Y-Chromosome Gonadal Dysgenesis

Y-chromosome gonadal dysgenesis (GD) is a rare cause of sexual developmental disorders. The term “Y-chromosome GD” is used for 46,XY pure gonadal dysgenesis, 45,XO/46,XY mixed gonadal dysgenesis and very rarely 47,XXY/46,XX mosaic Klinefelter syndrome. Here we aimed to evaluate the chromosome and gonad biopsy results of our Y-chromosome GD cases.

Between 2003-2019, 14 patients with gonadal dysgenesis with Y-chromosome were admitted to our hospital. The median age of the patients was 1.9 years (range, 8 days-14.9 years). Four of the patients were admitted to the neonatal period and five of them presented to puberty and after. Nine of the patients presented with ambiguous genitalia and 5 patients with amenorrhea. There were 8 cases of 45,X0 46 XY, 5 cases of 46,XY and one case of 47,XXY/46,XX. All but one patient had a gonad biopsy. Seven of the right gonads were testes, one was ovary, four were streak gonads and one of them was agenesis. Three of the left gonads were testes, two were ovaries, three were ovotestis, four were streak gonads, and one of them was agenesis. During the follow up period, it was found that one patient had gonadoblastoma and the other had dysgerminoma. Both of them were 46,XY gonadal dysgenesis patients.

Y-chromosome GD is a very heterogeneous group. Regardless of the karyotype obtained from peripheral blood, gonads may be composed in different combinations of ovary, testis, or ovotestis. Patients with Y-chromosome GD should be closely monitored for tumor development.

Y-kromozomlu gonadal disgenezis (GD), seksüel gelişim bozukluklarının nadir bir sebebidir. "Y-kromozom GD" terimi 46,XY saf gonadal disgenezi, 45,XO/46,XY karışık gonadal disgenezi ve çok nadiren 47,XXY/46,XX mozaik Klinefelter sendromu için kullanılır. Burada Y-kromozomlu GD olgularımızın kromozom ve gonad biyopsi sonuçlarını değerlendirmeyi amaçladık.

2003-2019 yılları arasında hastanemize başvuran Y-kromozomlu gonadal disgenezisli toplam 14 hasta mevcuttu. Hastaların başvuru yaşları ortanca 1,9 yıl (8 gün-14,9 yıl) idi. Hastaların 4'ü yenidoğan döneminde ve 5'i puberte ve sonrasında başvurmuştu. 9 hastada başvuru şikayeti kuşkulu genitalya, 5'i amenoreydi. Kromozom yapısı 8 hastada 45,X0/46,XY, 5 hastada 46,XY ve bir hastada 47,XXY/46,XX şeklindeydi. Hastaların biri hariç hepsinden gonad biyopsisi alınmıştı. Sağdaki gonadların 7'si testis, biri over, 4'ü çizgi gonad ve bir olguda ise gonad agenezikti. Soldaki gonadların 3'ü testis, ikisi over, 3'ü ovotestis, 4'ü çizgi gonad ve bir olguda ise gonad agenezikti. Hastaların takipleri sırasında birinde gonadoblastom, diğerinde disgerminom gelişimi saptandı, bu hastaların ikisi de 46,XY gonadal disgenezili hastalardı.

Y-kromozomlu GD oldukça heterojen bir gruptur. Periferik kandan elde edilen karyotip ne olursa olsun, gonadlar farklı over, testis veya ovotestis kombinasyonlarında oluşabilir. Y-kromozomlu GD'li olguların tümör gelişimi açısından yakın takip edilmeleri gereklidir.
