SACROCOCCYGEAL EXTRASPINAL MYXOPAPILLARY EPENDYMOMA: CASE REPORT

Introduction:

Myxopapillary ependymomas are common glial tumors in the lumbosacral region. They are usually located in the presacral area. Postsacral location is very rare. A patient who was operated for a sacrococcygeal mass is presented.

Case:

An 11-year-old male patient presented with a mass in the coccyx for 4 years and grew over time. On his physical examination, a subcutaneous, semi-mobile, approximately 4x3 cm mass was palpable with a slightly bluish color in the intergluteal groove. The patient’s physical examination findings were normal. On USG, a 39x18 mm solid mass with thin septa in the subcutaneous fatty tissue anterior to the distal sacral coccygeal vertebrae, and a 13x7 mm lobular lesion with the same echogenicity and a structure that may be a pedicle posteriorly was observed. The lesion was related with the coccyx. In the sacral Magnetic Resonance Imaging taken for differential diagnosis of the patient, who was initially diagnosed with hemangioma, a 43x36 mm mass with circumferential contrast enhancement was detected in the cutaneous-subcutaneous part of the inferior part of the coccyx. The patient's routine blood tests and tumor markers were normal. The mass was totally excised with the coccyx suspected for solid component at the end of the mass. Pathologically, the tumor cells were reported as myxopapillary ependymoma due to positive immunoreactivity with GFAP, S-100 and Vimentin in the immunohistochemical study. Atypia and mitosis were not observed. The patient was followed up with pediatric oncology.

Conclusion:

Myxopapillary ependymomas are almost always seen at the distal spinal cord level. Very rarely, they may develop extraspinally and be located in the sacrococcygeal and presacral regions. Preoperative diagnosis of these lesions is difficult. Total mass excision with coccyx in radiologically atypical masses is extremely important in long-term disease-free survival.

SAKROKOKSİGEAL EKSTRASPİNAL MİKSOPAPİLLER EPENDİMOM: OLGU SUNUMU

GİRİŞ:

Miksopapiller ependimomlar lumbosakral bölgede nispeten sık görülen, çoğunlukla preskaral yerleşimli glial tümörlerdir. Postsakral yerleşim son derece nadirdir. Sakrokoksigeal kitle nedeniyle ameliyat edilen bir hasta sunulmuştur.

OLGU:

11 yaşında erkek hasta, kuyruk sokumunda yaklaşık 4 yıldır var olan ve zaman içerisinde büyüyen kitle nedeniyle başvurdu. Fizik muayenesinde intergluteal olukta, subkutan yerleşimli, semimobil, yaklaşık 4x3 cm boyutlu üzerinde hafif mavimsi renk değişikliği olan kitle ele gelmekteydi. Ultrasonografide distal sakrokoksigeal omurların anteriorunda yerleşmiş cilt altı yağlı dokuda 39x18 mm boyutlarında içinde ince septalar olan solid kitle ve bunun posteriorunda 13x7 mm aynı ekojenitede lobuler bir lezyon ve pedikül olabilecek yapı izlendi. Lezyon koksikse uzanım göstermekteydi. Radyolojik olarak hemanjiom düşünülen hastanın ayırıcı tanı amaçlı çekilen sakral MRG’sinde de koksiks inferior kesiminde cilt-cilt altı yerleşimli 43x36 mm, çevresel kontrast tutulumu olan kitle saptandı. Hastanın rutin kan tetkikleri ve tümör belirteçleri normaldi. Muayene ile daha çok teratom olabileceği düşünülen lezyon ameliyatta alt ucunun solid olması sebebiyle koksiks ile beraber total olarak çıkartıldı. Patolojik olarak immunhistokimyasal çalışmada tümör hücreleri GFAP, S-100 ve Vimentin ile pozitif immunreaktivite göstermesi sebebiyle miksopapiller ependimom olarak raporlandı. Atipi ve mitoz gözlenmedi. Hasta, çocuk onkoloji ile beraber takibe alındı.

SONUÇ:

Miksopapiller ependimomlar hemen her zaman distal omurilik seviyesinde görülürler. Çok nadir olarak ekstraspinal gelişip sakrokoksigeal ve presakral yerleşimli olabilirler. Bu gibi lezyonlara ameliyat öncesi tanı konulması zordur. Radyolojik olarak atipik kitlelerde koksiksle beraber total kitle eksizyonu uzun dönem hastalıksız sağkalımda son derece önemlidir.
