Elective thoracoscopy after exhausted bullet injury

Aim: To demonstrate the effectiveness of the torcoscopic approach in chest wall injuries.

Video method and results: A 14-year-old male patient, who was injured by a tired bullet falling from above while playing in the garden of his house, is taken to the hospital. The bullet entering just posterior to the right sternocleidomastoid muscle appears to have entered the thoracic cavity. On CT, it is observed that vital organs are not damaged. The patient, who was followed up conservatively with an urgently inserted thoracic drain, was removed on the fifth day and the patient was discharged. Elective thoracoscopy is performed after three months due to legal and familial reasons, and the bullet is removed from the posterior thoracic wall, right next to the esophagus, and the contents of the abscess are cleaned. Since it is very difficult to find the bullet during the procedure, navigation is often delayed with the help of a scope. On the third postoperative day, the thoracic drain is removed and the patient is discharged. Our patient did not have any problems during the 1-year follow-up period after the operation.

Conclusion: Thoracoscopy is a safe and effective method in intrathoracic foreign body injuries. A new CT and perop scopy preparation for the localization of the foreign body facilitates the finding of the object in the tissue.

Yorgun kurşun yaralanması sonrası elektif torakoskopi

Amaç: Göğüs duvarı yaralanmalarında torkoskopik yaklaşımın etkinliğini göstermek.

Video yöntem ve bulgu: Evinin bahçesinde oynarken yukarıdan düşen yorgun bir kurşun nedeniyle yaralanan 14 yaşında erkek hasta hastaneye kaldırılıyor. Sağ sternokleidomastoid kasın hemen posteriorundan giren merminin toraks kavitesi içerisine girdiği görülüyor. Çekilen BT’de hayati organların zarar görmediği gözleniyor. Acilen takılan toraks dreni ile konservatif takipe alınan hastanın dreni beşinci gün çekiliyor ve hasta taburcu ediliyor. Takiplerinde hiçbir problem yaşamayan hastaya kanuni ve ailesel nedenlerden dolayı üç ay sonra elektif torakoskopi yapılıyor ve posterior toraks duvarı içerisinden, özefagus hemen yanından mermi çıkarılıyor, abse içeriği temizleniyor. İşlem esnasında mermiyi bulmak çok zor olduğundan skopi yardımıyla sık sık navigasyon gekiyor. Postop üçüncü gün toraks dreni çekilerek taburcu ediliyor. Ameliyat sonrası 1 yıllık takip süresinde hastamızın hiç bir problem olmuyor.

Sonuç: Toraks içi yabancı cisim yaralanmalarında torakoskopi güvenli ve etkin bir yöntemdir. Yabancı cismin lokalizasyonu için yeni çekilmiş bir BT ve perop skopi hazırlığı doku içerisindeki cismin bulunmasını kolaylaştırmaktadır.
