Staged excisional treatment of a complicated cutaneous lymphangioma (lymphangioma circumscriptum) involving the perianal region

Background/Aim: Cutaneous lymphangioma (lymphangioma circumscriptum, LC) is an infrequently seen hamartomatous lesion usually located at proximal part of the extremities. Genital or perianal skin involvement is extremely rare. In this case report, it is aimed to draw attention to the diagnosis and treatment of LC, which commonly involves the perianal region in an adolescent.

Case: A 16-year-old male patient presented with swelling, wound and pain around the anus. It was learned that the first operation was performed when she was 3 years old and she underwent two more operations (I. Total excision, II. Total excision + Grafting) in another center at the age of 8 and 11 due to recurrence of the disease. After these operations failed, the patient was referred to our center 5 years later. Physical examination of the patient revealed condylomatous (verrucous) lesions involving the right perianal region. Considering the possibility of HPV infection, the patient was consulted to dermatological department and incisional biopsy was performed. Histopathological examination was reported as LC. Staged excision of the lesion was decided to reduce surgical morbidity (surgical site infection, sphincter damage, colostomy requirement). The patient underwent 3 partial excisions within 9 months and finally total excision was succeeded. No recurrence was detected at the 1-year follow-up.

Conclusion: In perianal LC cases that are not treated properly or present late, the lesions may merge and become condylomatous. The treatment of enlarging and deepening lesions can be complicated. In these cases, staged excisions may reduce the likelihood of postoperative complications and increase the success of treatment.

Perianal bölgeyi tutan komplike seyirli bir kutanöz lenfanjiom (lymphangioma circumscriptum) olgusunda aşamalı eksizyonel tedavi

Önbilgi/Amaç: Kutanöz lenfanjiom (lymphangioma circumscriptum, LC) genellikle ekstremitelerin proksimalinde yerleşen ve az görülen hamartomatöz bir lezyondur. Genital ve perianal deriyi tutması oldukça nadirdir. Bu olgu sunumunda, ergen bir bireyde perianal bölgeyi yaygın olarak tutan LC’un tanı ve tedavisine dikkat çekilmesi amaçlanmıştır.

Olgu: 16 yaşındaki erkek hasta anüs çevresinde şişlik, yara ve ağrı yakınması ile başvurdu. İlk ameliyatının 3 yaşındayken yapıldığı, hastalığın tekrarlaması nedeniyle 8 ve 11 yaşlarında başka bir merkezde iki ameliyat daha geçirdiği (I. Total eksizyon, II. Total eksizyon+Greftleme) öğrenildi. Bu ameliyatların da başarısız olması üzerine hasta 5 yıl sonra merkezimize sevkedilmiş. Başvurudaki fizik incelemede sağ perianal bölgeyi yaygın olarak tutan kondülomatöz (verrüköz) lezyonlar görüldü. HPV infeksiyonu olabileceği düşünülerek dermatolojik görüş alındı ve insizyonel biyopsi uygulandı. Histopatolojik inceleme sonucu LC olarak rapor edildi. Cerrahi morbiditeyi (cerrahi alan infeksiyonu, sfinkter hasarı, kolostomi gereksinimi) azaltmak için lezyonun aşamalı olarak eksizyonuna karar verildi. Olguya 9 ay içerisinde 3 defa parsiyel eksizyon uygulanarak sonuçta total eksizyon gerçekleştirildi. Hastanın 1 yıllık izleminde rekürrens saptanmadı.

Sonuç: Uygun tedavi edilmeyen ya da geç başvuran perianal LC olgularında, lezyonlar birleşerek kondülomatöz görünüm kazanabilirler. Genişleyen ve derinleşen lezyonların tedavisi komplike seyredebilir. Bu olgularda, aşamalı eksizyon postoperatif komplikasyon olasılığını azaltarak tedavinin başarısını artırabilir.
