Robotic Soave cuff excision in a patient with obstructive symptoms after transanal endorectal pull-through (TEP)

A long aganglionic Soave cuff could cause obstructive symptoms in some of the patients after transanal endorectal pull-through (TEP). Secondary operations in this region could be challenging due to difficult exposure of the deep pelvis and adjacent hard adhesions. We aimed to present our experiences with an obstructive Soave cuff excised with robotic surgery.

A 16-year-old girl was admitted with obstructive symptoms caused by a long residual seromuscular cuff demonstrated on imaging studies. Firstly, a laparoscopic cuff “incision” was performed but symptoms recurred in the early postoperative period. Therefore, a tertiary procedure was conducted by robotic surgery to “excise” the Soave cuff. Robotic surgery gave the advantages of an excellent 3-D exposure and ergonomic freedom with 540-degrees flexibility during surgery. Residual Soave cuff was identified and separated from levator muscle fibers and the anterior portion of 270-degrees of the cuff was excised safely without injuring surrounding organs. Obstructive symptoms were resolved and the patient started passing stool daily after surgery.

Working in the deep pelvis or performing a tertiary operation on an organ with hard adhesions can be challenging using conventional techniques. Robotic surgery is seen as a safe and efficient surgical option in patients who require the excision of obstructive seromuscular cuff after TEP.

Robotik Soave cuff eksizyonu ile transanal endorectal pull-through (TEP) sonrası obstrüksiyon tedavisi

Amaç: Transanal endorectal pull-through (TEP) sonrası bırakılan uzun aganglionik Soave cuff bazı hastalarda obstrüktif semptomlara neden olabilmektedir. Bu alanda yapılan sekonder cerrahiler ise ekspojur güçlüğü ve sıkı yapışıklıklar nedeniyle güç olmaktadır. Robotik cerrahi yöntemle obstrüksiyona neden olan Soave cuff'ın robotik yöntemle eksize edildiği olgudaki deneyimlerimizi paylaşmayı amaçladık.

Video Yöntem ve Bulgular: Daha önce yenidoğan döneminde Hirschsprung hastalığı nedeniyle TEP uygulanmış 16 yaşında kız hasta kronik kabızlık ve gaita inkontinansı gibi obstrüktif semptomlar ile başvurdu. Opaklı kolon grafilerinde obstrüksiyon sebebi olarak Soave cuff gösterildi. Öncelikle laparoskopik olarak anteriordan cuff 'insizyonu' uygulandı ama hastanın şikayetleri erken dönemde tekrarladı. Bu nedenle Soave cuff 'eksizyonu' robotik olarak gerçekleştirildi. Robotik cerrahinin avantajları olan mükemmel 3 boyutlu görüş ve 540 derecelik hareket alanına izin veren ergonomi ve esneklik, derin pelviste çalışmayı oldukça kolaylaştırdı. Rezidü Soave cuff'ın anterior 270 derecelik kısmı, levator kas liflerinden ayrı olarak ortaya kondu ve güvenle rektuma veya vajinaya hasar vermeden eksize edildi. Cerrahi sonrası obstrüktif semptomlar düzeldi ve hasta günlük normal dışkılamaya başladı.

Sonuç: Erişilmesi zor olan derin pelvis gibi alanlarda ve yapışıklıkların olduğu, diğer dokulardan dikkatli disseksiyonu gereken organlara yönelik cerrahilerde konvansiyonel yöntemler güç olabilmektedir. TEP sonrası obstrüksiyon yaratan seromuskuler cuff eksizyonu için robotik cerrahi, etkin ve güvenli olarak uygulanabilir.
