Colon Located İnflammatory Myofibroblastic Tumor: Case Report

Inflammatory myofibroblastic tumor (IMT) is a rare benign childhood tumor that may arise from various regions, mostly the lungs, mesentery, liver, genitouriner tract and retroperitoneum. Colonic involvement is quite rare. We report a case with colon located IMT. A 3-year-9-month-old male patient admitted with a complaint of abdominal pain for 2 days. The patient had fullness and tenderness in the right lower quadrant in physical examination. Laboratory tests were unremarkable. Ultrasound (USG) revealed a solid hypoechoic mass lesion with a size of 6.5x3.5 cm in the right lower quadrant of the abdomen. Computerised tomography (CT) showed a solid mass with a size of 40x70x75 mm and heterogenous density. The patient was operated with a suspect of intestinal lymphoma or GIST. Intraoperatively, a solid fragile mass, with the size of 6x5 cm and not associated with the lumen, was observed and totally excised. The patient was capable of oral intake on the first postoperative day and was discharged on the second postoperative day. The pathology of the mass was reported as inflammatory myofibroblastic tumor. The patient's follow-up was uneventful.

Inflammatory myofibroblastic tumor is defined as a tumor composed of a mixture of spindle-shaped myofibroblastic tumor cells and inflammatory cells. Although it can be seen at any age, it is frequently seen in childhood. Malignant transformation is rare but possible. Colonic involvement has been reported very rarely in the literature. The treatment is surgical removal of the mass. Long-term follow-up should be kept in mind because of metastic potential and local aggressive behavior.

Kolon Duvarında İnflamatuar Miyofibroblastik Tümör: Olgu Sunumu

İnflamatuar myofibroblastik tümör (İMT), genellikle çocukluk çağında görülen nadir bir benign tümördür. Sıklıkla akciğerde, mezenterde, karaciğerde, genitouriner traktta, retroperitonda bildirilmesine rağmen birçok farklı lokalizasyonda bulunabilir, kolon tutulumu oldukça nadirdir. Bu olgu sunumunda kolon yerleşimli IMT olgusu sunulmaktadır. 3 yaş 9 aylık erkek hasta 2 günlük karın ağrısı şikayeti ile başvurdu. Yapılan fizik muayenesinde sağ alt kadranda dolgunluk ve ele gelen kitle mevcuttu. Laboratuvar testlerinde özellik saptanmadı. Yapılan abdominal ultrasonografide (USG) sağ alt kadranda 6,5x3,5 cm boyutta solid hipoekoik kitlesel lezyon tanımlandı. Bilgisayarlı tomografide (BT) batın sağ alt kadranda 40x70x75mm boyutlarında ileoçekal düzeyde heterojen solid kitle (intestinal lenfoma? GIST?) olarak tanımlandı. İntestinal lenfoma, GIST şüphesiyle kitlesinin eksizyonu planlandı. Ameliyatta transvers kolon başlangıcında, barsak lümeni ile ilişkisi olmayan, 6x5 cm büyüklüğünde sert frajil bir kitle olduğu görüldü. Kitle total olarak eksize edildi. Postoperatif 1. gününde oral başlanan hasta, postoperatif 2. gününde taburcu edildi. Kitlenin patolojisi inflamatuvar myofibroblastik tümör olarak raporlandı. Hastanın takipleri sorunsuz devam etti.

İnflamatuar myofibroblastik tümör iğsi şekilli myofibroblastik tümör hücreleri ve inflamatuar hücrelerin karışımından oluşan bir tümör olarak tanımlanır. Her yaşta görülebilmesine rağmen, sıklıkla çocukluk çağında görülür. Hastamızda olduğu gibi kolon tutulumu oldukça nadir bildirilmiştir. Tedavisi cerrahi olarak kitlenin çıkarılmasıdır. Nadiren de olsa metastaz yapabilme ve lokal agresif davranış gösterebilme potansiyeli nedeniyle hastaların uzun dönem takibe alınması gerektiği unutulmamalıdır.
