Our stepped laparoscopic and thoracoscopic approach in a patient with recurrent tracheoesophageal fistula, anastomotic stricture and gastroesophageal reflux

Aim: Recurrent tracheoesophageal fistula, anastomotic stricture and gastroesophageal reflux (GER) are the late complications of esophagus atresia (EA) and tracheoesophageal fistula (TEF) repair. In this study, our minimally invasive approach to a patient who has all three complications after a failed EA-TEF repair has been presented.

Case: A two-year-old girl, who has treated owing to type-3 EA-TEF repair during newborn, was admitted to our department. She was suffering from recurrent lung infection, coughing while swallowing and vomiting. From her history, it was realized that feeding jejunostomy was the initial approach but due to leakage, was switched to gastrostomy. In upper gastrointestinal series GER and TEF was detected. Stricture was confirmed resistant to dilatation. Initially antireflux surgery is planned. During laparoscopic bridectomy the old taken down ileostomy segment was stuck to anterior abdominal wall. The opened intestinal segment was repaired by primary suture thereafter Nissen fundoplication performed.

After a follow up of six months, solely gastrostomy feeding, in right-side up semi prone position, three 5 mm trocar assisted thoracoscopy was performed. Just at the time of recurrent fistula ligation, she was desaturated. Anesthesia and surgical team together decided to ended the surgery and agreed to postpone the resection and anastomosis to another session. When the position was switched to head up, it was realized that the endotracheal tube was obstructed with blood and secretion. 1.5 month after the first thoracoscopy, through the same port sides the second thoracoscopic resection and anastomosis was performed. During postoperative period leakage was observed for a short time that was treated with continue aspiration. After a follow up of 5 months she did well without any complain.

Conclusion: In conclusion, while fixing the complications of primary surgeries, minimally invasive approaches such as laparoscopy or thoracosocpy could be performed safely in pediatric age groups as well.

Nüks trakeoözefageal fistül, anastomoz darlığı ve gastroözefageal reflüsü olan olguda aşamalı laparoskopik ve torkoskopik yaklaşımımız

Amaç: Özefagus atrezisi(ÖA) ve trakeoözefageal fistül (TÖF) cerrahi tedavisi sonrasında nüks fistül, anastomoz darlığı, gastroözefageal reflü (GÖR) geç dönemde karşılaşılan komplikasyonlardır. Çalışmamızda ÖA-TÖF cerrahisi sonrasında GÖR, anastomoz darlığı ve nüks fistül gelişen olguya aşamalı mimimal invaziv yaklaşımımız sunuldu.

Olgu: Yenidoğan döneminde, tip 3 ÖA-TÖF nedeniyle opere olan iki yaşındaki olgu, beslenme sırasında öksürük, kusma, tekrarlayan akciğer enfeksiyonu yakınmaları ile başvurdu. Hastaya, beslenme amacıyla öncelikle jejunostomi yapıldığı, ancak gelişen komplikasyon nedeniyle gastrostomiye geçildiği öğrenildi. Özefagoskopide skarlı, dilatasyona yanıt vermeyen anostomoz darlığı, özefagus mide duedonum grafisinde de nüks TÖF ve GÖR saptandı. Hastaya gastrostomi bozulmadan laparoskopik Nissen Fundoplikasyonu (LNF) planlandı. LNF öncesinde bridektomi yapılırken eski jejunostomi segmentinin karın duvarından ayrılması sırasında oluşan açıklık intrakorporal sütür ile onarıldı. Sadece gastrostomiden beslenme planlanan olguya 6 ay sonra torakoskopi yapıldı. Sağ taraf yukarıda olacak şekilde, yarı yüzüstü yatar pozisyonda, 5. interkostal aralık arka aksiller çizgi, 3. interkostal aralık ön aksiller çizgi ve 4. interkostal aralıktan 3 adet 5mmlik trokar yerleştirildi. Fistül ligasyonu yapıldığı anda, akciğer havalanmasının bozulması üzerine işlem sonlandırıldı ve rezeksiyon anastomoz ikinci seansa bırakıldı. Hasta pozisyonu düzeltildiğinde endotrakeal tüpün kan ve sekresyon ile tıkandığı gözlendi. Daha sonra postoperatif (PO) 1. ayda aynı port girişleri kullanılarak özefagusa torakoskopik rezeksiyon anastomoz yapıldı. PO erken dönemde özefagusdan olan kaçağı kısa süreli nazogastrik aspirasyon ile kapandı. PO 5. ayında ağızdan beslenen, ek sorunu olmayan olgunun izlemine devam edilmektedir.

Yorum:Sonuç olarak ikincil cerrahi girişimlerde sıralama prognozda etkindir. Laparoskopi ve torakoskopi gibi minimal invaziv yaklaşımların çocuklarda ikincil cerrahi girişimlerde güvenle tercih edilebileceği kanısındayız.
