Laparoscopic Left Adrenal Neuroblastoma Excision

Aim:

The most common abdominal malignant tumor in children is neuroblastoma and usually originates from the adrenal gland. Because of the infiltrative nature of neuroblastoma, the open surgical approach remains the mainstay of treatment. With the development of laparoscopic instruments, laparoscopic resection is increasingly performed in well-circumscribed neuroblastomas because of the smaller incision area and faster postoperative recovery.

In this study, we aimed to present the transperitoneal laparoscopic neuroblastoma excision of a 7-year-old female patient with neuroblastoma 4 cm in diameter in the left adrenal region.

Video Method and Results:

Under general anesthesia, pneumoperitoneum was created with a Veress needle 2 cm lateral to the umbilicus in the left lateral position with 12 mmHg CO2. A 5mm camera port was inserted from the same place. A 5 mm working port was placed from the left midclavicular line subcostal area. A 10 mm working port was placed from the anterior axillary line in the left flank region. The splenic flexure was opened with Ligasure, and the retroperitoneal space was entered. The left adrenal mass was dissected from the surrounding tissues. The left adrenal vein was exposed, clipped with an endoclip, and transected. The adrenal mass was released from the surrounding tissues by Ligasure and harmonic scalpel. The mass was taken into an endobag and taken out of the abdomen through the 10 mm port entrance. No pathological lymph node was detected. It was observed that there was no bleeding focus, and the fascia at the 10mm port entrance was repaired with 2/0 Vicryl. The patient was discharged on the 2nd post-op day.

Conclusion:

Laparoscopic excision is a safe and feasible method in patients with well-circumscribed adrenal lesions that do not show any major organ or vascular invasion.

Laparoskopik Sol Adrenal Nöroblastom Eksizyonu

Amaç:

Çocuklarda en sık görülen abdominal malign tümör nöroblastomdur ve genellikle adrenal bezden kaynaklanır. Nöroblastomun infiltratif doğası nedeniyle açık cerrahi yaklaşım tedavinin temeli olmaya devam etmektedir. Laparoskopik aletlerin gelişmesiyle birlikte daha küçük insizyon alanı ve daha hızlı ameliyat sonrası iyileşme nedeniyle iyi sınırlı nöroblastomlarda laparoskopik rezeksiyon giderek daha fazla yapılmaktadır.

Bu çalışmamızda sol sürrenal bölgede 4 cm çapında nöroblastomu olan 7 yaşında kız hastamızın transperitoneal laparoskopik nöroblastom eksizyonunu sunmayı amaçladık

Video Yöntem ve Bulgu:

Genel anestezi altında sol lateral pozisyonda umblikusun 2 cm lateralinden Veress iğnesi ile 12 mmHg CO2 ile pnömoperitoneum oluşturuldu. Aynı yerden 5mm’lik kamera portu yerleştirildi. Sol midklavikular hat subkostal alandan 5 mm’lik çalışma portu yerleştirildi. Sol flank bölgesinde ön aksiller hattan 10 mm2lik çalışma portu yerleştirildi. Splenik fleksura Ligasure ile açılarak retroperitona geçildi. Sol adrenal kitle çevre dokulardan disseke edildi. Sol sürrenal ven ortaya konarak endoklips ile kliplendi ve transekte edildi. Sürrenal kitle çevre dokulardan ligasure ve harmonik scalpel ile serbestlendi. Kitle endobag içine alınarak 10 mm’lik port giriş yerinden karın dışına alındı. Patolojik boyutta lenf nodu saptanmadı. Herhangi kanama odağı olmadığı görülerek 10mm’lik port giriş yerindeki fasya 2/0 Vicryl ile onarıldı. Hasta post-op 2.gün taburcu edildi.

Sonuç:

Majör organ ve damar invazyonu göstermeyen, iyi sınırlı adrenal lezyonları olan hastalarda laparoskopik eksizyon güvenli ve uygulanabilir bir yöntemdir.
