Endoscopic Pilonidal Sinus Treatment (EPSIT): What has changed after the first application?

Aim: Evaluation of the EPSiT method used in the pilonidal sinus (PS) treatment and comparison of the new results with the first study in 2019.

Cases and Method: Eighty-five cases treated with the EPSiT method were reviewed retrospectively between June 2018 and August 2022. According to the mean time to recurrence, 75 patients with a follow-up period of > 8 months were included in the study. Demographic data of the cases, recurrence and follow-up time, epilation methods, and wound care were evaluated. The cases were examined separately before (group 1) and after (group 2) in July 2019, and treatment changes were determined.

Results: The mean age of 75 cases (52 boys, 23 girls) was 15.1 years (7-24 years).

The mean age of 14 relapsed cases (M:10, F:4) was 15.4 years (11-18 years), with a mean BMI of 28.17 (20.9-33.9 years). The time to complete recovery was 20 days (7-60 days), and the mean time to recurrence was eight months (5-22 months). The wet dressing was applied to eleven, and wet dressing+silver nitrate was applied to 3. Epilation (laser, razor, hair removal cream, etc.) was applied to seven cases (50%). Significant differences were observed in wound care and epilation practices in the relapsed and non-relapsed groups.

The recurrence rate in group 1 (n:29) and group 2 (n:46) was 28% (n:8) and 12.2% (n:6), respectively. There was no difference between the two groups regarding age, gender, and BMI. The time to complete wound healing in relapsed cases was 25.75 days (7-60 days) in group 1 and 12.3 days (7-30 days) in group 2. The rate of epilation application was 27% in group 1 and 78% in group 2. In wound care, only wet dressing was applied in group 1, while the rate of wet dressing + silver nitrate application in group 2 was 80%.

Conclusion: EPSiT, a minimally invasive intervention in the treatment of pilonidal sinus, can be safely applied with the support of technical changes in the application, postoperative wound care, and epilation methods, reducing the recurrence rates.

Amaç: Pilonidal sinüs (PS) tedavisinde uyguladığımız EPSiT yöntemininin değerlendirilmesi ve 2019’daki ilk çalışma ile yeni sonuçların karşılaştırılmasıdır.

Olgular ve Yöntem: Haziran 2018-Ağustos 2022’de EPSiT yöntemiyle tedavi edilen 85 olgu geriye dönük olarak incelendi. Ortalama nüks zamanına göre, izlem süresi > 8 ay olan 75 olgu çalışmaya dahil edildi. Olgular 2019 Temmuz öncesi (grup 1) ve sonrası(grup 2) olarak ayrıca irdelendi ve tedavi değişiklikleri belirlendi.

Bulgular: Yetmiş beş olgunun (52 erkek, 23 kız) yaş ortalaması 15,1 yaş (7-24 yaş) idi. İzlemde olguların 61’i sorunsuzdu, 14’ünde (%18,6) nüks izlendi.

Nüks eden 14 olgunun (E:10, K:4) yaş ortalaması 15,4 yaş ( 11-18 yaş), ortalama VKİ 28,17 (20,9-33,9) idi. Tam iyileşme zamanı 20 gün (7-60 gün), nüks zamanı ortalama 8 ay (5-22 ay) idi. Onbirine ıslak pansuman, 3’üne ıslak pansuman+gümüş nitrat uygulandı. Yedi olguya(% 50) epilasyon (lazer, jilet,tüy dökücü krem vb) uygulandı. Nüks olan ve olmayan grupta yara bakımı ve epilasyon uygulamalarında belirgin farklılık izlendi.

Grup 1 (n:29) ve grup 2 (n:46)’de nüks oranı sırasıyla % 28 (n:8) ve % 12,2(n:6) olarak bulundu ve bu fark anlamlıydı. İki grup arasında yaş, cinsiyet, VKİ açısından fark saptanmadı. Nüks eden olgularda tam yara iyileşmesi grup 1’de 25,75 gün (7-60 gün), grup 2’de 12,3 gün (7-30 gün) idi. Grup 1’de epilasyon uygulanması oranı % 27, grup 2’de % 78 idi. Yara bakımında grup 1’de ıslak pansuman yapılırken, grup 2’de ıslak pansuman+gümüş nitrat uygulaması oranı % 80’di.

Toplamda nüks saptanan 14 olgunun 2’sine primer onarım, 5’ine re-EPSiT yapıldı. 2 olgu takipten çıktı ve 5’ine tekrar EPSiT planlanmaktadır.

Sonuç: Pilonidal sinüs tedavisinde minimal invaziv bir girişim olan EPSiT, uygulamadaki teknik değişiklikler, postoperatif yara bakımı ve epilasyon yöntemlerinin desteği ile nüks oranlarında azalma sağlanarak güvenle uygulanabilir.
