Nissen antireflux surgery in children with neurological impairment and scoliosis: our clinical experience

Aim:The incidence of gastroesophageal reflux in children with neurological deficits is up to 75%. Children with neurological deficits are a significant amount of group among to children who requires antireflux surgery. Orthopedic problems such as scoliosis often coexist in these children. In antireflux surgery, laparoscopic methods are used more frequently in recent years. In our study, it was aimed to present our surgical experience in children with neurological impairment and scoliosis requiring antireflux surgery.

Methods:Patients with neurological deficits who underwent Nissen Fundoplication (NF) procedure in our clinic from january 2014 to june 2020, were reviewed retrospectively.

Results:NF was applied to 44 patients with neurological deficits. Three patients excluded owing to radiography was inavailable. There were 21 male and 20 female children with a mean age of 53,2 months. Underlying pathology was epilepsy (%51,2) and cerebral palsy (34,1%). Scoliosis was detected in 12 (29.2%). Two patients underwent open NF and gastrostomy, while 10 patients underwent laparoscopic NF and gastrostomy. The average duration of laparotomy was 127.5 minutes; meanwhile laparoscopy was 181. There were no perop complications in laparoscopy, and there was no need to laparotomy. Except one patient, others were followed up in the postoperative (PO) intensive care unit for an average of three days, started feeding from gastrostomy at an average of 48 hours. One patient who underwent laparotomy, need second look on the day of 6th because of acute abdomen. Wound infection developed around the gastrostomy in early PO in two. In the follow-up; six (50%) had no active complaints, pneumonia continued in one, results could not be achieved in two, and three patients died from non-surgical reasons.

Conclusion: In the light of the results, nowadays the scoliosis is not an absolute contraindication for laparoscopic NF.We think that the duration of operation will decrease with an increasing surgeon experience.

Nörolojik defisitli ve skolyozu bulunan çocuklarda Nissen antireflü cerrahi deneyimlerimiz

Amaç: Gastroözofageal reflü, bebek ve çocuklarda yaygın görülen bir sorundur. Nörolojik defisitli çocuklarda gastroözofageal reflü görülme insidansı %75’e kadar çıkar. Antireflü cerrahi gerektiren çocuklar içerisinde nörolojik defisitli çocuklar önemli bir grubu oluşturur. Bu çocuklarda sıklıkla skolyoz gibi ortopedik sorunlar birlikte bulunmaktadır. Antireflü cerrahide son yıllarda laparoskopik yöntemler daha sık kullanılmaktadır. Çalışmamızda antireflü cerrahi gerektiren nörolojik defisitli ve skolyozu bulunan çocuklardaki cerrahi deneyimimizin sunulması amaçlanmıştır.

Yöntem: Kliniğimizde ocak 2014-haziran 2020 yılları arası Nissen Fundoplikasyonu (NF) uygulanmış nörolojik defisitli hastaların kayıtları geriye dönük incelendi.

Bulgular: Belirtilen tarihler arasında 44 nörolojik defisitli hastaya NF uygulanmıştır. Sistemde grafisine ulaşılamayan 3 hasta çalışma dışı bırakıldı. Çalışmaya dahil edilen 41 hastanın 20'si kız, 21'i erkekti. Cerrahi uygulama zamanındaki ortalama yaş 53,2 aydı. Hastaların %51,2'sinin primer hastalığı epilepsi, %34,1'inin serebral palsi idi. Hastaların 12’sinde (%29,2) grafide skolyoz saptandı. Bu hastaların iki tanesine açık NF ve açık gastrostomi, 10 hastaya laparoskopik NF ve laparoskopik gastrostomi uygulandı. Açık operasyonların ortalama süresi 127,5 dakika; laparoskopik operasyonların ortalama süresi 181 dakika bulundu. Hiçbir hastamızda laparoskopik yönteme bağlı perop komplikasyon gelişmemiş ve açık yönteme dönülmesi gerekmemiştir. Bir hasta hariç, hastaların hepsi postoperatif (PO) yoğun bakım ünitesinde ortalama üç gün takip edildi. Ortalama 48. saatte gastrostomiden beslenmeye başlandılar. Açık cerrahi uygulanan bir hastada PO 6.gününde akut batın tablosu gelişmesi üzerine tekrar laparotomi uygulandı. İki hastada PO erken dönemde gastrostomi çevresinde yara enfeksiyonu gelişti. Laporoskopik cerrahi uygulanan 12 skolyozlu hastanın takiplerinde; altısında (%50) aktif yakınma kalmadı; bir hastada aspirasyon pnömonisi kliniği devam etti. İki hastanın geç dönem takip sonuçlarına ulaşılamadı. Üç hasta antireflü cerrahi dışı sebeplerle kaybedildi.

Sonuç: Laparoskopik NF geçmişte skolyozlu hastalar için kontrendike görülse de günümüzde mutlak kontrendikasyon değildir ve başarı ile uygulanabilir. Artan hekim deneyimleri ile birlikte uzun operasyon sürelerinin azalacağını düşünmekteyiz.
