Development of a low-cost and realistic thoracoscopic lung resection model for minimally invasive surgical training

Aim: The aim of this study was to develop a thoracoscopic lung resection model using a fresh sheep tissue obtained from a slaughterhouse.
Methods: A thoracoscopic lung resection model was established using fresh sheep tissues obtained from a slaughterhouse. The specimen was positioned to simulate a standard thoracoscopic surgical setup. Conventional thoracoscopic instruments were used. Continuous lung ventilation was provided throughout the procedure by connecting a bicycle pump to the trachea, thereby enhancing the physiological realism and dynamic operative conditions of the model.
Results: The thoracoscopic lung resection model was developed through a structured six-step training protocol. Step 1: a pneumothorax was created to establish an adequate working space and simulate intrathoracic conditions. Step 2: ligament / any adhesions were resected to allow appropriate mobilization of thoracic structures. Step 3: lung ventilation was simulated by connecting a bicycle pump to the trachea, enabling dynamic lung inflation and deflation throughout the procedure and enhancing physiological realism. Step 4: the bronchus was identified and suspended to facilitate precise dissection and safe handling. Step 5: the bronchus was ligated and transected under direct thoracoscopic visualization. Step 6: a leak test was performed. The per-session cost of the model was approximately $10 after the initial setup.
Conclusions: This model was designed as a low-cost, realistic, and easily accessible training model for thoracoscopic lung resection.

Amaç: Bu çalışmanın amacı, mezbahadan temin edilen taze koyun dokusu kullanılarak torakoskopik akciğer rezeksiyonu için bir eğitim modeli geliştirilmesidir.
Yöntem: Mezbahadan temin edilen taze koyun dokusu kullanılarak torakoskopik akciğer rezeksiyonu modeli oluşturuldu. Doku örneği, standart torakoskopik cerrahi düzeni taklit edecek şekilde konumlandırıldı. İşlem sırasında konvansiyonel torakoskopik cerrahi aletler kullanıldı. Trakeaya bir bisiklet pompası bağlanarak işlem boyunca sürekli akciğer ventilasyonu sağlandı; böylece modelin fizyolojik gerçekçiliği ve dinamik cerrahi koşullar arttırıldı.
Bulgular: Torakoskopik akciğer rezeksiyonu modeli, yapılandırılmış altı basamaklı bir eğitim protokolü şeklinde geliştirildi. Birinci basamakta, yeterli çalışma alanı oluşturmak ve intratorasik koşulları taklit etmek amacıyla pnömotoraks oluşturuldu. İkinci basamakta, torasik yapıların uygun şekilde mobilizasyonunu sağlamak için pulmoner ligaman ve adezyonlar rezeke edildi. Üçüncü basamakta, trakeaya bir bisiklet pompası bağlanarak akciğer ventilasyonu simüle edildi. Böylece işlem boyunca dinamik akciğer inflasyonu ve deflasyonu sağlanarak fizyolojik gerçekçilik arttırıldı. Dördüncü basamakta, diseksiyonu ve güvenli manipülasyonu kolaylaştırmak amacıyla bronş tanımlandı ve askıya alındı. Beşinci basamakta, bronş doğrudan torakoskopik görüntüleme altında bağlandı ve kesildi. Altıncı basamakta ise kaçak testi gerçekleştirildi. Konvansiyonel torakoskopi ekipmanlarının kurulumunun ardından modelin seans başına maliyeti yaklaşık 10 ABD doları idi.
Sonuç: Bu model, torakoskopik akciğer rezeksiyon eğitimi için düşük maliyetli, gerçekçi ve kolay erişilebilir bir eğitim modeli olarak düşünülebilir.
