Extrapulmonary sequestration incidentally detected in a patient with spontaneous pneumothorax

Introduction

Pulmonary sequestration is defined as abnormal, non-functional lung tissue supplied by abberant systemic arteries and not related to the bronchial system. Sequestrations are often asymptomatic and detected incidentally. In our case, we aim to share an extrapulmonary sequestration found during diagnostic thoracoscopy performed secondary to spontaneous pneumothorax.

Case

A 16-year-old male patient was referred to us from another center due to pneumothorax in the left lung that did not expand despite the presence of a chest tube. The patient underwent thoracoscopy because his lung did not expand despite long-term drainage. In the single-port video-assisted thoracoscopic surgery performed, the blebs located at the apex were excised, and upon observation of incidental extrapulmonary sequestration during exploration, the artery supplying the sequestration was ligated with stapler, and the sequatration was removed without enlarging the incision. The patient's chest tube was removed on day 4 after a postoperative X-ray showed lung expansion. The patient's postoperative follow-up was uneventful.

Conclusion

In spontaneous pneumothorax, even if the blebs or bullae causing the pneumothorax are removed, thoracoscopy, by providing adequate exploration, helps in diagnosing rare pulmonary sequestrations, and excising them is recommended because they may contain malignant masses, albeit rarely.

Spontan pnömotoraksa sekonder rastlanan ekstrapulmoner sekestrasyon

Giriş

Pulmoner sekestrasyon, bronşiyal sistem ile ilişkisi olmayan, abberant sistemik arterler tarafından beslenen, fonksiyon görmeyen anormal akciğer dokusu olarak tanımlanır. Sekestrasyonlar sıklıkla asemptomatiktir ve tesadüfen saptanır. Olgumuzda spontan pnömotoraksa sekonder yapılan tanısal torakoskopide rastlanan ekstrapulmoner sekestrasyonu paylaşmayı amaçladık.

Olgu

16 yaş erkek hasta dış merkezden göğüs tüpü olmasına rağmen ekspanse olmayan sol akciğerde pnömotoraks sebebi ile tarafımıza refere edildi. Hasta uzun süreli drenaja rağmen akciğerinin ekspanse olmaması sebebi ile tarafımızca torakoskopiye alındı. Yapılan tek port yardımlı video destekli torakoskopik cerrahide apekste yer alan bülleri eksize edildi ve eksplorasyonda insidental ekstrapulmoner sekestrasyon izlenmesi üzerine sekestrasyonu besleyen arter stapler ile bağlandıktan sonra kesi büyütülmeden sekestrasyon çıkartıldı. Postop grafisinde akciğerin ekspanse olduğu izlenen hastanın göğüs tüpü 4. gününde göğüs tüpü çekildi. Hastanın postop takibi sorunsuz idi.

Sonuç

Spontan pnömotoraksta pnömotoraksa sebep olan bleb veya büller çıkarılsa bile torakoskopi; yeterli eksplorasyon sağlaması sayesinde nadir görülen pulmoner sekestrasyonun tanı almasında yardımcı olur ve nadir de olsa içinde malign kitleler barındırabileceği için eksize edilmesi önerilir.
