A rare mass in an infant: Lipoblastoma of the anterior thoracic wall

Introduction: Lipoblastomas are rare benign tumors of embryonic adipose tissue. Chest wall lipoblastomas are very rare. Here we present a case of chest wall lipoblastoma in an infant who was successfully treated with surgery.

Case: A 6-month-old female patient presented with the complaint of a large mass that started under the right breast 3 months ago and enlarged rapidly, covering the right anterior chest wall. On physical examination, there was an immobile mass starting from the right clavicle and extending to the right costal arch and right axillary region. On ultrasonography, there was a 46x21x47mm mass pushing the areola outward on the right. Laboratory tests were normal. Thorax magnetic resonance imaging (MRI); A soft tissue mass lesion measuring 6x2,4x5,4cm, with lobulated contour, septal, thick-walled, located between the pectoralis major and minor muscles, with intrathoracic extension in the inferomedial, was detected on the anterior wall of the right thorax. In the intraoperative examination, the yellow colored, lobulated contoured mass was densely adherent to the pectoral muscles and the periosteum of the 4th and 5th ribs. The 8x6cm mass was progressing from the 4th intercostal space to the thoracic cavity by 1.5 cm. There was a second 3 cm mass located deep in the axillary region, both masses were totally excised. The patient had no postoperative problems and his pathology was reported as lipoblastoma. No residue/recurrence was detected in the control thorax MRI performed 3 months later, and 15x11 mm breast tissue was observed in the bilateral retroaerolar area.

Conclusion: Lipoblastoma, a rare tumor, is very rare in infants, especially in the chest wall and breast area. Lipomas and liposarcomas are included in the differential diagnosis of fat-containing tumors in children. In lipoblastoma, total excision of the mass is the curative treatment and the prognosis is good with timely treatment.

İnfantta nadir bir kitle: Göğüs ön duvarının lipoblastomu

Giriş: Lipoblastomlar, embriyonik yağ dokusunun nadir görülen iyi huylu tümörleridir. Göğüs duvarı lipoblastomları çok nadir görülür. Biz burada cerrahi ile başarılı bir şekilde tedavi edilen bir bebekte göğüs duvarı lipoblastomu olgusunu sunduk.

Olgu: 6 aylık kız hasta, 3 ay önce sağ meme altında başlayan ve hızla büyüyerek sağ göğüs ön duvarını kaplayan büyük kitle şikayeti ile başvurdu. Fizik muayenede sağda klavikula hizasından başlayıp kostal arka, ön orta hattan aksiller bölgeye doğru uzanan immobil kitle mevcuttu. Ultrasonografide sağda areolayı dışa iten 46x21x47mm büyüklüğünde kitle saptandı. Laboratuvar testleri normaldi. Toraks manyetik rezonans görüntülemesinde (MRG); sağ toraks ön duvarında, pektoralis major ve minor kasları arasında yerleşimli, inferomedialde intratorasik uzanımı olan, lobüle konturlu, septalı, kalın cidarlı 6x2,4x5,4cm boyutlarında yumuşak doku kitle lezyonu saptandı. İntraoperatif bakıda sarı renkli, lobule konturlu kitle, pektoral kaslarla, 4 ve 5. kot periostuna dens yapışıktı. 8x6cm büyüklüğündeki kitle medialde 4. interkostal aralıktan toraks kavitesine doğru 1,5cm kadar ilerliyordu. Aksiller bölgede derin yerleşimli 3cm'lik ikinci bir kitle daha mevcuttu, her iki kitle total olarak eksize edildi. Postoperatif herhangi bir sorunu olmayan hastanın patolojisi lipoblastom olarak raporlandı. Hastanın 3 ay sonra yapılan kontrol toraks MRG’sinde rezidü/nüks saptanmadı, bilateral retroaerolar alanda 15x11 mm boyutunda meme dokusu izlendi.

Sonuç: Nadir bir tümör olan lipoblastom, infantta özellikle göğüs duvarında, meme bölgesinde çok nadir görülür. Çocuklarda yağ içeren tümörlerin ayırıcı tanısında lipomlar ve liposarkomlar yer alır. Lipoblastomda, kitlenin total eksizyonu küratif tedavidir ve zamanında uygulanan tedavi ile prognoz iyi seyreder.
