Spontaneously Perforated Meckel’s Diverticulum in a Preterm Twin: A Rare Cause of Neonatal Pneumoperitoneum

1. Introduction
- Meckel’s diverticulum (MD) is the most common congenital abnormality of the gastrointestinal tract, resulting from the incomplete obliteration of the vitelline (omphalomesenteric) duct during the 5th–7th weeks of gestation.
- It typically follows the "Rule of 2s": 2% prevalence, 2:1 male-to-female ratio, and symptoms often manifesting before age 2.
- Neonatal symptomatic presentation is extremely rare, accounting for less than 20% of all pediatric MD cases.
- Perforation is a rare complication (seen in ~10% of symptomatic cases) and is especially infrequent in the preterm population.
2. Case Presentation
- Patient: Preterm male neonate (Twin II), born at 33+3 weeks via emergency cesarean section.
- Birth Weight: 1.826 kg. Apgar Scores: 7 and 9 (1 and 5 minutes).
- Initial Course: Developed early respiratory distress (tachypnea, retractions) requiring continuous positive airway pressure (CPAP).
- Clinical Deterioration: On the third day of life, the infant developed signs of sepsis and significant abdominal distension.
- Laboratory Findings: WBC 5.5 × 10⁹/L, CRP 5 mg/L, and Platelet count 301 × 10⁹/L.
- Note: The relatively low CRP and WBC are consistent with some reports of neonatal MD perforation, which may not initially show a pronounced inflammatory state compared to Necrotizing Enterocolitis (NEC).
6. Discussion and Conclusion
- Prognosis: The patient had an uneventful recovery. Early diagnosis and prompt surgical intervention are typically associated with a favorable prognosis in neonates with isolated MD perforation.
- Clinical Importance: MD should remain a differential diagnosis in any neonate presenting with a "surgical abdomen" and pneumoperitoneum, particularly when NEC is less likely due to early onset (within the first week) or normal bowel imaging elsewhere.
- Conclusion: This case underscores the importance of maintaining a high index of clinical suspicion for Meckel's diverticulum in the neonatal period to ensure timely, life-saving surgery

1. Giriş
- Meckel divertikülü (MD), 5–7. gebelik haftaları arasında vitellin (omfalomezenterik) kanalın tam olarak oblitere olmaması sonucu gelişen, gastrointestinal sistemin en sık görülen konjenital anomalisi olup gerçek bir divertiküldür.
- Klasik “2’ler Kuralı”na uyar: %2 prevalans, erkek/kız oranı 2:1, olguların çoğunda semptomların 2 yaşından önce ortaya çıkması.
- Yenidoğan döneminde semptomatik MD oldukça nadirdir ve pediatrik MD olgularının %20’sinden daha azını oluşturur.
- Perforasyon, semptomatik olguların yaklaşık %10’unda görülen bir komplikasyon olup, prematüre yenidoğanlarda son derece seyrektir.
2. Olgu Sunumu
- Hasta: 33+3 gebelik haftasında acil sezaryen ile doğan, erkek prematüre yenidoğan (İkiz II).
- Doğum Ağırlığı: 1.826 g; Apgar Skorları: 1. dakikada 7, 5. dakikada 9.
- Erken Dönem Seyir: Taşipne ve göğüs retraksiyonları ile seyreden solunum sıkıntısı nedeniyle CPAP tedavisi başlandı.
- Klinik Kötüleşme: Yaşamın 3. gününde karın distansiyonu ve sepsis bulguları gelişti.
- Laboratuvar Bulguları: Lökosit 5,5 × 10⁹/L, CRP 5 mg/L, trombosit 301 × 10⁹/L.
- CRP ve lökosit değerlerinin belirgin yüksek olmaması, literatürde de bildirildiği üzere, neonatal MD perforasyonunda NEK’e göre daha silik inflamatuvar yanıtla uyumludur.
6. Tartışma ve Sonuç
- Prognoz: Hastanın postoperatif seyri sorunsuz olup, erken cerrahi müdahale ile tam iyileşme sağlandı.
- Klinik Önemi: NEK düşünülmeyen veya NEK ile uyumsuz radyolojik/klinik bulguların olduğu, pnömoperitoneum saptanan her yenidoğanda Meckel divertikülü perforasyonu ayırıcı tanıda göz önünde bulundurulmalıdır.
- Sonuç: Bu olgu, neonatal dönemde nadir görülmesine rağmen Meckel divertikülüne yönelik yüksek klinik şüphenin, prematüre bebeklerde dahi zamanında ve hayat kurtarıcı cerrahi tedaviye olanak sağladığını vurgulamaktadır.
