Video presentation of morbid obese pediatric patient laparoscopic sleeve gastrectomy

Aim
Sleeve gastrectomy is an effective procedure for inducing sustainable weight loss in adolescents with obesity. This video highlights the technical preparation (equipment, patient positioning, pre-operative management), and conduct (anatomy, instruments, methodology, pitfalls) of the operation through a 17 years old male weight 150kg, BMI <50 with comorbidities.
Video method and results:
17 year old male adolescent (weighing 150kg, Body Mass Index<50kg/m2) with hypertension and insuline resistance has been followed up for the last seven years by pediatric endocrinologist. Surgical treatment was considered for this case by a multidisciplinary team. He underwent laparascopic sleeve gastrectomy for morbide obesity. Total operation duration with anesthesic manuplations was 156 min. Post operative second day after stapler line checked with oral contrast study oral intake started with clear liquids. On the fourth day the patient discharged. At last visit 1st year after operation weight lost was 50 kg.
Conclusion:
laparoscopic sleeve gastrectomy was a feasible and easy surgical method for morbidly obese children. A multidisciplinary approach and lifelong behavior therapy are the most important factors for outcome

Amaç:
Tüp mide ameliyatı, obez ergenlerde sürdürülebilir kilo kaybı sağlamak için etkili bir prosedürdür. Bu videoda, komorbiditeleri olan 17 yaş (vücut ağırlığı 150 kg, BMI <50) erkek hastanın operasyon teknik hazırlığı (ekipman, hasta poziyonu, ameliyat öncesi yönetim) ve operasyon süreci (anatomi, aletler, metodoloji, tuzaklar) sunulmaktadır.
Video yöntem ve bulgu:
Hipertansiyon ve insuline rezistansı olan 17 yaşında erkek adolesan VKİ<50 morbid obezite nedeniyle yedi yıldır pediatrik endokrinoloji tarafından takip edilmekte idi. Medikal, diyet ve egzersiz tedavilerine rağmen kilo vermeyen hasta multidisipliner konseyde tartışılarak obezite cerrahisine uygun bulundu. Hastaya laparaskopik sleeve gastrektomi yapıldı. Anestezi manipülasyonları ile toplam operasyon süresi 156 dakika idi. Post operatif ikinci günde oral kontrast grafi çekilerek kaçak olmadığı gösterildi. Oral beslenmesi sıvı ile başlandı. Operasyon sonrası dördüncü günde hasta taburcu edildi. . Ameliyattan sonraki 1. yılda son muayenede hastanın kilo kaybı 50 kg idi.
Sonuç:
Laparoskopik sleeve gastrektomi, morbid obez çocuklar için uygulanabilir ve kolay bir cerrahi yöntemdir. Başarılı bir sonuç için multidisipliner bir yaklaşım ve yaşam boyu davranış terapisi en önemli faktörlerdir.
