A Rare Entity in Immunocompromised Children: Fungal Appendicitis

Acute appendicitis is one of the causes of abdominal pain in children. The incidence of appendicitis in immunocompromised children has been reported to reach 4.4%. It is aimed to present a 7-year-old boy with relapse acute lymphoblastic leukemia, who developed appendicitis during the postchemotherapy neutropenic phase. He presented with complaints of abdominal pain and fewer that had started one day ago. He had tenderness at right lower abdominal quadrant and ultrasound examination revealed acute appendicitis. Current antibiotherapy was continued (meropenem, caspofungin, linezolid, voriconazole) and laparoscopic appendectomy was performed. The child was fed at postoperative 6th hour and transferred to oncology department the following day. The follow-up period was uneventful. Although appendicitis is usually treated with surgery in immunocompetent patients, in neutropenic patients, surgical treatment should be used with caution because of the increased risk of severe complications even death. Laparoscopic appendectomy should be considered as the gold standard procedure to avoid fungal dissemination.

İmmün sistemi baskılanmış çocuklarda ender bir antite: Fungal apandisit

Akut appendisit, çocuklarda karın ağrısının en sık nedenlerinden biridir. İmmün yetmezliği olan çocuklarda apandisit insidansının % 4.4'e ulaştığı bildirilmiştir. Bu çalışmada kemoterapi sonrası nötropenik faz sırasında appendisit gelişen relaps akut lenfoblastik lösemi tanılı çocuk hastanın sunulması amaçlandı. Hastanın bir gün önce başlayan karın ağrısı ve ateş şikayetleri mevcuttu. Fizik incelemesinde sağ alt kadranda hassasiyeti olan hastada ultrasonografik inceleme akut appendisit ile uyumlu bulundu. Mevcut antibiyoterapiye devam edildi ve laparoskopik appendektomi yapıldı. Çocuk ameliyat sonrası 6. saat beslendi ve postoperatif 1. gün onkoloji servisine devredildi. İzlemde postoperatif komplikasyon izlenmedi. Akut appendisit immün yetmezlikli çocuklarda genelde appendektomi ile tedavi edilse de, ölüme kadar gidebilen ciddi komplikasyon riskleri sebebi ile cerrahi kararı dikkatle verilmelidir. Laparoskopik appendektomi fungal disseminasyondan kaçınmak için altın standart cerrahi yöntemdir.
