The laparoscopic excision of the pyloric duplication cyst in a child

Aim: Digestive system duplications are relatively rare congenital anomalies that are found everywhere from mouth to anus and may be symptomatic or detected by chance. To our knowledge, a congenital pyloric duplication cyst is extremely rare. Almost all of the published cases were treated with laparotomy. Here, we present a 6-year-old girl diagnosed with pyloric duplication cyst treated with laparoscopic total surgical excision.

Case report and method: In the abdominal sonography of a 6-year-old girl who had complaints of abdominal pain and vomiting for two years, a cyst with a wall thickness of 2 mm measured in the gallbladder adjacent, which is thought to be caused by a stomach wall of approximately 3.9x2.4 cm, was detected. The physical examination of the patient was normal with normal laboratory values. In MRI, a detailed radiological examination was reported that it originated from the stomach wall. Pyloroduodenal lumen was intact in the upper gastrointestinal system endoscopy and observed no compression. After her complaints continued, the laparoscopy was performed. with a transumbilical camera and two working ports. In laparoscopy, an over-vascularized cystic structure was observed in the pyloric region and initially aspirated. After the decision that the cystic structure not related to the pylorus unroofing was performed, then highly adherent mucosa was peeled off. Omentopexy was performed without opening the stomach or duodenum. There was no complication during the follow-up of the patient. Histopathological evaluation was reported as gastric mucosa.

Conclusion: The presented case is the first child whose pyloric duplication cyst excised by laparoscopy without gastric opening. The cyst which had a common wall with the pylorus was excised effectively and safely albeit difficult by totally removing mucosa, and by unroofing.

Çocukta pilorik duplikasyon kistinin laparoskopik eksizyonu

Amaç: Sindirim sistemi duplikasyonları, ağızdan anüse kadar her yerde bulunan, nispeten nadir gözlenen konjenital anomalilerdir ve semptomatik olabilir veya tesadüfen saptanabilirler. Bildiğimiz kadarıyla konjenital pilorik duplikasyon kisti oldukça nadirdir. Yayınlanan vakaların neredeyse tamamı laparatomi ile tedavi edilmiştir. Burada, laproskopik total cerrahi eksizyon ile tedavi edilen pilorik duplikasyon kisti tanısı konmuş 6 yaşında kız hasta sunmaktayız.

Olgu sunumu ve yöntem: İki yıldan beri karın ağrısı ve kusma şikâyetleri olan 6 yaşındaki kız olgunun çekilen batın sonografisinde, safra kesesi komşuluğunda yaklaşık 3,9x2,4cm boyutlarında mide duvarından kaynaklandığı düşünülen, duvar kalınılığı 2 mm ölçülen, kist saptanmış. Fizik muayenesi normal olan olgunun laboratuar değerleri normal sınırlardaydı. Ayrıntılı radyolojik incelemede MRI’da mide duvarından kaynaklandığı rapor edildi. Üst gastrointestinal sistem endoskopisinde piloroduodenal lümen intakttı, bası yoktu. Şikayetleri devam etmesi üzerine laparoskopi yapıldı. Transumblikal kamera ve iki çalışma portu ile yapılan laparoskopide, pilorik bölgede üzeri aşırı vaskülarize kistik yapı önce aspire edildi. Pilor ile ilişkili olmadığı gözlenen yapıya unroofing yapılarak, ileri derecede yapışık mukozası soyuldu. Mide veya duodenum açılmadan omentopeksi yapıldı. Ameliyat sonrası dönemi sorunsuz geçen hastanın takiplerinde bir komplikasyon gelişmedi. Histopatolojik değerlendirilmesi, mide mukozası olarak rapor edildi.
