Laparoscopic cystogastrostomy in a patient with pancreatic pseudocyst secondary to MSUD

Introduction: Pancreatic pseudocyst is the most common complication that develops on the background of pancreatitis. There is a 40% possibility of development in chronic pancreatitis and 15% in acute pancreatitis. The standard treatment for pseudocyst treatment is cystogastrostomy. Here, it is aimed to present the laparoscopic treatment of pancreatic prodocyst in the patient followed by MSUD.

Case: A four-year-old girl followed with MSUD. She was admitted to the hospital with complaints of abdominal pain and vomiting during MSUD attack. Increased pancreatic enzymes and pancreatitis on abdominal ultrasonography (USG) were detected. The patient was discharged after medical treatment. When the patient's complaints were repeated two months later, USG was performed and a cystic structure of 53x22 mm was observed at the head of the pancreas. Magnetic resonance cholangiopancreatography showed that the cyst extended to the diaphragm level. In follow-up, it was observed that the cyst size gradually increased and the cyst wall thickness matured to allow anastamosis. The cyst was not found suitable for endoscopic cystogastrotomy, since it was not close to the anterior wall of the stomach and adhered to the small curvature of the stomach. The patient underwent laparoscopic cystogastrostomy. During laparoscopy, Veres needle was placed into the cyst to retrieve pancreatic fluid. An incision through the anterior cyst wall and gastric wall was carried out and posterior and anterior wall of the anastomosis was done in running fashion.Postoperatively fifth day, contrasty study was performed, showing no anastomotic leak. The patient is doing well during 18 months of follow up

Conclusion: Laparoscopic cystogastrostomy is a saf, technically feasible and effective method in treatment of the patients with pancreatic pseudocysts.

MSUD hastalığı nedeniyle pankreas psödokisti gelişen hastada laparoskopik kistogastrostomi

Giriş: Pankreas psödokisti, pankreatit zemininde gelişen en sık komplikasyondur. Kronik pankreatit zemininde %40, akut pankreatit zemininde de %15 oranında gelişme olasılığı vardır. Psödokist tedavisinde standart tedavi kistogastrostomidir. Burada maple syrup urine disease (MSUD) ile takipli hastada gelişen pankreas psödokistinin laparoskopik olarak sağaltımının sunulması amaçlandı.

Olgu: Dört yaşında MSUD nedeniyle takipli kız hasta MSUD atak dönemlerinde oluşan karın ağrısı ve kusma şikayetleri ile hastaneye başvurduğunda yapılan tetkiklerinde pankreas enzimlerinde artış ve abdominal ultrasonografide (USG) pankreatit saptandı. Medikal tedavi sonrası taburcu edilen hastanın şikayetleri iki ay sonra tekrarlayınca yapılan USG’sinde pankreas başında 53x22 mm’lik kistik yapı izlendi ve manyetik rezonans kolanjiopankreatografide kistin diyafram düzeyine kadar uzandığı görüldü. Takiplerinde kist boyutununu giderek büyüdüğü ve kist duvar kalınlığının anastamoz yapılmasına izin verecek düzeyde olgunlaştığı görüldü. Kist midenin ön duvarına yakın olmadığı ve mide küçük kurvaturuna yapışık olduğu için endoskopik kistogastrotomi için uygun bulunmadı. Hastaya pankreatitsiz dönemde laparoskopik kistogastrostomi uygulandı. Laparoskopi esnasında önce kist içeriği Veres iğnesi yardımıyla boşaltıldı, mide ve kist ön duvarından kesiler yapılarak arka ve ön duvar devamlı dikişlerle anastomoz edilerek ve kistogastrostomi yapıldı. Ameliyat sonrası 5. günde kontrastlı mide grafisi çekildi ve anastomoz kaçağı olmadığı saptandı. Hasta nazogastrik sondası çekilerek beslenmeye başlandı. Hastanın ameliyattan sonra 18 ay boyunca izleminde sorun yaşanmadı

Sonuç: Pankreas psödokisti olan hastalarda laparoskopik kistogastrostomi kolaylıkla uygulanabilir, güvenlive etkin bir yöntemdir.
