URETHROCUTANEOUS FISTULA: 20 YEARS’ SINGLE CENTER EXPERIENCE

Background/Aim: Urethrocutaneous fistula is the most common complication in the treatment of hypospadias, and there are controversial issues for its etiopathogenesis and treatment. In this study, we aimed to share our experience with patients who developed urethrocutaneous fistula after hypospadias repair.

Materials and Methods: 97 patients who underwent surgery for urethrocutaneous fistula after hypospadias repair in the last 20 years and whose treatment was completed were retrospectively evaluated. Demographic data, hypospadias and fistula related clinical findings, characteristics of repair method, postoperative follow-up and results were reviewed.

Results: Between 1998-2018, 730 patients underwent surgery for hypospadias. The mean age of patients during primary hypospadias repair was 54 months (18 months-168 months), the mean age at the time of fistula repair was 104 months (20 months-204 months), and the mean postoperative hospital stay was 4.5 days (1-10 days). In our series, the mean number of fistula repairs per patient was 1.2 (118/97). Out of the patients with fistula; 51.5% had distal and 48.5% had proximal hypospadias; an additional disease/syndrome of genitourinary system was found in 10%.

In our series, the incidence of multiple fistulas was 18.5% (18/97). This ratio; 0% for 13-24 months, 22% for 25-60 months, 20.6% for 6-12 years, 14.2% for 13-18 years of age; 12% (6/50) for distal hypospadias repair and 25,5% (12/47) for proximal.

The incidence of recurrent fistula in our series was 17.5 (17/97). This ratio; 22.2% in the 13-24 months of age, 8.8% in the 25-60 months, 31.0% in the age of 6-12 years, 14.2% in the 13-18 years; 22.0% for distal hypospadias and 12.8% for proximal; 16,1% (10/62) in Foley catheters used, 21,8% (7/32) in feeding tube used, and 0% (0/3) in others; 20,2% (16/79) in single fistula and 5,5% (1/18) in multiple fistulas. No recurrent fistula was detected in patients with genitourinary system disease.

Conclusion: In our series, the period between primary hypospadias repair and fistula repair was prolonged. Multiple fistulas were markedly more common between 2-12 years of age and in patients with proximal hypospadias. Recurrent fistulas are frequently seen between the ages of 6-12 and may not be associated with the catheter used. Because there are many factor affecting outcome of patients with fistula, these cases should be examined in more detail.

ÜRETROKUTANÖZ FİSTÜL ONARIMI: 20 YILLIK TEK MERKEZ DENEYİMİ

Önbilgi/Amaç: Üretrokutanöz fistül, hipospadiaslı olguların tedavi sürecinde en sık karşılaşılan komplikasyon olup etyopatogenezi ve tedavisinde oldukça tartışmalı noktalar vardır. Bu çalışmada, hipospadias onarımından sonra üretrokutanöz fistül gelişen olgularla ilgili deneyimlerimizin paylaşılması amaçlanmıştır.

Gereç ve Yöntem: Son 20 yılda hipospadias onarımı sonrası gelişen üretrokutanöz fistül nedeniyle kliniğimizde ameliyat edilen ve tedavisi tamamlanmış 97 olgu geriye dönük olarak değerlendirildi. Olgulara ait demografik veriler, hipospadias ve fistüle ait klinik bulgular, onarım yöntemine ait özellikler, postoperatif izlem ve sonuçları gözden geçirildi.

Bulgular: 1998-2018 yılları arasında kliniğimizde toplam 730 hasta hipospadias nedeniyle ameliyat edildi (distal hipospadias: 517 hasta, proksimal hipospadias: 213 hasta). Fistül onarımı yapılan hastalarımızın primer hipospadias onarımı sırasındaki ortalama yaşları 54 ay (18 ay-168 ay), fistül onarımı anındaki ortalama yaşı 104 ay (20 ay-204 ay), postoperatif ortalama yatış süresi 4,5 gün (1-10 gün) olarak bulundu. Serimizde hasta başına düşen ortalama fistül onarım sayısı 1,2 idi (118/97). Fistül saptanan olguların; %51.5’i distal, %48.5’si proksimal hipospadiaslıydı; %10’unda genitoüriner sistemi ilgilendiren bir ek hastalık/sendrom saptandı.

Serimizde multipl (tanı anında iki veya daha fazla) fistül görülme oranı %18,5 (18/97) idi. Bu oran; 13-24 aylıklarda %0 (0/9), 25-60 aylıklarda %22 (10/45), 6-12 yaşlarında %20,6 (6/29), 13-18 yaşlarında %14,2 (2/14); distal hipospadias onarımları için %12 (6/50), proksimaller için %25,5 (12/47) olarak bulundu.

Serimizde yineleyen fistül görülme oranı 17,5 (17/97) idi. Bu oran; 13-24 aylıklarda %22,2 (2/9), 25-60 aylıklarda %8,8 (4/45), 6-12 yaşlarında %31,0 (9/29), 13-18 yaşlarında %14,2 (2/14); distal hipospadias için %22,0 (11/50), proksimal için %12,8 (6/47); Foley sonda kullanılanlarda %16,1 (10/62), beslenme sondası kullananlarda %21,8 (7/32), diğerlerinde %0 (0/3); tek fistül olanlarda %20,2 (16/79), multipl olanlarda %5,5 (1/18) idi. Genitoüriner sistemi ilgilendiren ek hastalık/sendrom saptananlarda yineleyen fistül saptanmadı.

Sonuç: Serimizde primer hipospadias onarımı ile fistül onarımı arasındaki süre uzamıştır. Multipl fistüller, 2-12 yaş aralığında ve proksimal hipospadiaslı olgularda belirgin olarak daha fazla görülmüştür. Yineleyen fistüllerin ise sıklıkla 6-12 yaş aralığında görüldüğü ve kateter kullanımı ile ilişkili olmayabileceği düşünülmüştür. Fistüllü hastaların prognozunu etkileyebilecek çok sayıda etmen olduğundan bu olguların daha ayrıntılı olarak irdelenmesi gerekmektedir.
