tarafından yapılan kolonoskopide rektosigmoidde kitle saptanmış, biyopsi sonucu adenokarsinom gelmiştir. Inoperable değerlendirilmesi nedeniyle kemoterapisi başlandı. Obstrüksiyon semptomları gelişen hasta tarafımızdan acil operasyona alınmıştır. Kitlenin unrezektable olarak izlenmiş, uç-kolostomi açılmıştır. KT ve RT tedavisine rağmen çok agressif seyreden kliniği nedeniyle 13 ay sonra ameliyat edilemeden kaybedilmiştir.

Sonuç: Çocukluk çağında çok nadir izlenen kolorektal kanserin, erken tanısı için farkındalığın artması hastalığın seyrinde önem taşımaktadır.
