Complications of Meckel ‘s Diveriticulum in children: a 10 year experience

Aim: To evaluate patients with complications of Meckel’s Diverticulum (MD).

Method: The patients who were operated on for MD complications between 2008-2018 were evaluated retrospectively for age, gender, emergency or elective surgery, histopathological evaluation and surgical method.

Results: 62 patients (38 males, 24 females) (6 months-17,08 years), 35 underwent emergent and 27 elective surgery. 35 patients (median 7 years) who underwent emergency surgery, only 3 had preoperative diagnosis. 14 patients had invagination, 11 had internal herniation, 6 had diverticulitis and 4 had perforation. 13 patients underwent laparoscopy-assisted surgery and 22 undergone laparotomy. In 16 patients, ileal segment resection ( intussusception n=12, perforation n=2, internal herniation n=2), in 19 patients wedge resection (intussusception n=2, internal herniation n=9, diverticulitis n=6, perforation n=2) was performed. Histopathological evaluation revealed ectopic gastric tissue in 16 patients and both ectopic stomach and pancreas tissue in 6 patients. Ectopic tissue was not observed in 13 patients.

27 patients (6 months-14,25 years, median 4,73 years) who underwent elective surgery had rectal bleeding. Tc-99m pertechnetate imaging was performed to all patients. Ectopic focal substance accumulation was detected in 22 patients.. Five patients with negative scintigraphic finding were diagnosed by laparoscopy. 6 patients underwent laparotomy and 21 undergone laparoscopy-assisted surgery. In 11 patients, ileal segment resection was performed whereas wedge resection was performed in 16 patients. Histopathological examination revealed ectopic gastric mucosa in all cases.

Conclusion: The most frequent complication of MD is invagination. Whether MD contains ectopic tissue does not affect the development of complications. MD which does not contain heterotropic mucosa may cause invagination and internal herniation. Complications due to MD are usually diagnosed during surgery.

Amaç: Meckel Divertikülü’ne (MD)bağlı komplikasyon gelişimindeki risk faktörlerinin belirlenmesi

Yöntem: 2008-2018 yılları arasında MD komplikasyonu nedeniyle ameliyat edilen hastalar yaş, cinsiyet, acil veya elektif cerrahi, histopatolojik değerlendirme ve cerrahi yöntem açısından geriye dönük olarak değerlendirildi.

Bulgular: 62 hasta (38 erkek, 24 kız) (6 ay-17,08 yıl). 35 hastaya acil ve 27 hastaya elektif cerrahi uygulandı. Acil cerrahi uygulanan 35 hastanın (ortanca 7 yıl) sadece 3’ünde preoperatif MD tanısı konabildi. MD’nün hastalardan 14'ünde invaginasyon, 11'inde internal herniasyon, 6'sında divertikülit ve 4'ünde perforasyona neden olduğu görüldü. 13 hastaya laparoskopi yardımlı cerrahi, 22 hastaya laparotomi yapıldı. 16 hastada ileal segment rezeksiyonu (intussusepsiyon n = 12, perforasyon n = 2, internal herniasyon n = 2), 19 hastaya wedge rezeksiyon (intussusepsiyon n = 2, internal herniasyon n = 9, divertikülit n = 6, perforasyon n = 2) yapıldı. Histopatolojik değerlendirmede 16 hastada ektopik mide dokusu ve 6 hastada hem ektopik mide hem de pankreas dokusu tespit edildi. 13 hastada ektopik doku gözlenmedi.

Elektif cerrahi uygulanan 27 hasta (6 ay-14,25 yıl, ortanca 4,73 yıl) rektal kanama ile başvurdu. Tüm hastalara Tc-99m perteknetat görüntüleme yapıldı. Ektopik fokal madde birikimi 22 hastada tespit edildi. Negatif sintigrafik bulgusu olan beş hastaya tanısal laparoskopi ile tanı kondu. 6 hastaya laparotomi ve 21 hastaya laparoskopi yardımlı cerrahi yapıldı. 11 hastada ileal segment rezeksiyonu, 16 hastada wedge rezeksiyon yapıldı. Histopatolojik inceleme sonucunda tüm olgularda ektopik mide mukozası tespit edildi.

Sonuç: Omfalomezenterik kanal artığı olan MD’nün en sık acil müdahele gerektiren komplikasyonu invajinasyondur. MD’nün ektopik doku içerip içermemesi komplikasyon gelişimini etkilememektedir. Heterotropik mukoza içermeyen MD invajinasyon ve internal herniasyona neden olabilir. MD’ne bağlı komplikasyonlar çoğunlukla cerrahi sırasında tanı alır.
