Intrathyroidal Ectopic Thymus

Introduction:Intrathyroidal ectopic thymus (IET) is usually found in the thyroid as a fusiform nodule. IET tends to shrink over time due to the natural involution of the thymus. The objective of this study was to describe the frequency of IET and follow-up results of thyroid US in children.

Material-Methods:From the hospital database, "thymus/thymic tissue" words were searched in thyroid US reports of children aged 0-18 years from the last 3years. Among these reports, children with pre-diagnosis of IET and follow-up using US were included in this study. First and last US reports were recorded. Between first and last US reports, if the difference of IET diameter is ≥1cm, it was accepted as “involution” and if this difference is <1cm, it was accepted as “stabile”. Length of follow-up was established by the number of months between the date of first and last US.

Results:These words were found in 69 of the 1974 children's thyroid US. Thirty-three children were excluded because of 17 reports that lacked sufficient information, 10 children with an extrathyroidal thymic tissue, and 6 children that lacked of follow-up. Eventually, 36 children(15M, 21F, 6years(0-15y)) with 37 typical sonographic features of IET lesion was included. In the first US, the median transvers diameter 5mm(1.5-18mm), median antero-posterior diameter 3mm(1.5-7mm) was found. The dimension of IET was ≤1cm in 84%(n=31) and >1cm in 16%(n=6) IET. The median length of follow-up was 9 months(1-35). In the last US “involution” and “stabile” was found in 22(59%) and 15(41%) IET (respectively). All of the >1cm IETs and 16(52%) of the rest IETs were shrunk at follow-up US. It was found that the decrease in IET size started from the age of 1 year and peaked during adolescence.

Conclusion:IET is rarely found in thyroid US and can be confused as thyroid nodule. Differentiation of other thyroid lesions can be made by US and when reduction in size by the follow-up US was found, the diagnosis of IET was supported. When thyroid US is performed with appropriate intervals, some invasive procedures will be avoided in these children.

İntratiroidal Ektopik Timus

Giriş: İntratiroidal ektopik timus (IET), tiroid içinde genellikle fusiform bir nodül şeklinde bulunur. IET, timusun doğal regresyon seyri nedeniyle zamanla küçülme eğilimindedir. Çalışmamızda çocuklarda yapılan tiroid US'lerindeki IET sıklığı ve takip sonuçları değerlendirildi.

Araç-Gereç:Hastane veri tabanından son 3 yıllık döneme ait 0-18 yaş arası çocuklara yapılan tiroid US raporlarında “timus/timik doku” kelimeleri tarandı. Raporlarında tiroid içinde IET bulunan ve takiplerine gelen olgular çalışmaya dahil edildi. İlk ve son US arasında boyutta azalma 1cm’den fazlaysa “regresyon” ve 1cm’den azsa “stabil” olarak değerlendirildi. İlk ve son US tarihi arasındaki zaman takip süresi olup, ay olarak hesaplandı.

Bulgular:3 yılda 1974 çocuğa tiroid US yapılmış olup, bunlar arasında 69 çocuğun raporunda “timus/timik doku” kelimeleri bulunuyordu. Bunlardan 10'u tiroid dışında lokalizeydi. Raporlardan 17'sinde yetersiz bilgi olması ve 6 çocuğun kontrole gelmemesi nedeniyle toplamda 23 rapor daha dışlandı. Sonuçta 36 hastada (15 E, 21 K, median yaşı 6y (0-15)), 37 tipik sonografik özellikleri olan lezyon çalışmaya dahil edildi (bir hastada iki IET vardı). İlk US’de IET transvers boyutu median 5mm (min-maks:1,5-18mm), ön-arka boyutu median 3mm (min-maks: 1,5-7mm) idi. IET boyutu %84'ünde (n=31) 1cm'den küçük, %16'sında (n=6) 1cm'den büyüktü. Takip median süresi 9 ay (1-35ay) idi. Son US incelemelerinde IET’lerin 22’sinde (%59) regresyon izlenirken, 15’inde (%41) lezyon stabildi. 1cm'den küçük IET'lerin %52'si (n=16), büyüklerin tamamı takiplerde regrese olmuştu. IET boyutunda azalmanın 1 yaşından itibaren başladığı, ergenlik döneminde pik yaptığı bulundu.

Sonuç:Sonografik olarak IET çocuklarda nadiren görülür ve diğer tiroid nodülleriyle karışabilir. Sonografik görüntüsü ile diğer lezyonlardan ayırt edilebilmesi mümkündür ve takiplerde boyutunun azaldığının gösterilmesi de tanıyı destekler. Olguların uygun aralıklarla tiroid US kontrolünün yapılması ile çocuklarda invaziv girişimlerin önüne geçilebilir.
