A rare cause of intestinal obstruction: intrauterine midgut volvulus

Aim of the Study: Intrauterine midgut volvulus is a very rare, life-threatening condition and prenatal diagnosis is difficult. In this article, we present a case of midgut volvulus followed by a prediagnosis of antenatal jejunal atresia.

Case description: A one-day-old male baby who was followed with the diagnosis of antenatal jejunal atresia, with a birth weight of 3600 gr, born by C/S at 38 weeks of pregnancy from 19-year-old mother in her 4th pregnancy was taken to newborn intensive care unit. On physical examination, the abdomen was soft and had minimal distention. Bilious drainage from nasogastric catheter was detected. There was no distal gas passage on the abdominal X-ray and there were air-fluid levels suggesting proximal obstruction. The patient was operated on the postnatal first day with a preliminary diagnosis of jejunal atresia. It was seen that the small intestine returned 2 full cycles from the mesenteric root. Bowels circulation was good. Volvulus corrected. There was a significant difference in diameter in the small bowel segment about 20 cm distal from Treitz but no atresia was detected. There was malrotation. Ladd's operation was performed. He was discharged on the postoperative 7th day with full oral feeding. The patient is still in the 6th postoperative month and the follow-up is uneventful.

Conclusions: Intrauterine midgut volvulus has been associated with high mortality in the literature. Although the presented case had good results, Differential diagnosis of midgut volvulus in patients with antenatal intestinal obstruction, close prenatal follow-up, appropriate delivery and timing of surgical intervention may significantly reduce morbidity and mortality.

İntestinal obstruksiyonun nadir bir nedeni: intrauterin midgut volvulus

Amaç: İntrauterin midgut volvulus oldukça nadir görülen, hayatı tehdit edebilen ve prenatal tanısı zor bir durumdur. Bu yazıda antenatal jejunal atrezi ön tanısıyla takip edilen bir midgut volvulus olgusu sunulması amaçlandı.

Olgu sunumu: 19 yaşında annenin 4. gebeliğinden 4. canlı, 38 haftalık C/S ile 3600 gr doğan, antenatal jejunal atrezi tanısı ile takip edilen erkek bebek yenidoğan yoğum bakım ünitesine yatırıldı. Fizik muayenesinde karın yumuşak olup, minimal distansiyon dışında özellik yoktu. Nazogastrik sondadan safralı drenaj saptandı. Ayakta direk batın grafisinde distale gaz geçişi olmayıp proksimal seviyede tıkanıklık düşündürecek hava-sıvı seviyeleri mevcuttu. Hasta postnatal 1. gününde jejunal atrezi ön tanısı ile ameliyata alındı. Ameliyatta ince barsakların mezenter kökünden 2 tam tur dönmüş olduğu görüldü. Barsakların dolaşımı iyiydi. Volvulus düzeltildi. Treitz yaklaşık 20 cm distaldeki ince barsak segmentinde belirgin çap farkı olmakla birlikte atrezi saptanmadı. Malrotasyon mevcuttu. Ladd ameliyatı yapıldı. Postoperatif 7. günde tam oral beslenerek taburcu oldu. Hasta, halen ameliyat sonrası 6. ayda olup takibi sorunsuzdur.

Sonuç: Intrauterin midgut volvulus literatürde yüksek mortaliteyle ilişkilendirilmektedir. Her ne kadar sunulan olguda iyi sonuç alınmış olsa da, antenatal intestinal obstruksiyon saptanan hastalarda midgut volvulusun ayırıcı tanıda bulundurulması, yakın prenatal takip, uygun doğum ve cerrahi girişim zamanlaması gelişebilecek morbidite ve mortaliteyi büyük ölçüde azaltabilir.
