Purse-String Technique for Pediatric Stoma Prolapse: A Minimally Invasive Approach Compatible with ERAS Principles

Aim
Stoma prolapse is one of the most common complications in childhood and may lead to serious problems such as mucosal edema, bleeding, obstruction, and strangulation. Currently, minimally invasive approaches have gained importance in line with the principles of Enhanced Recovery After Surgery (ERAS) due to their advantages, including earlier enteral feeding, shorter hospital stays, and lower morbidity. The aim of this study was to present our clinical experience with the purse-string suture technique for the management of stoma prolapse.
Methods
A total of 86 ostomies created at our institution between January 2022 and March 2026 were retrospectively reviewed. Stomal prolapse developed in 10 patients, and those who underwent the purse-string suture procedure were included in the study.
Results
A total of 10 patients were included in the study. Nine patients were male and one was female, with ages ranging from 9 months to 10 years. The primary diagnosis was Hirschsprung disease in eight patients, anorectal malformation in one patient, and colonic atresia in one patient. Seven patients had a colostomy and three had an ileostomy. Regarding stoma construction, a loop stoma had been created in eight patients and a divided stoma in two patients. Following reduction of the prolapse, all patients underwent a subcutaneous purse-string suture procedure using 1-0 polypropylene suture.
Oral feeding was initiated within 3 hours postoperatively in all patients. Five patients were discharged on postoperative day 1, four on postoperative day 2, and one on postoperative day 4. During follow-up, recurrent prolapse developed in one patient. Early postoperative stomal narrowing was observed in one patient and resolved following suture revision. One patient died due to severe comorbidities unrelated to the stoma. No additional surgical revision was required in the remaining patients.
Conclusions
The purse-string suture technique is a simple, minimally invasive, and low-morbidity method for the management of pediatric stoma prolapse. Owing to its ability to facilitate early enteral feeding and shorten hospital stay, it may be considered an effective bridge treatment option consistent with the principles of Enhanced Recovery After Surgery (ERAS). Our findings support the safety and feasibility of this technique in clinical practice.
Pediatrik Ostomi Prolapsusunda Purse-String tekniği: ERAS Prensipleriyle Uyumlu Minimal İnvaziv Yaklaşım

Amaç
Stoma prolapsusu; çocukluk çağında enterostomi sonrası sık görülen komplikasyonlardan biridir ve mukozal ödem, kanama, obstrüksiyon, strangülasyon gibi ciddi sorunlara yol açabilmektedir. Cerrahi gerektiren olgularda morbidite artmakta, özellikle küçük çocuklarda laparotomi gereksinimi önemli problemler oluşturmaktadır. Günümüzde minimal invaziv yaklaşımlar; erken enteral beslenme, kısa hastane yatışı, düşük morbidite sağlamaları nedeniyle ERAS(Enhanced Recovery After Surgery) prensipleri doğrultusunda önem kazanmaktadır. Bu çalışmada, ostomi prolapsusunda uygulanan purse-string sütür tekniğine ilişkin klinik deneyimimizin sunulması amaçlandı.
Yöntem
Kliniğimizde Ocak 2022–Mart 2026 tarihleri arasında açılan toplam 86 ostomi retrospektif olarak değerlendirildi. Hastaların 10’unda stomal prolapsus gelişti ve purse-string sütür uygulanan olgular çalışmaya dahil edildi.
Bulgular
Çalışmaya toplam 10 hasta dahil edildi. Hastaların 9’u erkek, 1’i kız olup yaşları 9 ay-10 yıl arasında değişmekteydi. Primer tanı 8 hastada Hirschsprung hastalığı, 1 hastada anal atrezi ve 1 hastada kolonik atrezi idi. 7 hastada kolostomi, 3 hastada ileostomi mevcuttu. Stoma tekniği olarak 8 hastada loop, 2 hastada diverjan stoma uygulanmıştı. Tüm hastalara prolapsus redüksiyonu sonrası 1/0 prolen sütür ile subkutan purse-string sütür tekniği uygulandı.
Tüm hastalarda işlem sonrası oral beslenmeye postoperatif 3 saat içinde başlandı. Hastaların 5’i postoperatif 1.günde, 4’ü 2.günde ve 1’i 4.günde taburcu edildi. Takiplerde 1 hastada nüks prolapsus gelişti. Bir hastada erken dönemde stomal pasaj darlığı izlendi ve sütür revizyonu sonrası düzeldi. Bir hasta ciddi komorbiditeler nedeniyle, stoma ilişkili olmayan nedenlerle kaybedildi. Diğer hastalarda ek cerrahi revizyon gereksinimi olmadı.
Sonuç
Purse-string sütür tekniği, pediatrik ostomi prolapsusunda kolay uygulanabilir, minimal invaziv ve düşük morbiditeli bir yöntemdir. Erken enteral beslenme ve kısa hastane yatışı sağlaması nedeniyle ERAS prensipleriyle uyumlu etkili bir köprü tedavi seçeneği olarak değerlendirilebilir. Bulgularımız yöntemin güvenli ve uygulanabilir olduğunu desteklemektedir.
