Using the TWIST score in the management of children with acute scrotum: short-term results

Aim: The acute scrotum is an emergent condition, can be caused by testicular torsion (TT), torsion of the appendix testis, or epididymo-orchitis. The Testicular Workup for Ischemia and Suspected Torsion (TWIST) score based on clinical parameters for diagnosis of TT, and low scores are reliable and accurate enough to rule out TT in the pediatric population. In this study, we aimed to present the early results of the TWIST score system for differential diagnosis of TT in patients presenting with acute scrotum.

Methods: Retrospective study of the patients presenting with acute scrotum and managed as inpatient hospitalization between March 2020 and August 2022 was carried out. In the last 5 months, the TWIST scoring system has been used. We divided our patients into the TWIST score used group and non-used, and the groups subdivided into two groups as those who underwent surgery and those who did not. Comparison was performed in terms of clinical characteristics, laboratory results, final diagnosis and postoperative results.

Results: A total of 218 children were hospitalized, 109 (50%) of them underwent scrotal exploration. A total of 40 patients were treated after starting to use the scoring system. The mean age was significantly higher (12.0±3.8vs10.8±3.4 years, p<0.05), and the hospital stay was significantly longer (3.5±1.4vs2.2±1.0 days, p<0.05) in the surgical group. Compared to the previous period, medically treated patient was significantly higher.(n=31vsn=9, p<0.05), but no significant difference was found in terms of demographic, clinical, and laboratory results (p>0.05). The interval between admission and surgery was lower in the TWIST group, but difference was not significant (6.3±10vs10.6±16.2 hours, p>0.05). There was no significant difference in terms of orchiectomy rate and postoperative complications (p>0.05). Non-TT pathologies were detected at a rate of 51% in scrotal explorations previously, this rate was 11% in the last period, difference was significant (p<0.05).

Conclusion: The use of TWIST score in the approach to acute scrotum may prevent unnecessary scrotal explorations, as well as enabling patients with suspected TT to undergo surgery immediately.

Akut skrotumlu çocukların yönetiminde TWIST skorunun kullanılması: kısa dönem sonuçları

Amaç: Akut skrotum, testis torsiyonu (TT), apendiks testis torsiyonu veya epididimo-orşit gibi durumların neden olduğu acil bir durumdur ve öncelikle erken skrotal eksplorasyon gerektiren TT'nu dışlamaktır. TT için düşük veya yüksek risk altındaki hastaları değerlendirebilen ve gereksiz cerrahi girişimleri azaltan birkaç farklı klinik skorlama sistemi geliştirilmiştir. İskemi ve Şüpheli Torsiyon için Testiküler Çalışma (TWIST) skoru, TT tanısı için klinik parametrelere dayanmaktadır ve çocuklarda TT'yi ekarte etmekte güvenilir olduğu bildirilmiştir. Bu çalışmada akut skrotum ile başvuran hastalarda TT ayırıcı tanısında kullanmaya başladığımız TWIST skor sisteminin erken dönem sonuçlarını sunmayı amaçladık.

Yöntem: Mart 2020 ile Ağustos 2022 tarihleri ​​arasında akut skrotum ile başvuran ve yatarak tedavi edilen hastaların verileri retrospektif incelendi. Son 5 ayda TWIST puanlama sistemi kullanıldı. Hastalarımızı ameliyat olanlar ve olmayanlar, TWIST skoru kullanılanlar ve kullanılmayanlar olarak ikiye ayırdık. Demografik özellikler, klinik bulgular, laboratuvar sonuçları, kesin tanı ve ameliyat sonrası sonuçlar açısından karşılaştırma yapıldı.

Bulgular: Çalışma süresince akut skrotum tanısı ile hastaneye yatırılan toplam 218 çocuğun (93 sağ, 125 sol, ortalama yaş 11.4±3.7 yıl) 109'una (%50) skrotal eksplorasyon uygulandı. Cerrahi grubunda yaş ortalaması anlamlı olarak daha yüksekti (12.0±3.8 yıl ve 10.8±3.4 yıl, p<0.05) ve hastanede kalış süresi anlamlı olarak daha uzundu (3.5±1.4 gün ve 2.2±1.0 gün, p<0.05). Skorlama sistemini kullanmaya başladıktan sonra toplam 40 hasta yatış yapılarak tedavi edildi. Bir önceki döneme göre medikal tedavi gören hasta sayısı anlamlı olarak daha yüksek bulundu (n=31 ve n=9, p<0.05), ancak yaş, klinik bulgular ve laboratuvar değerleri açısından fark saptanmadı (p>0.05). Başvuru ile ameliyat arasındaki süre açısından ise TWIST grubunda daha düşük olmasına rağmen aradaki fark anlamlı değildi (6.3±10 ve 10.6±16,2 saat, p>0.05). Orşiektomi oranı ve ameliyat sonrası komplikasyonlar açısından cerrahi uygulanan hastalar arasında önceki dönemle anlamlı fark yoktu (p>0.05). Bir önceki dönemde skrotal eksplorasyonlarda TT dışı patolojiler %51 oranında saptanırken, son dönemde bu oran %11 idi, aradaki fark anlamlıydı (p<0.05).

Sonuç: Akut skrotuma yaklaşımda TWIST skorunun kullanılması, gereksiz skrotal eksplorasyonların önüne geçebileceği gibi, TT şüphesi olan hastaların daha erken cerrahiye alınmasını da sağlayabilir.
