An Innovative Surgical Treatment Method in Persistent Rectal Prolapse: Ventral Abdomino-Rectosigmoidopexy via Tube Sigmoidostomy Combined with Ekehorn’s Rectopexy

Background/Aim: We discussed the results of our cases who were treated with sclerosing agents in external centers, but who underwent ventral abdomino-rectosigmoidopexy via tube sigmoidostomy combined with Ekehorn’s rectopexy due to recurrent rectal prolapse.

Materials/Methods: The demographic characteristics, surgical technique and results of children who were operated in our clinic for rectal prolapse between 2004-2022 were retrospectively analyzed.

Results: In 18 years, 6 patients (2 girls [33%] and 4 boys [67%]) were operated for persistent rectal prolapse. The mean operative age of the patients was 7.8 years (2.4-17), and all had severe rectal prolapse. These patients were followed in other centers and their rectal prolapse continued despite diet changes, toilet training and the use of sclerosing agents.

Ekehorn rectopexy was performed in the first two of the cases. Rectal prolapse developed after colostomy closure in the first case who underwent laparoscopy-assisted fistula ligation and anorectoplasty the age of one year. The prolapse of the patient who underwent Ekehorn’s rectopexy at the age of six did not recur. Considering that rectosigmoid colon adhesions formed on the anterior abdominal wall due to colostomy opening-closing may provide ventral sigmoidopexy, it was decided to offer the option of applying both methods together.

Three of the next four cases were diagnosed with cystic fibrosis. All four underwent ventral abdomino-rectosigmoidopexy via tube sigmoidostomy combined with Ekehorn’s rectopexy. Ekehorn butterfly sutures were removed on day 15 and Foley catheters on day 21. Three cases with cystic fibrosis were uneventful. However, a 14-year-old girl with a history of sexual abuse relapsed 6 months later.

Conclusions: The ventral abdomino-rectosigmoidopexy via tube sigmoidostomy method combined with Ekehorn’s rectopexy (for minimally invasiveness) is a successful and unique method in terms of providing intestinal fixation. It may be the primary option for definitive surgical treatment of persistent rectal prolapse.

Persistan Rektal Prolapsusta Yenilikçi Bir Cerrahi Tedavi Yöntemi: Ekehorn Rektopeksi ile Kombine Tüp Sigmoidostomi Yoluyla Ventral Abdomino-Rektosigmoidopeksi

Giriş/Amaç: Şiddetli rektal prolapsuslu çocuklarda geleneksel tedavi yöntemleri genellikle başarılı değildir. Burada, dış merkezlerde sklerozan ajan uygulanmış, ancak tekrarlayan rektal prolapsus nedeniyle Ekehorn rektopeksi ile kombine tüp sigmoidostomi yoluyla ventral abdomino-rektosigmoidopeksi uygulanan olgularımızın sonuçları tartışıldı.

Gereç/Yöntem: 2004-2022 yılları arasında rektal prolapsus nedeniyle kliniğimizde ameliyat edilen çocukların demografik özellikleri, cerrahi teknik ve sonuçları geriye dönük olarak incelendi.

Bulgular: 18 yılda, 2'si kız (%33) ve 4'ü erkek (%67) 6 hasta persistan rektal prolapsus nedeniyle opere edildi. Hastaların ortalama ameliyat yaşı 7,8 yıldı (2,4-17) ve tümünde şiddetli rektal prolapsus vardı. Bu hastalar dış merkezlerde takipli olup diyet değişikliği, tuvalet davranış eğitimi ve sklerozan ajanların kullanımına rağmen rektal prolapsusları devam etmekteydi.

Olgulardan ilk ikisine Ekehorn rektopeksi yapıldı. Bir yaşındayken rektoprostatik fistüllü anal atrezisi için laparoskopi yardımlı fistül ligasyonu ve anorektoplasti yapılan ilk olguda kolostomi kapatılması sonrası rektal prolapsus gelişti. Anal mukozal trimming ile düzelmeyen ve giderek artan prolapsus nedeniyle altı yaşındayken Ekehorn rektopeksi yapılan hastanın prolapsusu tekrarlamadı. Kolostomi açma-kapama nedeniyle batın ön duvarında oluşan rektosigmoid kolon yapışıklıklarının ventral sigmoidopeksiyi sağlayabileceği düşünülerek, tekrarlayan ağır prolapsuslu olgularda, ailelere iki yöntemin birlikte uygulanması seçeneğinin sunulmasına karar verildi.

Sonraki dört olgudan üçü kistik fibrozis tanılı olup, dış merkezlerde yapılan sklerojan madde enjeksiyonuna rağmen, tekrarlayan prolapsusları mevcuttu. Dördüne de Ekehorn rektopeksi ile kombine tüp sigmoidostomi yoluyla ventral abdomino-rektosigmoidopeksi uygulandı. Ekehorn kelebek sütürleri 15. gün, Foley sondaları 21. gün çıkarıldı. Sigmoid fistülleri spontan olarak üç hafta içerisinde kapandı. Kistik fibrozisli üç olgu sorunsuz seyretti. Ancak 14 yaşındaki cinsel istismar öykülü kız hastada 6 ay sonra nüks görüldü ve hasta klinik takibimizden çıktı.

Sonuç: Rektal prolapsusun cerrahi tedavisinde Ekehorn rektopeksiye kombine edilen (minimal invazif olması açısından) tüp sigmoidostomi yoluyla ventral abdomino-rektosigmoidopeksi yöntemi, rektum ve sigmoid kolon fiksasyonunu sağlaması açısından başarılı ve özgün bir yöntemdir. Özellikle persistan rektal prolapsusu olan çocuklarda definitif cerrahi tedavi için öncelikli seçenek olabilir.
