Gastric Perforation in Newborns: Single Center Experience

Aim: Gastric perforation is a rare but life threatening problem in newborns. The aim of this study is to present gastric perforation cases in newborns of a single center.

Material and Methods: The charts of patients who underwent surgery because of gastric perforation in neonatal period between 2010 and 2018 were retrospectively reviewed. Children were evaluated by the means of prematurity, predisposing factors and surgical outcomes.

Results: There was eight newborns in the study. Five of them were boys and three were girls. Three of the cases were term (%37,5) and five of them were preterm babies(%62,5). The mean gestational age was 35 weeks (range 28-42 weeks) and the mean birth weight was 2100 grams (range 850-4600 grams). All eight patients had associated congenital anomalies. Among these patients five of them required resuscitation and positive pressure ventilation in labor due to low APGAR scores. In three cases there were premature rupture of the membrane.Two of the newborns were twins and one of the twins’ mother had preeclampsia. Three of the patients had severe cardiac anomalies and two of them had tracheoesophageal fistula. In total, seven of the patients (87,5%) had orogastric tube insertion and/or positive ventilation before the symptoms showed up. All patients were operated and the perforation repair was performed. Three of the patients were lost due to sepsisand serious additional comorbidities. There were no postoperative complications in rest of the children.

Discussion: Even though gastric perforation in neonates is a rare entity, it may seen more commonly in premature babies. It is a life threatening situation and early diagnosis, an appropriate surgical approach and neonatal intensive care may improve the prognosis.

Yenidoğanlarda Mide Perforasyonu: Tek Merkez Deneyimi

Amaç: Mide perforasyonu yenidoğanlarda ender görülen fakat hayatı tehdit eden bir problemdir. Bu çalışmanın amacı tek merkez yenidoğan mide perforasyonu deneyimlerinin sunulmasıdır.

Materyal ve Metodlar: Mide perforasyonu nedeni ile 2010-2018 yılları arasında ameliyat edilmiş olan 8 yenidoğan hastanın dosyaları geriye dönük olarak tarandı. Doğum haftaları, predispozan faktörler ve cerrahi sonuçlar değerlendirildi.

Sonuçlar: Çalışmada sekiz yenidoğan mevcuttu. Hastalardan beşi erkek üçü kızdı. Çalışmadaki hastalardan üçü term, beş tanesi premature idi. Ortalama doğum haftası 35 hafta (28-42 hafta) ve ortalama doğum ağırlığı 2100 gramdı (850-4600gram). Tüm hastalarda eşlik eden anomaliler mevcuttu. Olguların beşinde düşük APGAR nedeniyle doğumda resüsitasyon ve PPV gerekmişti, üç hastada erken membran rüptürü mevcuttu. Olguların ikisi ikiz eşiydi ve birinin annesinde preeklampsi mevcuttu. Hastaların üçünde ciddi kardiyak ek anomali, ikisinde trakeoözofageal fistül mevcuttu. Toplamda yedi hastada semptomlar başlamadan önce orogastrik tüp ve/veya pozitif basınçlı ventilasyon uygulaması yapılmıştı. Tüm hastalar ameliyat edildi ve perforasyonlar onarıldı. Hastalardan üçü sepsis ve ciddi komorbiditeleri nedeni ile kaybedildi. Diğer çocuklarda ameliyat sonrası komplikasyon görülmedi.

Tartışma: Yenidoğanlarda mide perforasyonu ender bir durum olsa da premature çocuklarda daha sık meydana gelebilir. Hayatı tehdit eden bir durumdur. Erken tanı, uygun cerrahi tedavi ve uygun yenidoğan yoğun bakım temini prognozu iyileştirebilir.
