Treatment approaches in anterior abdominal wall defects

Introduction: Omphalocele and gastroschisis are rare anomalies of the anterior abdominal wall. Its incidence is known to be 1-3 per 10000 live births. Since there are pathologies with high mortality and morbidity in the neonatal period, they require long-term follow-up in neonatal intensive care units. While additional congenital anomalies may accompany omphalocele, gastroschisis should be operated immediately after birth since there is no protective sac.

Method: A retrospective analysis of 9 patients who were followed up and treated with the diagnosis of gastroschisis and omphalocele in the Neonatal Intensive Care Unit of Hatay Mustafa Kemal University between 2019-2022 was performed.

Results: 5 patients were male (55.5%) and 4 patients were female (44.5.). Mean delivery weeks were 35 weeks and 4 days and mean birth weight was 2590 gr. 5 of the patients were omphalocele and 4 of them were gastroschisis. One patient with a diagnosis of omphalocele had tricuspid atresia. Meningomyelocele was present in another patient with omphalocele, the patient with a low Apgar score at birth died in the 1st postnatal hour. While primary repair was performed in 3 omphalocele patients, the silastic silo method was used in 1 omphalocele patient due to high intra-abdominal pressure. In the 2nd week, this patient's defect was closed primarily. PDA was found to accompany one patient with gastroschisis. Due to the fact that 2 patients with gastroschisis were premature and low birth weight, bedside reduction was performed with the Bianchi procedure and primary closure was performed. No complications were observed. One patient with gastroschisis was re-operated for postoperative intestinal adhesion. Short bowel syndrome developed as a result of resection performed due to intestinal necrosis. Except for the patient with Ex, the other patients were discharged with good recovery.

Conclusion: The mortality and morbidity rates of all babies diagnosed with antenatal anterior abdominal wall defect are significantly reduced by the delivery of a multidisciplinary team consisting of neonatal intensive care specialists and pediatric surgeons. Antenatal follow-up is very important since additional anomalies are an important condition in bad clinical situation.

Giriş: Omfalosel ve gastroşizis karın ön duvarının nadir görülen anomalilerindendir. Görülme sıklığı yaklaşık 10000 canlı doğumda 1-3 olduğu bilinmektedir. Yenidoğan döneminde mortalite ve morbiditesi yüksek patolojiler olduğundan yenidoğan yoğun bakım ünitelerinde uzun süreli takip gerektirmektedir. Omfalosele ek konjenital anomaliler eşlik edebilirken gastroşizis koruyucu bir kese olmadığından doğumdan hemen sonra opere edilmelidir.

Yöntem: 2019-2022 yılları arasında Hatay Mustafa Kemal Üniversitesi Yenidoğan Yoğun Bakım Ünitesinde Gastroşizis ve Omfalosel tanısı ile takip ve tedavi edilen 9 hastanın retrospektif analizi yapıldı.

Bulgular: 5 hasta erkek(%55.5), 4 hasta kız(44.5.) idi. Ortalama doğum haftaları 35 hafta 4 gün ve ortalama doğum kilosu 2590 gr idi.Hastaların 5’i omfalosel,4’ü gastroşizis idi. Omfalosel tanısı olan 1 hastada triküspit atrezisi mevcuttu. Başka bir omfalosel tanılı hastada Meningomyelosel mevcuttu, doğumda apgar skoru düşük olan hasta postnatal 1. saatte ex oldu. 3 omfalosel hastasında primer onarım yapılırken 1 omfalosel hastasında batın içi basıncın yüksek olması nedeniyle silastik silo yöntemi kullanıldı. 2. haftada bu hastanın defekti primer olarak kapatıldı. Gastroşizis olan 1 hastada PDA’nın eşlik ettiği görüldü. Gastroşizis olan 2 hastaya prematüre ve düşük doğum ağırlıklı olması nedeniyle Bianchi prosedürü ile yatak başı redüksiyon yapılarak primer kapatıldı. Komplikasyon görülmedi. 1 gastroşizis hastasında postoperatif intestinal adhezyon nedeniyle tekrar opere edildi. Bağırsakların nekroze olması nedeniyle yapılan rezeksiyon sonucu kısa bağırsak sendromu gelişti. Ex olan hasta dışında diğer hastalar şifa ile taburcu edildi.

Sonuç: Antenatal batın ön duvarı defekti tanısı almış tüm bebeklerin, yenidoğan yoğun bakım uzmanları ve çocuk cerrahlarından oluşan multidisipliner bir ekibin olduğu yerde doğumunun gerçekleşmesi ile mortalite ve morbidite oranları belirgin azalmaktadır. Kötü gidişatta ek anomaliler önemli bir durum olduğundan antenatal takip oldukça önemlidir.
