Foreign body aspiration in children: experience in a peripheral university

INTRODUCTION

Foreign body aspiration is common during childhood, and is a leading cause of sudden child death. In this study, we aimed to highlight the importance of this subject through data from cases treated in our clinic upon the diagnosis of foreign body aspiration.

MATERIAL AND METHOD

The records of 45 cases treated upon the diagnosis of foreign body aspiration retrospectively reviewed. The cases were evaluated in terms of age, sex, type of foreign body, localization of foreign body, type of intervention, clinical and radiological findings, educational status of parents, and professions of parents.

RESULTS

A total of 45 cases were included in the study. 28 (62.2%) of them were male, and 17 (37.8%) were female. 73.3% (33 cases) of all cases were between the ages 0-3. The most common complaints of those admitted to the emergency department were cough, difficulty in breathing, and wheeze. The most common pathological image (37.8%) seen in the first posteroanterior (PA) chest radiograph of cases admitted to the emergency department with the suspicion of foreign body aspiration was unilateral air trapping in the lung. PA chest radiograph of 51.1% (23 cases) (37.8%, 17 cases) of all cases was normal. 71.1% (32 cases) of all cases underwent rigid bronchoscopy as a method of intervention. Among those undergoing bronchoscopy, foreign body was detected in 50.0% (17 cases) at the right main bronchus, in 26.5% (9 cases) at the left main bronchus, and in 11.8% (4 cases) at the trachea. 86.7% of foreign bodies were organic (hazelnut, peanut, seeds and so on). It was found that 68.4% (26 mothers) of the mothers of cases treated upon the diagnosis of foreign body aspiration were primary school graduates and 88.9% (32 mothers) were housewives. It was learned that 50% (19 fathers) of their fathers were high school graduates and 83.8% (31 fathers) were self-employed.

CONCLUSION

The approach to foreign body aspiration, which is a serious, life-threatening pathology in children, is vital. Bronchoscopic intervention should be planned if history and physical examination support the diagnosis in patients admitted to the emergency department with suspicion of foreign body aspiration.The most important step in the diagnosis of foreign body aspiration is suspicion.

Giriş

Yabancı cisim aspirasyonları çocukluk çağında sık görülür ve ani çocuk ölümleri arasında önemli bir yer tutar. Bu çalışmada kliniğimizde yabancı cisim aspirasyonu tanısıyla tedavi edilen olgularımızın verileri eşliğinde bu konunun önemini vurgulamayı amaçladık.

Gereç ve yöntem

Ocak 2013–Kasım 2018 tarihleri arasında Afyon Sağlık Bilimleri Üniversitesi Tıp Fakültesi Hastanesi Çocuk Cerrahi Servisinde yabancı cisim aspirasyon tanısı ile tedavi edilen 45 olgunun kayıtları geriye dönük olarak incelenmiştir. Olgular yaş, cinsiyet, yabancı cisim türü, yabancı cisim lokalizasyonu, girişim türü, klinik ve radyolojik bulgular, anne ve baba eğitim durumu, anne ve baba meslekleri açısından değerlendirilmiştir.

Bulgular

Toplamda 45 olgu çalışmaya dahil edildi. Bunların 28’i erkek (%62.2), 17’si kız (37.8) olarak bulundu. Olguların %73.3’ü (33 olgu) 0-3 yaş arasındaydı. Acil servise en sık geliş şikayetleri öksürük, nefes darlığı ve hırıltılı solunumdu. (%37.8) Acil servise yabancı cisim aspirasyon şüphesiyle getirilen olguların çekilen ilk posterioanterior (PA) akciğer grafilerinde karşılaşılan en sık patolojik görüntü akciğerde tek taraflı hava hapsiydi. (%37.8, 17 olgu) %51.1’inin (23 olgu) PA akciğer grafisi normaldi. Girişim olarak olguların %71,1’ine (32 olgu) rigid bronkoskopi işlemi yapılmıştır. Bronkoskopi işlemi yapılanların %50,0’sinde (17 olgu) sağ ana bronş, %26,5’ında (9 olgu) sol ana bronş, %11,8’ inde (4 olgu) trakeada yabancı cisme rastlanılmıştı. Yabancı cisimlerin %86,7’si organik’di (fındık, fıstık, çekirdek vb ). Yabancı cisim aspirasyon tanısıyla tedavi edilen olguların annelerinin % 68,4 ‘ü (26 anne) ilkokul mezunu olduğu ve %88,9’unun (32 anne) ev hanımı olduğu öğrenildi. Babalarının ise % 50 ‘sinin (19 baba) lise mezunu olduğu ve % 83,8 ‘inin (31 baba) serbest meslek sahibi olduğu öğrenildi.

Sonuç

Çocuklarda hayatı tehtit eden ciddi bir pataloji olan yabancı cisim aspirasyonuna yaklaşım çok önemlidir. Acil servise yabancı cisim aspirasyon şüphesi ile gelen hastalarda öykü ve fizik muayene tanıyı destekliyorsa bronkoskopik girişim planlanmalıdır. Yabancı cisim aspirasyonunda tanıya giden en önemli yol şüphedir.
