CHOLEDOCAL CYSTS IN CHILDREN: A SINGLE CENTER 10-YEAR EXPERIENCE

Aim: We aimed to present the preoperative and postoperative evaluations of patients who underwent surgery due to the choledochal cysts.

Materials and Methods: The records of patients who were operated for choledochal cyts in our hospital between January 2009 and May 2019 were analyzed retrospectively

Results: A total of 26 patients with choledochal cyst were treated within 10 years. The ages of the patients ranged from 20 days to 15 years (mean 50 months). 15 patients were female and 11 were male. Eleven of the patients presented with jaundice (8 had a palpable mass in the right upper quadrant), 14 of the patients with abdominal pain, and the other one patient was referred to our clinic for pancreatitis. Ultrasound was performed in all patients. MDCT in 3 patients and MRCP was performed as further imaging studies in 21 patients. Twenty-four patients (92%) had type I cysts and 2 (8%) patients had type II, according to Todanı’s classification. The average cyst diameter was 3.7 cm. In all but two patients, total cyst excision and Roux-en-Y hepaticojejunostomy were chosen. Two patients who had type II cyst was treated by cyst excision. The average time of hospital stay was 7 days. In the early postoperative period, wound infection was detected in one patient. In follow-up period of our patients except for one patient were uneventfull. The one patient operated due to the ileus eight months later.

Conclusion: Choledochal cyst should not be ignored in differential diagnosis in patients presenting with jaundice in newborn and infant period and with abdominal pain in further age.

ÇOCUKLARDA KOLEDOK KİSTLERİ: TEK MERKEZ 10 YILLIK DENEYİM

Amaç: Çalışmamızda 2009-2019 yılları arasında koledok kisti nedeniyle opere ettiğimiz 26 hastanın preoperatif ve postoperatif değerlendirmelerini sunmayı amaçladık.

Gereç ve Yöntem: Ocak 2009 ile Mayıs 2019 tarihleri arasında hastanemizde koledok kisti tanısı ile ameliyat edilen hastaların kayıtları geriye dönük olarak incelendi.

Bulgular: Koledok kisti olan toplam 26 hasta 10 yıl içinde tedavi edildi. Hastaların yaşları 20 gün ile 15 yıl arasında değişmekteydi (ortalama 50 ay). Hastaların 15'i kadın, 11'i erkek idi. Hastaların 11'i sarılık şikayeti ile getirildi (8'inde sağ üst kadranda palpe edilen kitle), 14'ünde karın ağrısı vardı ve diğer bir hasta ise pankreatit nedeniyle kliniğimize sevk edildi. Tüm hastalara USG yapıldı. 21 hastaya MR, 3 hastaya BT görüntüleme yapıldı. Todani sınıflamasına göre, 24 hastada tip 1, 2 hastada tip 2 koledok kisti saptandı. Tip 2 olan iki hasta dışında tüm hastalarda cerrahi yöntem olarak total kist eksizyonu ve Roux-en-Y hepatikojejunostomi yapıldı. Tip 2 koledok kisti olan 2 hastada sadece kist eksize edildi. Ortalama yatış süresi 7 gün idi. Postoperatif erken dönemde bir hastada yara enfeksiyonu saptandı. Hastaların takip süresince bir hasta dışında herhangi bir sorun ile karşılaşılmadı, bir hasta 8 ay sonra ileus nedeniyle opere edildi.

Sonuç: Yenidoğan ve infant döneminde sarılık, ileri yaşta karın ağrısı ile başvuran hastalarda ayırıcı tanıda koledok kisti göz ardı edilmemelidir.
