Vacuum bell therapy for pectus excavatum: A single-center experience based on an objective treatment endpoint

Aim:

Pectus excavatum (PE) is the most common congenital chest wall deformity and may require treatment because of cardiac, respiratory, and psychosocial effects. Vacuum bell (VB) therapy is the most widely used non-invasive treatment. This study aimed to evaluate the outcomes of VB and to establish an objective treatment endpoint by determining the normal chest wall depth (CWD) in the Turkish pediatric population.

Methods:

Patients with PE treated with VB between June 1, 2024, and June 1, 2026, were retrospectively reviewed. CWD was measured using the Rod scale before treatment and at 3, 6, 9, and 12 months. To determine normal CWD, 30 age- and sex-matched children without PE were evaluated. The mean CWD of this reference group was defined as the target value for treatment success and discontinuation. Structural changes were monitored using lateral chest radiographs.

Results:

A total of 189 participants were included: 159 patients with PE and 30 controls. The mean CWD in the reference group was 4.7 ± 1.4 mm. In the PE group, mean CWD decreased from 20.0 ± 7.1 mm before treatment to 12.7 ± 7.0, 9.7 ± 7.0, 11.8 ± 6.6, and 12.3 ± 5.8 mm at 3, 6, 9, and 12 months, respectively. Treatment was discontinued after achieving a CWD of ≤5 mm in 28, 55, and 14 patients at 3, 6, and 9 months, respectively. Overall, 105 patients (66.3%) reached the target CWD by 12 months. Improvement was greatest during the first 9 months, with minimal additional benefit thereafter.

Conclusions:

Vacuum bell therapy is an effective treatment for children with pectus excavatum. A CWD of ≤5 mm, based on normative data from the Turkish pediatric population, may serve as a practical objective endpoint for treatment completion.

Amaç:

Pektus ekskavatum (PE), en sık görülen konjenital göğüs duvarı deformitesidir. Kardiyak, solunumsal ve psikososyal etkileri nedeniyle tedavi gerektirebilir. Son yıllarda non-invaziv tedavi yöntemleri ön plana çıkmış olup, bunlar arasında en yaygın kullanılan yöntem vakum bell (VB) tedavisidir. Bu çalışmada kliniğimizde PE nedeniyle VB tedavisi uygulanan hastaların tedavi sonuçlarının değerlendirilmesi ve Türk çocuk popülasyonunda normal göğüs duvarı derinliğinin (GDD) belirlenerek objektif bir tedavi sonlanım kriteri oluşturulması amaçlandı.

Yöntem:

1 Haziran 2024–1 Haziran 2026 tarihleri arasında VB uygulanan PE hastaları retrospektif olarak değerlendirildi. Tedavi başlangıcında ve tedavinin 3., 6., 9. ve 12. aylarında GDD Rod ölçeği ile ölçüldü. Ayrıca, Türk çocuk popülasyonunda normal GDD değerini belirlemek amacıyla yaş ve cinsiyet açısından uyumlu, PE bulunmayan 30 çocukta GDD ölçümü yapıldı. Bu grubun ortalama GDD değeri tedavi başarısı ve tedavinin sonlandırılması için hedef değer olarak kabul edildi. Kemik ve kıkırdak yapıdaki değişiklikler lateral akciğer grafileri ile takip edildi.

Bulgular:

Çalışmaya 159 PE hastası ile normal referans grubunu oluşturan 30 çocuk olmak üzere toplam 189 olgu dahil edildi. Referans grubunda (ortalama yaş: 11.0±0.9 yıl) ortalama GDD 4.7±1.4 mm olarak saptandı. PE grubunda (ortalama yaş: 10.3±3.7 yıl) tedavi öncesi ortalama GDD 20.0±7.1 mm iken, 3. ayda 12.7±7.0 mm (n=159), 6. ayda 9.7.9±7.0 mm (n=131), 9. ayda 11.8±6.6 mm (n=76) ve 12. ayda 12.3±5.8 mm (n=62) olarak bulundu. GDD'nin ≤5 mm olduğu 28 hastada tedavi 3. ayda, 55 hastada 6. ayda ve 14 hastada 9. ayda sonlandırıldı. 12. ayda hedef değere ulaşan hasta değeri 105 olarak bulundu ve hastaların %66.3'ünde tedavi başarıyla tamamlandı. GDD'deki düzelmenin ilk 9 ayda belirgin olduğu, 9. aydan sonra ise ilave kazanımın sınırlı kaldığı gözlendi.

Sonuç:

Çalışmamız, PE’li çocukların tedavisinde VB’nin etkin bir tedavi yöntemi olduğunu işaret etmektedir.
