Massive pneumoperitoneum: intestinal perforation? / Macklin effect?

Introduction

Massive pneumoperitoneum is the most significant sign of gastrointestinal perforation and requires urgent surgical intervention. However, pneumoperitoneum may also occur alongside pneumothorax and pneumomediastinum following aggressive mechanical ventilation. This situation presents a significant diagnostic dilemma in critically ill patients.

Case Presentation

A 3-year-old female patient diagnosed with B-cell acute lymphoblastic leukemia (B-ALL) was intubated at another center due to poor general condition and respiratory distress and was transferred to our center because she required ECMO. The patient, who was in poor general condition and presented with tense abdominal distension, had a plain abdominal X-ray that revealed widespread free air in the abdomen; a chest X-ray showed no pneumothorax. The patient, who was septic and pancytopenic, first had an intraperitoneal drain placed at the bedside, which drained only air. During exploratory surgery performed due to suspected intestinal perforation, the gastrointestinal tract was found to be intact, and there was no evidence of intraperitoneal contamination. Widespread air was observed in the retroperitoneal space.

Conclusions

Although pneumoperitoneum is frequently associated with intestinal perforation, it can rarely develop due to the Macklin effect at high ventilator pressures. In cases of massive pneumoperitoneum developing after mechanical ventilation, the lungs, pleural space, mediastinum, and subcutaneous tissues must be evaluated for the presence of free air.

Giriş

Masif pnömoperitoneum, gastrointestinal sistem perforasyonunun en önemli bulgusu olup, acil cerrahi girişim gerektirmektedir. Bununla birlikte, zorlu mekanik ventilasyon sonrası pnömotoraks ve pnömomediastinum ile birlikte pnömoperitoneum da görülebilir. Bu durum, kritik hastalarda önemli bir tanısal ikilem oluşturmaktadır.

Olgu Sunumu

B hücreli akut lenfoblastik lösemi (B-ALL) tanılı 3 yaşında kız hasta, genel durum bozukluğu ve solunum sıkıntısı nedeni ile başka bir merkezde entübe edilip, ECMO ihtiyacı nedeniyle merkezimize sevk edildi. Genel durumu bozuk ve gergin abdominal distansiyonu olan hastanın direkt batın grafisinde, batın içinde yaygın serbest hava saptandı, akciğer grafisinde pnömotoraks saptanmadı. Septik ve pansitopenik olan hastaya önce yatak başında intraperitoneal dren yerleştirildi ve sadece hava drenajı oldu. İntestinal perforasyon şüphesiyle yapılan eksplorasyonda gastrointestinal sistemin intakt olduğu ve intraperitoneal kontaminasyon olmadığı görüldü. Retroperitoneal alanda yaygın hava varlığı izlendi.

Sonuç

Pnömoperiton yüksek oranda intestinal perforasyon ile ilişkili olmakla beraber, nadiren yüksek ventilatör basınçlarında Macklin etkisine bağlı olarak da gelişebilir. Mekanik ventilasyon sonrası gelişen masif pnömoperitoneum olgularında mutlaka akciğerler, plevral alan, mediasten ve cilt altı dokular serbest hava varlığı açısından değerlendirilmelidir.
