Biomechanical Design and In Vitro Model of a Dynamic Spring System Providing Automated Continuous Traction (AITS) in Long-Gap Esophageal Atresia

Aim: Internal thoracoscopic static traction sutures used in the treatment of long-gap esophageal atresia (LSTEA) loosen rapidly and lose efficacy due to the viscoelastic nature of the esophagus. This study investigated the in vitro model, biomechanical testing, and efficacy of a dynamic spring system designed to eliminate this suture relaxation problem.
Methods: A dynamic spring system experimental group (n=3) was compared with a conventional suture technique control group (n=3) in an in vitro sheep esophageal model. In the experimental group, micromechanical spring mechanisms calibrated to generate different initial tension levels were integrated into each esophagus. To prevent the post-mortem tissues from drying out during the decay process, all specimens were humidified daily. Tension changes and net elongation amounts of the esophagi were prospectively measured on days 1, 2, and 3 in both groups. Strain testing was performed on both models. At the end of day 3, primary anastomosis was performed on all esophagi, followed by a burst pressure test. A tensile test was performed following anastomosis.
Results: Following the three-day observation period, all three esophageal specimens in the dynamic spring system group demonstrated a significantly more pronounced and higher reduction in tension (force attenuation) across all days compared to the conventional suture technique. Concurrent with this mechanical relaxation, significantly greater net elongation in esophageal length was achieved in the spring system group than in the control group. In the AITS group, it was associated with a higher strain test result. No significant difference was found in terms of burst pressure test results.
Conclusion: Our in vitro findings indicate that the AITS system absorbs early tissue relaxation by converting it into continuous traction. By overcoming tissue resistance more effectively, this system provides a higher rate of esophageal elongation in a much shorter time compared to the conventional technique. This designed dynamic model has the potential to clinically pioneer the development of next-generation continuous traction devices in LSTEA surgery.
Uzun Segment Özofagus Atrezisinde Otomatik Sürekli Traksiyon Sağlayan Dinamik Yay Sisteminin (AITS) Biyomekanik Tasarımı ve İn Vitro Modeli

Amaç: Uzun segment özofagus atrezisi (LSTEA) tedavisinde kullanılan internal torakoskopik statik traksiyon dikişleri, özofagusun viskoelastik yapısı nedeniyle hızla gevşemekte ve etkinliğini yitirmektedir. Bu çalışmada, dikiş gevşemesi problemini ortadan kaldıran dinamik bir yay sisteminin in vitro modeli, biyomekanik testleri ve etkinliği araştırılmıştır.
Yöntem: İn vitro sheep özofagus modelinde, dinamik yay sistemi uygulanan deney grubu (n=3) ile klasik dikiş tekniği uygulanan kontrol grubu (n=3) karşılaştırılmıştır. Deney grubundaki her bir özofagusa, başlangıçta farklı gerginlik seviyeleri üretecek şekilde kalibre edilmiş mikromekanik yay mekanizmaları entegre edilmiştir. Post-mortem dokuların canlılığını yitirmesi sürecinde kurumasını önlemek amacıyla, tüm numuneler günlük olarak nemlendirilmiştir. Her iki grupta da özofagusların 1., 2. ve 3. günlerdeki gerginlik değişimleri ve net uzama miktarları prospektif olarak ölçüldü. Her iki modelde de strain testi yapıldı. 3.günün sonunda tüm özofaguslara primer anastomoz yapıldı ve Burst Pressure testi uygulandı. Anastomoz sonrası tensile test yapıldı.
Bulgular: Üç günlük gözlem sonucunda, her üç özofagus numunesinde de tüm günlerde klasik dikiş tekniğine kıyasla dinamik yay sisteminde çok daha belirgin ve yüksek bir gerginlik azalması (kuvvet sönümlenmesi) kaydedilmiştir. Bu mekanik gevşemeye eş zamanlı olarak, yay sistemi uygulanan gruptaki özofagus boylarında, kontrol grubuna göre anlamlı derecede daha fazla net uzama elde edilmiştir. AITS grubunda daha yüksek strain test sonucuyla ilişkiliydi. Burst pressure açısından anlamlı fark saptanmadı.
Sonuç: İn vitro bulgularımız, AITS sisteminin erken doku gevşemesini sürekli traksiyona dönüştürerek absorbe ettiğini göstermektedir. Bu sistem, doku direncini daha efektif kırarak klasik tekniğe kıyasla çok daha kısa sürede ve daha yüksek oranda özofagus uzaması sağlamaktadır. Tasarlanan bu dinamik model, LSTEA cerrahisinde yeni nesil sürekli traksiyon cihazlarının geliştirilmesine klinik olarak öncülük edebilecek niteliktedir.
