Hepatopulmonary Fusion: A Rare Variant of Congenital Diaphragmatic Hernia

Objective:Congenital hepatopulmonary fusion (HPF) is a rare anomaly characterized by fusion of liver and lung parenchyma.There are approximately ten cases of HPF reported in the literature In this report, a case of congenital diaphragmatic hernia (CDH) with HPF is presented.

Case Report:The case without antenatal diagnosis was hospitalized in the pediatric neonatal intensive care unit due to respiratory distress after a normal spontaneous vaginal delivery of 3120 grams at the 40th gestational week.In the examination of the case, no lung sound could be heard in the right hemithorax, and no pathology was detected in the abdomen.The difference between preductal and postductal oxygen saturation was found to be over 10%.Right diaphragmatic hernia was detected in the direct X-ray, and it was reported that the right lobe of the liver herniated into the right hemithorax in the ultrasonography. Sildenafil treatment was started in the patient due to pulmonary hypertension (Mean PAP: 40-45mmHg). After the patient stabilized, he was taken to laparotomy.In the operation, it was observed that the right lobe of the liver and small intestine were also in the right hemithorax and there was a fusion of approximately 4-5 cm between the liver and the right lung.The two tissues could not be separated from each other.By surrounding the liver, diaphragmatic rim on the posterior wall and the pleural structures on the anterior wall were sutured.The patient was discharged on the 101st day of his life.In the follow-up, the patient was hospitalized twice due to upper gastrointestinal system bleeding.He was diagnosed as portal hypertension. Endoscopic and medical treatment was applied.He is now two years old and still being followed up.

Conclusion:Hepatopulmonary fusion is a rare variant of CDH and standard surgical treatment has not been reported.Surgical treatment varies according to the degree of fusion and vascular anomalies.In our report, the first hepatopulmonary fusion case that caused portal hypertension and related esophageal varices in the literature is presented.

Hepatopulmoner Füzyon: Konjenital Diyafram Hernisinin Nadir Bir Varyantı

Amaç: Konjenital hepatopulmoner füzyon (HPF), karaciğer ve akciğer parankiminin füzyonu ile karakterize nadir görülen bir anomalidir. Literatürde bildirilmiş yaklaşık on tane HPF vakası bulunmaktadır. Bu bildiride HPF saptanan bir konjenital diyafram hernisi (KDH) olgusu sunulmuştur.

Olgu Sunumu: Antenatal herhangi bir tanısı olmayan olgu 40. Gestasyonel haftada normal spontan vajinal yolla 3120 gram doğumun ardından solunum sıkıntısı gelişmesi nedeniyle solunum desteğinde çocuk sağlığı yenidoğan yoğun bakıma yatırılmıştı. Olgunun yapılan muayenesinde dinlemekle sağ hemitoraksta akciğer sesi duyulmamaktaydı, batın muayenesinde patoloji saptanmamıştı. Olgunun preduktal ve postduktal oksijen satürasyonu farkı %10’un üzerinde saptandı. Çekilen direkt grafide sağ diyafram hernisi saptanan hastanın ultrasonografiside karaciğer sağ lobunun sağ hemitoraksa herniye olduğu bildirildi. Pulmoner hipertansiyonu olan (Ort. PAP:40-45mmHg) olguya sildenafil tedavisi başlandı. Genel durumu stabilleşen olgu opere edildi. Operesyonda karaciğer sağ lobu ile ince barsakların da sağ hemitoraksta olduğu görüldü. Karaciğer ve sağ akciğer arasında yaklaşık 4-5 cm uzunluğunda füzyon olduğu gözlendi, iki doku birbirinden ayrılamadı. Posterior duvardaki diyafragma rimi ile anterior duvardaki plevra yapıları karaciğeri çepeçevre saracak şekilde orta hatta birleştirildi. Post-operatif dönemde sorunu olmayan olgu yaşamının 101. gününde taburcu edildi. Takibinde olgunun iki kere üst gastrointestinal sistem kanaması nedeniyle hastane yatışı oldu. Bu dönemde yapılan endoskopilerinde özofagus varisleri saptanan ve bu sebeple portal hipertansiyon tanısı alan olguya endoskopik ve medikal tedavi uygulandı. Şu an iki yaşında olan olgu tarafımızca halen takip edilmektedir.

Sonuç: Hepatopulmoner füzyon nadir görülen bir KSH varyantıdır ve standart cerrahi tedavi bildirilmemiştir. Cerrahi tedavi füzyon derecesine ve vasküler anomalilere göre değişmektedir. Bildirimizde literatürde portal hipertansiyon ve buna bağlı özofagus varislerine neden olmuş ilk hepatopulmoner füzyon olgusu sunulmuştur.
