Contribution of imaging methods to clinical diagnosis and treatment in pediatric thoracic trauma: An experience of a pediatric trauma center

INTRODUCTION: We aimed to determine our clinical evaluations and the contribution of imaging studies to treatment in pediatric thoracic trauma admitted to a trauma center in our study.

MATERIAL METHODS: Forensic traumas with the age of 0-18 years between January 2015 and December 2018, who were admitted to our trauma center, were included in the study. The statistical relationship between the presence of the pathologic findings at physical examination, imaging studies, and clinical results of thoracic trauma cases was analyzed.

RESULTS: In our study, 11,174 forensic trauma and 819 thoracic trauma were evaluated. Chest trauma accounted for 7.3% of the series. The most common pathology detected by posterior-anterior chest X-ray (CXR, n=650) was clavicle fracture with 21%, pneumothorax rate was 4%. The most common pathologies detected in chest computed tomography (CCT, n=492) were lung contusion (54%) and pneumothorax (45%). A statistically significant difference was found between CCT and CXR results in terms of recognizing the pathological finding. However, despite the increased diagnosis rate with CCT, it was found that it did not affect the treatment process of the chest trauma patients (p<0.001). Detection of pathological findings with CXR and CCT was low in patients with significant physical examination findings due to chest trauma. The rate of tube thoracostomy and thoracic surgery was 6.7-1.3% respectively. All patients who underwent thoracic surgery were discharged uneventfully. The mortality rate was 5.9% (n=48) in our thoracic trauma group, and the most common cause of death was head trauma. Thoracic trauma accompanying multiple traumas was one of the causes of death in 16 patients, but no patient died due to isolated thoracic trauma.

CONCLUSION: We determined that the rate of establishing pathological finding is low with CXR, which is the most commonly used imaging method in pediatric thoracic traumas, and the rate of detecting a pathologic finding by CTT is higher by CCT than CXR, but increased diagnosis rate did not contribute to the clinical treatment process, and it was found statistically significant. To reduce radiation exposure in children, we recommend that imaging studies should be planned after a detailed evaluation of the patient and when necessary.

Çocuk toraks travmalarında görüntüleme yöntemlerinin klinik tanı ve tedaviye katkısı: Bir çocuk travma merkezinin deneyimi

GİRİŞ: Çalışmamızda travma merkezine başvuran çocuk toraks travmalarında klinik değerlendirme sonuçlarımızı ve görüntüleme tetkiklerinin tedaviye katkısını belirlemeyi amaçladık.

GEREÇ YÖNTEMLER: Ocak 2015-Aralık 2018 tarihleri​​ arasında travma merkezimize başvuran 0-18 yaş arası adli travmalar çalışmaya dahil edildi. Adli travmalara ilişkin veriler ile torasik travma olgularının fizik muayene ve görüntüleme çalışmalarında var olan patolojik bulguların varlığı ile klinik sonuçları arasındaki istatistiksel ilişki literatür eşliğinde tartışıldı.

BULGULAR: Çalışmamızda 11.174 adli travma ve 819 toraks travmalı olgu değerlendirildi. Serinin %7,3'ünü göğüs travması mevcuttu. Hastaların posterior anterior akciğer grafisinde (PAAG, n=650) saptanan en sık patoloji %21 ile klavikula kırığı iken, pnömotoraks %4 oranında saptandı. Torakal bilgisayarlı tomografi (TBT, n=492) incelemesinde tespit edilen en sık patolojiler %54 oranıyla akciğer kontüzyonu ve %45 oranıyla pnömotoraks idi. TBT ve PAAG sonuçları arasında patolojik bulguyu tanıma yönünden istatistiksel fark anlamlı bulundu (p<0,001). Ancak göğüs bilgisayarlı tomografi ile artmış tanı oranına rağmen bunun tedavi sürecine etki etmediği saptandı (p<0,001). Toraks travma olgularında fizik muayene bulgusu olmasına rağmen PAAG ve TBT görüntülemelerinde patolojik bulguyu saptama oranı düşük saptandı (p<0,001). Tüp torakostomi uygulama oranı %6,7 iken göğüs cerrahi girişim oranı %1,3 idi. Toraks cerrahisi uygulanan tüm hastalar taburcu oldu. Toraks travma grubumuzda exitus oranı %5,9 (n=48) iken, en sık ölüm nedeni kafa travmasıydı, 16 hastada çoklu travmaya eşlik eden toraks travması ölüm sebeplerinden biriydi ancak izole toraks travması nedeniyle kaybedilen hasta olmadı.

SONUÇ: Çocuk toraks travmalarında en sık kullanılan görüntüleme yöntemi olan PAAG ile patolojik bulgu saptama oranının düşük olduğu ve toraks bilgisayarlı tomografisi ile PAAG'e göre tanı konan patolojik bulgu oranının yüksek olduğu, ancak bunun tedavi sürecine katkısının anlamlı olmadığı saptanmıştır. Çocuklarda radyasyon maruziyetininin azaltılması için görüntüleme istemlerinin hastanın detaylı değerlendirilmesi sonrasında ve gereğinde planlanmasını önermekteyiz
